While tamping a hole underground, employee slipped on an oil slick and fell landing hard on right shoulder and arm. Reporting shoulder strain.
While hanging a cable up, he stepped on the first rung of a two step ladder and felt the ladder tip backwards causing him to fall onto the belt line hitting his head on the belt line rail. The fall caused a head wound requiring 8 staples, to the back of his head.
Running a come-a-long, the tooth broke on the machine making him fall, injuring neck and left shoulder.
Inspecting overhead welding leads did not see hole in deck and stepped into the hole injuring his knee and straining his back.
The employee was working on an elevated deck, did not hook the access chains and fell about 10 feet when the pry bar slipped.

Had an initial injury date of 1/28/10, received treatment but was not classified as recordable at that time. No further activity until several months later(Sept 15, 2010)when he had surgery on shoulder. It was unclear whether this surgery was related to work injury or a pre-existing condition related to aging. Review of injury has determined that it is reportable at this time.

Employee was decending from top of a vessel being cleaned, when he slipped on the wet surface and fell aproximately 3 feet to the floor level, hitting his side on a protruding pipe flange and breaking two ribs.
Employee slipped on a loose rock and fell to the ground injuring right knee going to change house at EOS. No medical treatment.. surgery 11/18/2009. Pre-existing knee injury in 2003. Not clear if the new injury was new -no MRI performed for April 2009 event. Post-Op notes received on December 30, 2009 noted medial meniscus torn during the April incident.
Employee walked between switch stand and rail. Employee stepped on a railroad tie. Employees foot slipped off rail road tie. Employee's foot wedged between railroad tie & switch stand bridal rod. Employee fell fracturing the tibia bone in his right leg just above his ankle.
Moving 6" vacuum hose to clean out the screw pit of ash. While holding/moving the hose with both hands employee lost balance & fell landing on left shoulder. This 70001 is being filed at this time because it has just now been determined that it was a work related injury.

Mechanic climbed up & was standing on a 3 1/2" angle iron. He was putting a chocker around a counterweight in order to lift it up & change the take up roller. After chocker was in place, he was attempting to get down, when he lost his footing, fell & landed on his back on the grating below.
Employee was walking west on the road between Mono Elect Shop & Mono I Calciner. Started to cross to the north side of road, looked back to the east to see if any traffic was coming & stepped on edge of pot hole causing her to fall. This resulted in a sprained right ankle & bruised left knee.

Employee slipped on film of mud on floor east of TC-2317
While checking a water leak outside of a building the employee slipped on the ice, striking his left hand, hip and straining his elbow. The injury did not become recordable until he missed a day, November 18th. FMC was not notified that this absence was due to the injury until November 21. At this time the injury is just "pain" but tests are still being performed.
Employee caught his foot in the edge of the Admix Sump Pump. The sump hole had a small submersible pump that was barely keeping up with the inflow of water which caused a layer of water/mud to hide the adjusted grating. Employee's left foot slipped in hole; his momentum kept driving him forward twisting his ankle and applying pressure in the lower shin area.
PAIN IN LEFT KNEE AGGRAVATED BY THE NEED TO CLIMB STAIRS TO DO JOB. HAS GOTTEN WORSE SINCE MOVING TO NEW JOB. HAS BEEN PLACED ON RESTRICTED DUTY.
Employee was walking along a conveyor belt and tripped, falling into the rib hitting his shoulder. He then fell to the ground straining his neck. He then missed 2 days of work to recuperate.
Employee had washed an area down and then gone to lunch. Over lunch the water had frozen and when he returned he slipped while pulling the hose out of the washdown ditch. This are was indoors but there was freezing air blowing through the ditch and an open door. The employee suffered a broken foot and ankle that required surgery.
Employee descending stairs, foot caught on the non-skid tape, causing the employee to twist ankle and break bone in foot.
Employee was stepping backwards off the middle step of a 2' ladder. He stepped on a 13"x24"x7" rock that was slanted causing him to twist and fall to the ground. Injury became recordable when he had surgery on 1/10/05, however FMC did not learn of this until 1/26/05. The left knee injury was a torn meniscus.
The employe was on top of the tailgate drive cleaning off rocks. He started to get down, reached for a hose to hold on to, his toe hung up causing him to loose his balance miss the hose, falling head first into the pontoon on the shield. The tailgate drive is about 4 feet off the grouond. He required 8 stitches above his right eye and fractured the tip of his left thumb.
Employee was working on P-348; as he was walking aroung P-219 to get to P-348, he slipped on some ice and hit his left knee on the ground. This did not become a recordable until 3/16, when the employee went off to have surgical repairs to the knee.
OPERATOR WAS WALKING DOWN STAIRS AND SLIPPED ON ICE, FALLING DOWN SEVERAL. HE LANDED ON HIS BACK AND HIT HIS HEAD AND KNEE ON THE WAY DOWN. HE WAS USING HANDRAILS AT THE TIME.
FELL OFF PLATFORM; MIGHT HAVE BLACKED OUT, AND WITHOUT SAFETY CHAIN, FELL FROM PLATFORM TO NEXT DECK. TOOK HIGH BLOOD PRESSURE PILL 30 MINUTES BEFORE ACCIDENT ON EMPTY STOMACH.
THE AREA AROUND THE 3 EAST CONVEYOR TAKE UP HAD BEEN WET DOWN TO SUPPRESS THE DUST. THE PLYWOOD LAYING UNDER THE BELT FOR SPLICING WAS ALSO WET. AS THE EE WALKED ACROSS THE PLYWOOD HIS FOOT SLIPPED CAUSING HIM TO FALL.
THE DRIVER WAS WALKING ALONG THE EDGE OF THE TRAILER IN THE PROCESS OF HOOKING UP CHAINS TO THE SKIP. THE SKIP WAS PLACED ON THE TRAILER. HE FELL OFF HIS TRAILER ONTO THE GROUND.
EE WAS USING A FOUR FOOT STEP LADDER TO WELD. HE REPOSITIONED THE LADDER, WAS ON THE SECOND STEP WHEN THE LADDER TIPPED OVER CAUSING THE EE TO LAND ON HIS RIGHT SHOULDER.
EE WAS USING A STEAM HOSE CONNECTED TO A DRAIN VALVE ON PUMP SUCTION LINE TO CHECK OUT A LOW FLOW PROBLEM. WHEN EE STARTED DISCONNECTING STEAM HOSE FROM DRAIN VALVE THE WELD ON THE PIPE NIPPLE ATTACHED TO THE VALVE FAILED WHICH CAUSED LIQUOR TO SPRAY OUT OF LINE. EE STEPPED DOWN FROM BOTTOM RUNG OF LADDER HE WAS STANDING ON & HIS FOOT SLIPPED ON THE FLOOR THAT WAS NOW WET.
EE WAS INSPECTING EQUIPEMTN USING AN FGA PLATFORM. WHILE DESCENDING THE PLATFORM SHE HAD TO TURN TO GET THRU THE OPENING TO DESCEND THE PLATFORM. SHE TURNED TO DO SO HER RIGHT FOOT STUCK ON TH E PLATFORM GRATING CAUSING HER TO TWIST HER RIGHT KNEE.
EE HAD STEPPED UP ON LOWER RAKC AND WAS HOLDING ONTO A RACK 2 LEVELS UP, REACHING FOR AN AIR FILTER HIGHER UP. THE SHELF HE WAS HOLDING ONTO CAME LOSSE, HE STEPPED DOWN & BACK, STEPPED ACROSS THE AISLE INTO THE OTHER SIDE. HIS RIGHT FOREARM STRUCK A MOUNTING BRACKET REQUIRING 44 STITCHES.
EE WAS TAKING AN 8 FOOT 2 INCH HOSE FROM THE YARD CREW STORAGE SHED. HE PUT THE HOSE ON HIS SHOULDER AND STARTED OUT OF THE SHED. ONE END OF THE HOSE THAT WAS BEHIND HIM GOT HUNG UP AS HE EXIT ED THE DOOR. HE PULLED HARD ON THE HOSE TO FREE IT. WHEN THE HOSE BROKE FREE, HE LOST HIS BALANCE AND FELL HITTING HIS ARM ON SIDE BOARDS OF TRAILER.
WHILE CLIMBING OVER A PIECE OF MOBILE EQUIPMENT TO RUN JUMPER CABLES, HE FELL OFF CRACKING 2 OF HIS RIBS.
