Coming down ladder from scale house and knee buckled and fell backwards.

Employee loosened radiator cap to check coolant level and burned his right hand/wrist from hot pressurized coolant.

Lifting a belly pan back onto haul truck and employee felt sharp pain in his lower abdomen.

Employee was walking to van to get dunnage to load equipment at pit and stepped on uneven ground, twisted and broke left ankle.

Croell Redi-Mix employee, was changing a track and rollers on dozer and when lying underneath trying to reposition roller he twisted his body and felt pain in lower back and tail bone.

EE had just finished installing new drive belts on feeder on 08IC01. He was sitting on a angle iron support frame handing tools to another employee when he slipped off the angle iron striking a frame support tubing with his left knee. Employee was helped out and transported by the supt. To Speaefish Regional Hospital where he was treated and released.

Croell employee was coiling electric cord and strained his right shoulder.

While the employee was pulling on wires with pliers, slipped and cut left wrist.

Injury was climbing up into the loader and hit the ladder with his left knee. He did not report the injury to us as a workplace injury until 05/26/09. This is a questionable work comp case because of conflicting information to the doctor and employer so is under investigation. I will update you when the case is denied or confirmed as a workplace injury.

Working on dredge, anitfreeze splattered and burned both wrists and elbow.

Employee injured arm & back while loading utility vehicle onto pickup.

Employee was changing screens at Wasta Pit and pulled muscle. Went to doctor and is a possible hernia. Going back to doctor on 08/07/2008 to confirm. Questionable. Off work from date of injury until hernia is confirmed and if surgery is needed.

He was dumping a haul truck and it tipped on side and he injured his right shoulder.

Climbing off loader slipped and fell off and injured right arm

Employee was lifting a rock at pit and felt a sharp pain in back. Went to doctor on April 30, 2008. Do not know restrictions as of yet.

The employee was prying a drain plug & pry bar slipped out & smashed his left hand into the frame of wash plant. Employee received medical. Will return to doctor Monday, December 3rd for x rays to diagnose fracture vs contusion. Is on restrictions and is working in the limits of the restrictions.

Bumped leg on equipment

Emloyee was welding with proper PPE and a flash burnt his eyes. Went to the emergency room and was restricted from coming to work until follow-up appointment.

Employee was getting into haul truck and the wind blew the door shut on left thumb resulting in a laceration.

The employee was stepping off truck on uneven ground after loading and felt a sharp pain in the lower abdominal area. He went to the doctor and was diagnosed with a pulled abdominal muscle. He was released to full duty on 08/30/06. Employee was released from medical on 08/29/06. This employee does not work in the pit.

Taking bucket off excavator and smashed 2 fingers on left hand. Six stitches in one finger and cracked bone on tip of finger on same hand.

Was pumping up fuel into Gen Set on 05/23/06 when he felt a pain and heard a pop. Went to doctor later that afternoon and was diagnosed with a pulled groin muscle. EE was given a work release on 05/24/06 and released for full duty on 05/25/06.

Walking on catwalk, slipped and twisted ankle.

He got a large piece of coarse sand in right eye which needed medical attention. Employee was wearing safety glasses.

While running gradation for sand and gravel samples strained her back. This mine was not operational. Only use trailer for sampling purposes. Went to doctor and had a work release until 3-31-05. Was released on 3-31-05 with restrictions of wt lifting equal or less than 25 pounds until recheck on 4-11-05.

Doing maintenance, fell off wash plant & landed on left arm and left leg.

USING A WRENCH TO LOOSEN BOLTS ON A 50 HP MOTOR, FELT SHARP PAIN IN LEFT SHOULDER.
