Mostrando los 23Employee was servicing equipment, EE started to descend the ladder. When EE got to the last rung, EE's left foot going to the ground and as EE was letting go of the rails with EE's hands and stepping down with EE's right foot...EE caught EE's boot solidly on the last step which caused EE to fall back. EE's body twisted as EE was falling, landing EE on EE's right hip and back.
Employee was performing maintenance activities inside the Barge Jaw Crusher. EE was standing on a piece of plywood that shifted, causing EE to lose footing and fall into the jaw area. EE was wearing fall protection and SRL (Self-retracting lanyard) that stopped EE from falling farther into the jaw crusher.
Employee was shutting down operations for the day due to inclement weather and was walking around a Connex when EE slipped on a slight slope, rolled ankle and fell down.
EE was fueling an excavator when EE slipped. EE stated as EE slipped EE's hard hat went over EE's eyes. EE fell to the ground landing on EE's feet but didn't have the chance to grab anything to soften the fall.
Designated parking place had ice and was uneven
Employee lost sight of step and was in a hurry exiting the skid steer. Right shoulder strain.
Employee was picking up tickets 200ft from the scale house when employee slipped on ice, braced fall with right arm and broke right wrist.
EMPLOYEE STEPPED OFF EQUIPMENT, LEFT LEG FIRST, FELT PAIN IN LEFT HIP.
Tripped over hose and rolled left foot.
EMPLOYEE WAS WALKING FROM ONE SIDE OF PLANT TO THE OTHER TO ACCESS THE FEED TUNNEL. EMPLOYEE STEPPED FROM CEMENT GROUND TO DIRT GROUND, ROLLING RIGHT ANKLE AS HE STEPPED ON TO DIFFERENT SURFACE.
EE was going to shovel sand from underneath the tailpulley of the sand conveyor. He thought the ground would be harder and when he went to push the shovel in, it sank into the wet sand like water. EE lost balance, falling forward and caught the meat of his hand between his R thumb & forefinger on the guard around the tailpulley, cutting it open and requiring 4 stitches.
Employee was changing the screens at the crusher when he lost his balance and the screen fell back on his left kneecap. He called our occupational nurse who advised him to ice and take ibuprofen. Two days later the nurse advised him to see a doctor. He went to the clinic on 1/24/08 and was diagnosed with a knee contusion.
Employee was carrying a wrench to the wash plant. He had turned near the pump. Employee's foot slipped on ice.
Foot slipped off step of excavator lost grip and fell to the ground hurting his right shoulder.
Employee was walking from the shop to the crusher when he slipped and fell on snow covered ice causing his right knee to twist.
EE HAD CLIMBED UP ON TOP OF A PULLEY V-GUARD TO INSPECT A ROLL CRUSHER. AS HE ATTEMPTED TO DESCEND, HE STEPPED FROM THE GUARD WITH HIS LEFT FOOT, BUT HIS RIGHT FOOT SLIPPED FROM THE GUARD AND HE FELL, LANDING ON HIS RIGHT FOREARM.
WHILE LIFTING A LEAD, EMPLOYEE CAUGHT LEFT FOOT ON A BERM AND FELL, TWISTING HIS ANKLE.
GROUND WAS FROZEN AND UNEVEN. WORKER STEPPED OFF THE CEC SCREEN. PUT EIGHT FOOT DOWN, TURNED TO THE RIGHT A DIRT CLOD BROKE LOOSE AND FOOT ROLLED-CAUSING KNEE TO POP OUT TO THE SIDE.
WHILE GETTING OFF CRANE, WORKER SLIPPED (SNOWY & WET CONDITIONS). GRABBED A HOLD WITH RIGHT ARM & PULLED SHOULDER.
WORKING ON CONVEYOR BELT-JUMPED OFF, LANDED ON ARM
WHILE REACHING TO REMOVE A WIPE FROM THE CONVEYOR BELT, EE TRIPPED IN MUD AND FELL HITTING FACE ON A ROCK BOX.
EMPLOYEE WAS CLIMBING THE LADDER INTO HER OFF ROAD DUMP TRUCK #884 WHEN HER FOOT SLIPPED, SHE FELL TO THE GROUND INJURING HER KNEE. WEATHER CONDITIONS WERE COLD AND WET. CAUSATION: MUST USE EX TRA CAUTION AND MAINTAIN THREE POINTS OF CONTACT WHEN MOUNTING AND DISMOUNTING EQUIPMENT IN WET WEATHER CONDITIONS.
