Employee got in truck and started to leave going to Dukes Mine. Minutes later EE began to feel jittery and dizzy and turned around. EE's coworker went to check on EE and saw that the EE was pale in color and nauseated. The decision was made to take EE to the ER.

Mostrando los 23IW and co-worker were returning from lunch and headed back to maintenance shop on the golfcart when the IW collapsed and became unresponsive. The Co-worker called 911 and started CPR. The Co-worker was immediately assisted by other co-workers with first aid/CPR efforts until EMS arrived. EMS arrived and transported IW to a Medical Center.
The employee operated a loader that had not been operated in about 3 months. The employee noticed a spider bite on right arm when employee was showering after work. Bite was deemed to be from a brown recluse.
4 QC IW came down with symptoms of COVID-19 on 7-30-21 and all tested positive. It is unknown who they caught it from, but the employees worked in close proximity with one another while not wearing PPE. 1st day of work missed 8-2-21

Employee was exiting the cab of Excavator 349, the wind blew the door shut on left hand
The employee was standing at the work table weighing clay. When they turned to the right, knees buckled and the employee fell to their knees. The employee complained of knee and back pain.
The employee was trying to loosen a manway bolt on a tankcar when they felt a sharp pain in groin area. The employee did not report the incident until 10/19/20 but did not request to see a doctor until the 20th. EE missed the first shift on 10/23/20 but there are conflicting diagnoses varying from a mass to a hernia.
EMPLOYEE GOING UP LONG STAIRWAY TO TOP OF TANK TO CHECK LEVEL AND EXPERIENCED A POPPING OF LEFT KNEE. THIS ACCIDENT HAPPENED ON 4/25/2018 BUT EMPLOYEE DID NOT GO TO DOCTOR UNTIL TODAY 5/09/2018.
EMPLOYEE HAD BEEN LETTING DOWN TRAIN CARS AND WAS WALKING BACK DOWN THE TRACKS - EE TURNED TO LOOK BACK AND FELT A POP IN THE LEFT KNEE. EE IS AWAITING AN ORTHOPEDIC APPOINTMENT TO DETERMINE EXTENT OF DAMAGE. EMPLOYEE IS WORKING AT A DIFFERENT JOB POSITION ON RESTRICTIVE DUTY.
EMPLOYEE REPORTED TO WORK AND STARTED FEELING WEAK AND LOSING BLOOD. 911 WAS CALLED AND EE WAS TAKEN TO HOSPITAL BY AMBULANCE. THIS IS NOT DUE TO AN INJURY OR ACCIDENT. THIS IS A MEDICAL CONDITION. MSHA 800 NUMBER WAS CALLED AND THIS WAS INVESTIGATED BY INSPECTOR OF THE MACON, GA FIELD OFFICE.
Driver felt sick and was short of breath. EE called 911, and an ambulance picked EE up and took EE to the ER.
Root Cause: It was determined that this was a pre-existing health issue and not a work related issue. Employee had a artery blockage. Employee did not pass away on project site. Employee passed away at the hospital. MSHA was notified immediately on November 6, 2014.
Employee was spraying coal tar on inside of tank when a co worker discovered he had collapsed from unknown reasons.
EMPLOYEE COMPLAINING OF CHEST PAINS AND FEELING SICK AND COLLAPSED. APPARENT HEART ATTACK.
Employee was sitting in air-conditioned controlled room after experiencing chest pains. THIS WAS REPORTED TO MSHA AS SOON AS WE KNEW CONDITION WAS RELEVANT TO CALL.
EMPLOYEE WAS FINISHING CEMENT WORK, WHEN A GUST OF WIND BLEW TRASH INTO RIGHT EYE, CAUSING IRRITATION.
Employee had been cutting wood with Skil saw earlier and some sawdust was on clothing. A gust of wind blew debris in employee's left eye. The employee was wearing safety glasses at time of occurrence.
EMPLOYEE WORKING TO PALLETIZE FILTER CLOTHS WHEN HE NOTICED HE HAD BEEN BITTEN BY A TICK.
This is not an accident. Emp was working with supervisor welding, wearing all PPE, when supervisor noticed emp's coloring not right. Supervisor told emp to take off PPE and sit down. Supervisor noticed emp's condition did not improve & proceeded to take emp to local EMR & he was diagnosed as having a heart attack.
Driver was picking up a container of clay to take to port. After he hooked truck to container and was pulling away he noticed container had come loose and fell on landing gear. Other j. driver was assiting him by backing his truck under side of container to lift it up to enable driver to rehook container. The second driver noticed that EE had fell to ground. cpr but no avilable.
Employee was walking to loader to fill feeder hoppers when he felt stinging sensation on upper left arm. He lifted shirt sleeve and saw brown spider. He killed spider. The next day he still had stinging sensation and a small lump where spider had been.
