Employee finished EE's shift that day and continued to work. On 2/4/26 employee went for further medical evaluation and was put off work.

After being professionally examined, it was diagnosed as an acute displaced left femoral neck fracture and surgery was required to correct.

A small weld was still in-tacked & as the employee was descending the ladder the weld came loose making the plate spin hitting the ladder & making the EE fall.
Employee was hanging a Ventilation tube when EE slipped twisting ankle. The ankle was sprained and EE was not able to return to normal job the following shift.
