Injured employee was holding a truck part while another employee used Sawzall to cut the mounting bolts. The sawblade slipped and cut employee across back of Left hand, through gloves. The employee was seen by on-site medic and referred to further care. EE received suture to close the laceration.

Employee reports EE was drilling a reamer hole when the steel shanked. EE was contacted by an object in the shoulder but thought nothing of it. Upon changing out EE noticed a small laceration that required one suture to close.
Employee stated while operating a shot-crete hose that it bucked from a partial plug. Employee braced self but in process of maintaining control of the hose EE felt a pop in left knee followed by mild pain. Seen site medic, went to doctor and was issued a one day rest restriction making this reportable.
Employee was pinning up a wire panel in the back as part of the bolting cycle, while adjusting the wire one of the mickey bolts used to hold the wire up fell out of the back and contacted the employee below the left eye resulting in a laceration, sutures were required & given full work release making this a medical reportable incident.
Miner was drilling a raise round on an Alimak raise climber. Miner noticed a stoper falling over on miner's right side and reached to stop the stoper from falling. The falling stoper struck the employee on the right arm as it fell to the Alimak deck.
Employee was prepping the engine compartment head of LHD for removal. While removing the last bolt, the hood accidentally fell striking the employee on the finger causing injury.
Employee cut left middle finger tip off while using an electric skill saw cutting a piece of plywood standing vertical when saw blade made contact with finger.
Employee bolting heading on CMAC Bolter & was collaring a hole in the back a piece of rock broke, as it was falling employee stepped off the bolter deck to avoid being struck. In doing so they felt pain in lower back, reported to site medic, referred to ER, x-rays were negative, referred to occupational health, restrictions were issued for low back strain.
While manually removing a 6" Reamer and Hex Rod due to completion of the drill hole, the injured employee's left pointer finger was smashed between the Hex Rod and a guard in place over the jack controls of the drill.
As the drill crew was changing mast position to set up on correct azimuth for a downhole, a bolt on the crowd cylinder broke and a section of the bolt impacted the Driller in the left eye.
Employee was attempting to thread a bolt driver into the striker bar on Sandvik Bolter turret that was positioned horizontally and still powered up. In the process EE's right hand & forearm was pulled into the turret as the drill steel side of turret was still rotating resulting in an amputation of right index finger at the 2nd joint & a fractured right arm.
Employee was bolting with a CMAC bolter. As EE was inserting a bolt driver steel into the chuck of the machine, EE had placed the right gloved hand below the steel taper that holds it in the machine resulting in a pinching laceration to EE's palm area below the pinky finger. Two sutures were required and a full work release was issued.
Miner was mucking the face after blast and hit a toe in the sill which caused the mucker to stop abruptly, miner hit their head in the cab.
Employee using an airbag to lift the front of a de-railed ore car when the airbag underload popped out from placement area contacting employee knocking EE backwards into a concrete barrier. Seen site medic, referred to the ER, x rays negative, referred to a occupational health provider. EE was issued restrictions for mid back muscle bruising/strain.
On 2/4/23 an employee turned on a fan after shutting it off to use the underground phone. Upon starting fan backup some dust came out of the fan where EE felt a foreign body enter right eye. EE flushed eye & didn't report incident to supervisor that shift. 2/5/23, EE's eye was irritated so EE went to physician & had metal flake removed & was given restrictions.
Employee attempting to get drill unstuck, they placed a bundle of Mickey bolts under the outrigger jack, partner deployed the jack, as pressure from jack was applied bundles metal bands failed resulting in a bolt to contact the employees left gloved hand resulting in lacerations to #2 & 3 fingers sutures were required, full work release given making this medical reportable
Employee drilling a Backstope panel with longhole drill on 1-7-23 when water & grit impacted EE's face resulting in foreign body to left eye. Employee washed it out & did not report it. On 1-9-23 at end of shift EE reported to supervisor due to irritation, seen site medic, referred ophthalmologist to remove foreign body was issued a full work release making this medical reportable.
Employee was using a MAG drill to drill holes into a structural beam while on the same beam in a congested area in the Ore Building. After the employee drilled the 3 holes, the drill bit failed while drilling the fourth and final hole. Resulted in the drill bit breaking, and right hand getting caught in-between the drill and scaffold leg after a sudden jolt from the MAG drill.
Employee was bolting up a round utilizing a jackleg when EE was contacted by small rock causing laceration to lip. The laceration required sutures to close and the employee was issued a full release.
Employee was using the teeth on the excavator bucket to pull on the stump/root ball of a large tree to get it to level ground so it could be limbed and cut into manageable lengths for hauling. The bucket of the excavator slipped under heavy load and part of the root ball with dirt and rocks shot into the glass of the excavator. The debris struck employe in the left shoulder area.
Employee was dumping a bucket of muck in a muck bay & as EE was lowering the loader bucket it caught a threaded rebar bolt on rib & pulled the bolt plate & nut off the bolt resulting in the bolt & plate to contact EE's left cheek resulting in a laceration. Seen site medic, referred to ER, Sutures required & full release issued making this a medical reportable.
Employee was using a CMAC to bolt round. EE twisted and strained their back as EE was attempting to remove the steel from the drill hole. Employee was evaluated by on site medic and referred to ER. EE was issued restrictions by the ER.
While changing drill steel in a CMAC the employee pinched left middle finger tip between drill steel and drifter. Employee reported incident was evaluated by medic and transported to St Vincent Hospital in Billings for further evaluation. X-rays were positive for a fracture to EE's left middle finger tip.
Employee was stabilizing the Hydraulic Torqueing tool while also activating the tool to a point where it would apply pressure to the reactionary arm. When activated, the reactionary arm pinched the employee's hand causing injury to the ring finger of the left hand. The injury required 5 sutures.
Employee was utilizing a 4 yard mucker to tram muck down a ramp. The mucker contacted the rib with the cab of the mucker. The impacted caused the employee to strike head on cab resulting in a laceration to left side. Employee was evaluated at a clinic and required 4 staples to close laceration and issued a full release.
During the bolting cycle the employee had just finished driving a bolt when employee released the air pressure on the jackleg machine to release the leg extension and as employee was lowering the machine, the back of employee's left upper arm made contact with the corner of an installed bolt plate on the rib resulting in a puncture wound to left triceps area of upper arm
Cutting 3x10 lagging with a pneumatic chainsaw employee lost EE's footing allowing saw to make contact with EE's left leg causing a laceration that required sutures.
Employee was moving a mechanical bolter into the heading when the brakes setup on the bolter causing the employee to contact the canopy resulting in a laceration to the lower lip and a chipped tooth. Employee received sutures for the laceration and repairs to chipped tooth. A full work release was given, making this a medical reportable.
The miner III trainee was utilizing a jackleg to install a low bolt. The trainee was applying pressure to the cob to keep the leg from pushing the machine up. The crows foot of the jackleg kicked out causing the machine to fall pinching the trainee's index finger between the cob and the leg. The incident resulted in a laceration requiring sutures to close, full release issued.
Mill Operator was attempting to unplug a circuit with an air-lance when the fittings contacted structural steel and separated from the compressed air line. While the operator was attempting to shut off the air, the compressed air hose disconnected from the whip-check and struck the operator in the back of the head causing a laceration. Employee returned to work the following day.
Miner jackleg bolting 82-655 +100W production stope. Drilling into rock and rock fell. Jackleg machine pitched sideways pinching the miners right little finger between drill and right rib causing a laceration (5 sutures) and fracture to the tip of right little finger.
The employee was operating a jackleg when EE was contacted in the head while wearing a hardhat. EE was seen by the medics and transported to the Medical Center where EE was diagnosed with a concussion. EE was admitted for observation and not given a release.
The employee was operating a jackleg machine and a finger was pinched in-between the machine and the leg. This caused a fracture to the finger tip and a laceration requiring stitches. Employee was wearing gloves but not impact gloves.
Employee using a battery powered impact drill/socket attachment to remove a fuel filter housing bracket. When the nut loosened the bracket started to spin & the edge contacted EE's gloved right index finger resulting in a laceration. Sutures were required, a full release was issued making this a medical reportable incident.
Employee was installing a flywheel/converter in a loader. When assembly was in place EE used EE's gloved middle finger to feel for bolt hole alignment, at the same time EE bumped the air driven ratchet used to lift the assembly in place causing it to move resulting in right middle finger to be pinched. Sutures were required & full release issued making this a reportable incident.
Employee was utilizing an air powered impact tool to tighten an anchor bolt. The bolt broke causing the plate, bolt, or tool to fall striking the employee in the face. The contact caused a small laceration to EE's cheek. EE was seen by medic and referred to PMC for sutures. Issued a full release.
On 3/9/21 employee was on a bolter when the sound windshield moved contacting hardhat jarring head and neck. Was seen at ER, CT scan done, & full release issued. Returned to work 1st shift back 3/16/21 & on 3/17/21 they informed the company they had seen their own doctor & taken off work for four weeks to do physical therapy making this reportable on 3/17/21.
Driller was adding a reamer to the centralizer while the drill was in the "Helac Under" position. The rod and striker got into a bind causing the rod to kick out and pinch Driller's left thumb between the rod and slide resulting in a distal fracture to left thumb.
Employee was using a CMAC to install a bolt when a finger was contacted by the drifter. Employee was attempting to line up drifter and driver. Employee was seen by Medic and referred to ED where restrictions were issued for ligament damage to finger.
While using a CMAC bolter employee was in the process of placing the split set bolt driver into the machine chuck when EE pinched EE's gloved right palm between the driver steel and the machine causing an avulsion. Seen site medic referred to ER, then a physician were restrictions were issued.
Stinger Drill Rig operators Kevlar sleeve caught the feed control lever and energized the drifter which moved and pinched the operators right hand between the shank and the drill rod.
A driller was manual handling the tube assembly when the tube unexpectedly stopped. The force that they were applying caused the spear head to lacerate the driller's hand. This resulted in six stitches. The employee received no work restrictions (released to full duty).
A miner was pulling off a driven bolt in the rib and as they did the jackleg pivoted foreword and pinched their left ring finger between the machine and leg causing a laceration. Saw site medic, referred to the ER, x-rays were negative, 3 sutures were required, no work restrictions, making this a medical reportable injury.
Employee was applying shot-crete to the rib when the nozzle end started to plug. EE's partner was attempting to shut down the air to the pump when the nozzle separated from the hose causing the hose to whip the employee towards the opposite rib injuring EE's right leg. Seen by site medic, referred to ER, x-rays were positive for a fracture.
Employee using a powered drill to loosen some overhead bolts on the Tunnel Boring Machine on 6/18 when the drill spun out of their grip & contacted right fourth finger. Was seen by the site medic who advised it should be x-rayed, employee refused further care. On 6/21 they saw a doctor & x-rays were positive for a fracture on right fourth finger, restrictions were issued.
Employee was cleaning a piece of metal with a hand held angle grinder with a wire wheel attachment & as EE worked the grinder back and forth on the metal surface the wire wheel caught a headlamp strap hanging out of EE's pant pocket and pulled the grinder into EE's groin area. Seen site medic, referred to ER, restrictions were issued.
Employee was installing a 4" to 1" header onto a 4" airline. When energizing header, it came off and employee fell off of ladder to the ground.
On 12/16/17 employee operating jackleg machine after drilling a hole ee released leg pressure & as machine was coming out of hole it started to spin catching left little finger between throttle & muffler of machine. Seen at the ER, X-rays were negative. On 12/26/2017 was seen again by physician diagnosed with mallet finger, restrictions were issued.
Employee was using a jackleg machine to retrieve a stuck 2 foot split bolt driver steel in the back. When the driver broke free the dolly fell off the steel contacting employees upper lip causing a laceration, sutures were required, full release was issued making this a medical reportable.
