Employee was cleaning a screw conveyor. Employee had just returned to conveyor after taking a break to warm up. Employee collapsed and was unresponsive. 911 call was made & CPR was started and continued until EMT's took over.

Mostrando los 7Employee was maneuvering around under a haul truck during an inspection and twisted/moved just right to exacerbate a pre-existing hernia condition. (Employee planned on having the hernia operated on in November 2017). EE finished shift and worked 2 hours, went to doctor after.
Left Ankle Sprain. Causation; unknown Employee was working on a D10T and noticed EE's ankle was starting to hurt. No specific incident. Pain worsened and reported to EE's supervisor next morning. Did not receive medical treatment until 11-13-16. In which was put on restricted duty.
Employee sat down for his morning break, when he developed stroke symptoms. Another employee called 911 and the employee was transported to the local hospital. The employee was diagnosed with 100% blockage in his artery in the neck and is undergoing angioplasty surgery.
Employee was running the dozer and had an apparent heart attack and dozer traveled 715 feet into a pond and submerged the dozer. Employee was extracted from the dozer shortly after entering the water and CPR was administered. He was taken by ambulance to the local hospital where he was pronounced dead.
Employee was remotely monitoring operation of crush/screen operation from a sitting position inside control room trailer. Sharp pains felt in left side of chest. Supervisor called and first aid was given assisted by MSHA personnel who were on property at that time. Transported by ambulance to hospital. Released same day return to full duty. Diagnosis incomplete.
WALKING FROM WORK SITE TO SHOP. DIDN'T FEEL ANYTHING HIT EYE. HE WAS WEARING SAFETY GLASSES. EYE FELT IRRITATED AT 6:00PM DROVE HOME IN A OPEN TOP BRONCO. EYE FELT BURNING. FLUSH WITH WATE R AND SALINE AT HOME. WORKED SUNDAY AND MONDAY. EYE STILL IRRITATED WENT TO DR ON 7-12 FOUND PARTICLE OF METAL AND REMOVED.
