The operator was performing a wash-down using hot condensate when the hose split, resulting in second-degree burns to the chest area.
The separation of the rib/arch struck the employee
An employee was conducting an inspection of the burner in DR-8, Argo Kiln. The employee was wearing a Tyvek dust suit, N95 dust mask, gloves, a hard hat, and safety glasses. As they were exiting the kiln, a small piece of dried soda ash struck their hard hat, and dust from the soda ash made contact with their left eye.
Employee activated the spring return on the lubrication hoses and became tangled in the hose and fell.
An employee was standing on a 17 inch two step working platform performing maintenance work. As EE applied force to the box end wrench, the wrench slipped causing EE to lose balance and fall. In falling, EE's right knee contacted a ceramic liner, resulting in a fractured knee cap.
The employee was climbing down a scaffold ladder. Employee was tied off to an overhead SRL while descending a ladder. Their personal srl got caught on the scaffold gate. While descending the ladder, they noticed the caught psrl hook. As they looked up at it, the hook freed itself and retracted as designed. In that retracting process, the hook hit our employee in the mouth.
A Mine Production employee was in the process of bolting at 8 BMR. Employee noticed a bad top in the entry and exited the cab of the bolter to bar down the hazard. As employee barred down, the arch behind the employee came loose and fell and struck the employee from behind.
A Mine Production employee was operating bolter drilling a 14 foot drill hole when the steel became stuck. EE placed EE's left hand on the drill steel to apply outward force to free the steel and reached for what EE thought was the lowering lever and inadvertently activated the rotation lever. In doing so EEs hand rotated with the drill steel causing a dislocated left pinky finger.
The underground Utility Crew was taking structure down from a panel. As they were bringing in the forklift EE noticed a rail was close to the forklift tire and was going to run over it. EE bent down to move it out of the way, but the tire of the forklift ran over the end of the rail picking up the other side, hitting the employee in the lip.
On February 11, 2025, a mine maintenance employee was performing routine maintenance on a borer miner. While stepping onto the bore miner rotor bit block, EE's foot slipped, resulting in a fall and a laceration to EE's left calf from contact with the borer miner bit. The injury required sutures.
An employee was standing on the FCT ramp for a belt move and slipped on the fines. Once the employee slipped on the fines, they felt pain in right knee. MRI was completed 1/27/2025 and was determined to be reportable on 2/7/2025.
While changing bits on a boar miner the bit wrench slipped smashing their left thumb between it and the tooling plate requiring four sutures.
Employee was lifting a heavy drill into place when they felt a pull in their lower stomach and groin. The employee was evaluated by a physician and diagnosed with an inguinal hernia. Employee is scheduled for surgery to repair it on 11/27/2024.
An employee was removing a cable spool when the cable spool above it fell striking EE in the left arm, fracturing the ulna bone.
The CZ-3 level indicator was incorrect and was blocked and salt-encrusted. Valves were opened during attempts to flush the sight glass with condensate, the sight glass ruptured. The employee was sprayed with hot condensate and crystallizer liquor, resulting in 2nd-degree burns.
Helped a forklift operator position a 6" wide/300 lb. flange I-beam onto a steel rack, placing theirleft hand underneath the I-beam and pushed with right hand while the forklift operator withdrew the forks. As the forks were removed, the I-beam rolled and pinched their right middle finger between it and a 4-inch I-beam positioned below it causing a laceration needing sutures.
Employee working overtime was manually carrying a 6-foot-wide bottom belt roller, weighing approximately 70-80 lbs., on EE's shoulder. Upon reaching EE's destination, EE began to set the belt roller down when EE felt lower back muscles tense up and spasm.
Installing 14 roof bolts in the 2W Mains, employee experienced pain in shoulder as they lifted the cable bolt to insert it into the roof bolter chuck. EE is advised to rest it on days off, re-evaluate 6th when they receive a full work release. EE notifies Solvay of an MRI scheduled on 20th, 22nd they are seeking Ortho for acute tearing and surgery is scheduled for June 6th, 2024.
Employee was assisting in putting a piece of culvert back into place on BC-57. The cover was lifted over the handrail, set on the deck and then lifted to move into place. The employee positioned self to assist placement. In the process of pushing the cover into place EE leaned against a handrail. The handrail gave way and the employee fell approximately 10' to the ground.
A mine production crew pulling excess miner cable back to the power center had an employee watching the cable being pulled so they could stop them when the new cut location was reached. When finished, the employee stepped backward, stumbled and fell. In an attempt to catch themselves, they extended their arms backward while falling from ground level and injured right wrist.
Employee was preparing a maintenance kitting pallet and moving material on top of the bottom pallet to stack another pallet on top. Positioning self in a manner to obtain proper leverage to slide a crusher anvil, then applying force to move the crusher anvil, EE felt pain in the lower back. Seeking medical treatment on March 12, EE was not released to work until March 18.
Employees were pulling a feed side conveyor chain into place with a chain fall hoist, the hook shaft broke and pulled out of the knuckle ring assembly. When this occurred, the employee was struck in the elbow causing a one-half-inch laceration which required two (2) sutures.
While two employees were attempting to move an angle iron brace one of the workers had picked it up but dropped because they lost their grip before the second employee could grab it. This caused the angle iron to drop about 16 inches the second EEs left foot.
Employee was grinding on some metal and claimed a foreign body from material, flew past employee's face shield and safety glasses entering the left eye.
At approximately 9:25 am on Friday the 29th of September 2023 while conducting media loading activities on the Heat Recovery Chamber-6A, at the Vulcan Project site, the employee forced between the pallet of media and support brace inside the HRC. The employee sustained soft tissue injury and minor fractures to the pelvic area.
Exiting feet first from a boiler EE had been working on, through an opening at the traveling grates, EE's feet were not on the outer ledge, so EE slid off the traveling grates, through an opening between the front ledge and the traveling grate that was twelve (12) inches wide, falling in the ash hopper approximately nine (9) feet below and suffered an ankle injury.
Employee secured a section of the miner cable with both hands and proceeded to lift the cable upwards to place on the cable hook, then felt a pop in their right shoulder and sharp pain causing them to drop the cable. Received a full work release from the clinic but opted to have surgery after MRI results which became a lost time injury starting 8/15/23.
An employee was working on a roof replacing metal roofing. EE got to the end of the SRL and it pulled EE back. Causing EE to trip. EE tried to catch self with left arm.
Attempting to line up the pan line, an employee stepped up on a relay bar and placed hand on top of the spill plate to get a better view. As EE was looking towards the headgate, a large rock slid down the pan line and slid across EE's left hand, pinching it between the rock and the spill plate causing a laceration.
Using a battery-powered chainsaw to cut out foam stopping. Cutting upward so EE placed hand underneath and when EE did this, left ring finger entered the clean-out area of the chainsaw, resulting in EE's finger contacting the chain causing a laceration. Had on a cut-resistant glove which got pulled off, wadded up in the chain causing the brake to be applied to stop.
Employee was working on un-salting a production line. EE was bleeding steam off the line, isolating the steam, taking the hose off to drain and open the line. In doing so, EE turned and hit EE's head, spinning EE around and EE's hand went under the hot condensate.
An employee was walking down the underflow tunnel of TA-110 when their hard hat struck a light fixture.
Employee was attempting to clear the suction of an underflow pump that had plugged. The plug cleared and then they slipped while exiting the area and fell to their knees in the hot liquor on the ground, receiving burns to legs and arms.
Employee was emptying the S-300 hopper in the grits pile when a wind gust blew dust into EE's face and right eye. EE's eye was still irritated the next day and EE was taken to seek medical attention for an abrasion to EE's eye, which EE received prescription eye drops for.
Employee rolled up area rug weighs 26 pounds in prep of mopping floor. When placing rug onto a cart, experienced pain in the L arm from shoulder to elbow. MRI's on shoulder and elbow show chronic degenerative disease, frayed labrum, torn rotator cuff, bicep tendon tear. Accepted as work related on 10/19 following medical review.
Employee received surgery 11/10 for torn rotator cuff after a couple months of pain that was not improving. MRI revealed full thickness perforation of supraspinatus tendon and concomital tear; supra- and infra-spinatus tendinopathy, partial and full thickness tears and bursal surface fraying. Degenerative arthritis in AC joint, bursopathy, and mild nonspecific synovitis.
Employee was walking down a flight of stairs on Cooling Tower 3 (CT-3). While descending the stairs, utilizing the handrail, EE stumbled, and caught self in a twisting motion, while applying weight to EE's left leg and knee. An MRI was completed indicating a torn meniscus, a fresh tear, as well as some other historical injuries. Surgery was performed.
An employee was confirming that a valve was shut, requiring EE to duck under a set of pipes. EE ducked back under the set of pipes to leave the area, walking bent over, EE struck the back of head/neck on a wash down water drop causing a laceration behind the right ear. EE was taken to the company physician for evaluation and received three stitches to close the wound.
A surface mechanic suffered a laceration to left middle finger, while picking up a volute liner which had a sharp edge, cutting through EE's gloves, requiring three sutures.
Cleaning the kitchen floor with a mop and bucket, some of the soapy water had slushed out of the bucket. As EE was mopping the floor, EE slipped and fell on left side and shoulder.
The miner operator stopped mining, swung the tail toward the right rib and exited the cab. It is unclear why EE exited the cab of the miner. EE was a couple feet from the operator's compartment when the arch fell, striking EE in the head/neck/shoulder/upper back area. The injured employee was provided first aid in the mine and taken to the Memorial Hospital of Sweetwater County.
Performing an inspection inside of a tank, coming out of the tank from the side opening, feet first and had pressure to EE's biceps pulling self through the opening, lost grip with EE's right hand, used EE's left hand and arm to catch self and immediately felt pain and burning sensation in EE's left bicep.
An EE was jack hammering the Debris when it broke free. IE's hand was on the stack wall when the debris impacted EE's fingers
Removed a counter weight guide rail on a bucket elevator to work on a bearing behind it and when re-installing it, the lower portion slid off of the base and started to fall downward. As it fell, the injured employee grabbed the rail so it would not fall towards EE. This resulted in EE's left index finger being pinched between a stop block and the counter weight frame.
Assisting with a horizontal pressure leaf filter, being filled with hot water, in preparation for an acid wash, on the deck to confirm when the filter was full. The carriage bolt assembly that holds the filter head, to the filter shell, failed and sprayed the employee with hot water. Employee was treated for 1st and 2nd degree burns and given a full work release on 08/02/21.
An employee was walking and stepped on a rock and rolled right ankle but continued working through the shift. Medical treatment was sought and an MRI was obtained on July 20th which determined that they had ligament issues in the ankle then employee was placed on restricted duty July 22nd.
Engineer was climbing the steps on a locomotive when hips and back began to hurt. Later claimed to have numbness in hands and legs.
Employee struck head on a roof support cable bolt. The suspension in EE's hard hat broke and EE received a laceration, to the scalp, likely from the rivet for the suspension.
Track laborer was using a track wrench on a joint bar bolt. EE slipped and twisted left ankle.
Employee was adjusting fly ash system when a piece of material blew into EE's eye. EE was checked by a physician and received a prescription antibiotic ointment for precautionary purposes. This injury report was overlooked during furloughs, etc. amidst the COVID crisis.
