Employee was attempting to ascend a ladder to the cab of a CAT 777 haul truck, when he slipped and fell on his back. Employee was in pain and tolerated it without reporting this original injury. Employee claims to have aggravated the injury on 6/11/09 while trying to bang a pin from a bucket of a CAT 365 excavator. The injury was not reported to the employer timely.
Employee was trying to turn over a very heavy chain, when he felt a "pop" in his back. This was dismissed as a minor pulled muscle. When trying to walk up a ladder to the dragline, his legs gave out, resulting in a fall from the ladder.
