The cutoff wheel fractured during grinding, and a fragment struck the employee's right arm, causing a laceration that required sutures to close. The disc had the proper RPM rating for the grinder, and the employee was wearing all necessary PPE at the time of the incident.

Mostrando los 12EE was pushing dirt off a coal fender to bury warm coal when the fender gave out and the dozer slide off sideways. The dozer overturned landing upright on the tracks. EE sustained a 1 inch laceration on their scalp requiring a suture to close.

Employee was raising and tamping #1 track. Employee was resetting track jack and a rock got lodged in the release. His left hand was on the top of the jack body. When the rock was removed, the jack released, dropped, pinching his left hand and cutting his finger.
