Mostrando los 27Employee missed a step while climbing off the back of EE's mechanic's truck. EE fell to the ground, hitting EE's elbow. X-rays showed EE fractured EE's elbow.
Employee was working on a mud pump, slipped in the mud and fell on left shoulder. Fracture to upper end of left humerus.

EMPLOYEE WAS CHANGING SCREENS ON A CONE CRUSHER. AS HE THREW AN OLD SCREEN OFF THE SIDE, IT SNAGGED THE ZIPPER PULL ON HIS VEST AND PULLED HIM OFF THE SIDE OF THE DECK. HE WAS NOT WEARING HIS FALL PROTECTION. HE FELL APPROXIMATELY 13' TO THE GROUND. HE WAS DIAGNOSED WITH A BROKEN LEFT ARM AND A RUPTURED SPLEEN. HE WILL MISS 10-20 DAYS THEN RETURN WITH "NO LIFTING" RESTRICTION.
