Employee was working off of a ladder and his boots were muddy causing him to lose traction and slip. He fell off of the ladder and landed on his elbow resulting in two fractured bones.

Walking to mine office, slipped and fell on ice.

Employee was exiting a 930 loader, when he lost his footing and fell off of the steps to the ground, landing on his right knee and bending it back under his body.

EE was pulling belt with channel locks and changed locks, slipped and EE fell into rib and hurt right shoulder. EE had to have surgery on 12/8/08. Returned to work on light duty 12/9/08.

While cutting a piece of belt to make a splice, the utility knife blade came out of the knife, causing EE to fall forward. When EE attempted to catch himself, he accidentally fell forward on the knife blade that was still stuck in the belt, lacerating his left hand which required several sutures. On July 9, 2008 an employee was crossing the conveyor chain on the miner when he slipped and twisted his right knee. Due to complications the employee started missing work on October 10, 2008.

Employee slipped and fell in muddy conditions resulting in a fracture to a bone in his right hand.

An employee of Superior Steam Cleaning & Sandblasting was on a small section of roof. He was removing sheeting on the side of East Gulf Prep Plant. As he was tying off his safety harness he slipped and fell though a skylight. He fell and cut his head on the right side.

The injured was walking around the bucket of the parked endloader in order to get in the machine when his right foot slipped on a patch of ice in the roadway and his left foot remained stationary. This caused his left foot/leg to twist and resulted in his full body weight to shift to his left foot/leg causing a fracture just above the ankle.

Employee was carrying a bag of rock dust when he slipped and fell to the mine floor, hitting on his right hip, knee, and thigh. Due to complications He started missing work on January 21, 2008.

While crossing the belt at the #2 tailpiece employee's right foot slipped off the tailpiece causing him to twist his right knee. He continued to work until the day of his surgery.

Employee was pulling a water hose down a set of steps in the sample building when he slipped and fell hitting the steps with his butt. Due to complications he started missing work on August 14, 2007.

Employee stepped over a roof bolter cable and knee gave away.

Employee was cutting belt with a knife when he fell and accidentally cut his right thigh. Wound required staples to close.

Employee was climbing down from belt crossover at No. 5 drive and his foot slipped. This is being reported to abate violation No. 7436824
