Employee states that he was boarding a haul truck and as he pushed off with his right foot he felt popping in his right knee.

Employee was leaning up against the loading bay door way frame and fell backwards, landing on his right side. Employee had a hand held radio fastened to his right side pant waist band. Employee was given first aid and refused transport to a medical facility. He later sought medical treatment and was diagnosed with a fracture.

Received notification that employee was diagnosed with simple pneumoconiosis category s/t, 1/0

While walking to the shot he stepped into a borehole, that had been frozen over with mud, and fell twisting his left knee. As a result he has a mild sprain to his left knee.

Employee states he was checking engine oil in haul truck and as he pulled the dip stick out he lost his balance and fell to the ground He was seen at CAMC ER and was released but later had pain in his buttocks and tingling in his leg. He returned to ER and was removed from work.

Employee noticed a flame from engine compartment and then set off the fire suppression. As he exited the machine the wind blew the dry chemical into his eyes causing him to lose sight of the step and he fell onto the push arm of the of the dozer. This resulted in a hair line fracture of the lower leg.

Employee was reclaiming a high wall in area N. Employee states that he had pushed to a bench and was in the process of turning to go back up the slope when a rock dislodged causing the tractor to slide over the bank onto the machines right side. This resulted in the tractor overturning approximately three times before coming to rest in an upright position. Received 9 sutures.

Employee filed a claim for pneumoconiosis.

Driver enter company property approximately four tents of a mile went unconscious roll back into ditch line vehicle came to stop. Driver was taking medication he was not subscribed for.

Employee stated he and another employee were using air chisels to remove a bearing race from a bearing assembly for the hoist drum on the 495 shovel. In doing so, the chisel tool had a piece of metal chip off and struck the employee in the left leg just below the knee.

A contract mechanic received a laceration to the side of his head while attempting to run the boom on the service truck out. Mechanic toggled the boom swing button and he meant to press the boom extension button. Mechanic was taken to CAMC General ER where he received 4 sutures to close the wound. He returned to work on 12/10/2013.

As employee was walking around truck, his feet slipped on ice and he fell to the ground onto his back.

Employee was pushing the rubber basket in the pre-cleaner housing on a D11R when his left hand slipped and he struck his left wrist on the adjacent bolt.

Service Technician was standing along side his service truck. At a distance of aprox. 30 feet another crew was installing a track on a Hitachi EX600-6, using the prescribed method when a piece of metal flew across the 30 feet and struck the employee in the neck.

Employee stated he twisted his left knee while stepping from truck.

Injured employee was handling a sixty inch long, 6 1/2 inch diameter piece of drill steel used to anchor the highwall miner when he slipped and fell. The end of the employee's left pinky finger was mashed between the steel he was handling and the steel structure of the highwall miner.

EE was carrying tool tray onto D11R dozer, right elbow popped and started burning.

Employee was handing employee a string of insulators. They slipped out of his hand and hit injured in mouth damaging his 2 front teeth.

EE was removing a bolt that was holding the oil cooler adapter plate. Bolt was extremely tight and he was using both hands. While pulling, felt pain in right elbow.

It started snowing fast, employee was going down hill when his truck started sliding. He turned a 180 and turned over on its side. He got out himself.

Employee was walking from his personal vehicle to the rock truck he was going to operate when his feet slipped out from him and he fell landing on his right leg.

Traveling down haulroad in loaded truck, driver thought he was in the wrong gear and got truck out of gear. Driver applied brakes. Truck wouldn't slow down enough to get it in gear. Truck made left hand turn. The truck tipped to the right, the driver over compensated and the truck turned over on left side.

Two employees changing bell crank on a loader. One was striking a bearing with a hammer and two metal fragments hit the other standing to the side. One fragment embedded in the right cheek and had to be removed.

Securing 2 hydraulic pumps to the bed of parts truck with chains and binder. Strained low back.

Employee states he was changing a Western Star tire, he was rolling the tire and it almost fell over; he grabbed hold of it to keep it from hitting the pickup causing him to hurt his back.

Company notified by area hospital that a claim was being filed two (2) days after the alleged incident occurred. Employee returned call on Thursday afternoon and stated that he hurt his back lifting truck hood. "Report filed under protest."

Employee alleges that while reaching out to adjust the truck mirror, he felt pain in abdomen. "Report filed under protest."

Employee stated that he was getting out of truck when he missed a step, falling to ground on right shoulder. "Report filed under protest".

Occupation Pneumoconiosis - State Disability Award of 10%

While dumping load from truck bed, the bed fell off when raised to full extension of the first jack.

Employee was stepping down from a ledge and his foot went into about 10 inches of mud. While trying to take a step he pulled his knee trying to dislodge his foot. This event was not reported for 3 days.

Re-injured knee (from 11//3/07 incident) while diving under truck to avoid potential fly rock.

Employee was picking up 2 full boxes of Pentex 3/4# boosters when he felt "it pull in lower abdomen & back".

Employee was attempting to unclog powder from the hose that was connected to the discharge auger. The hose came off the discharge tube and while trying to avoid the falling tube, he twisted his right knee.

Employee was lifting a full box of Pentex 227 primers out of day box on a truck, when he felt his shoulder blade begin to "burn".

Injured employee was dismounting the rear of a grader after checking the engine oil. As he placed his right foot on the ground, his ankle gave out causing him to fall and fracturing the right ankle. The ground surface was smooth where he stepped. He may have stepped down on the side of his foot.

Employee was using a hacksaw to cut a piece of 3/8" all thread being held by a vise. Employee was using left hand on top of right hand holding the hacksaw. When the saw cut through the all thread, his left hand struck the vise. He continued to work but the hand later swelled. Upon examination, he was found to have a minor fracture to the left hand.

EE stated, while his truck was being loaded, a rock rolled & struck the inside of the bed of his truck & caused a severe jolt to his neck & then when he went to dump the load, felt pain in neck. When he returned to the loader, he contacted his foreman & requested an injury report be filled out; he completed his shift & drove himself home.

Not sure when or how. Employee discovered a metal particle in his left eye the following morning after his shift. He never felt the particle enter the eye. Wind apparently blew the particle in sometime during his shift. Particle had to be removed by his eye physician.

EMPLOYEE STEPPED IN ATF WHILE DOING P M ON RIMPULL WATER TRUCK AND WHEN HE STEPPED UP TO GET IN TRUCK HIS FEET SLIPPED AND HE FELL HITTING ELBOW ON STEP.

EE stated he was changing a tire; as he was lifting the tire, he felt a pain in his side.

Employee was standing on top of truck under powder bin. While holding onto the safety rail, he attempted to push open the door to one of the truck's bins and twisted his knee.

Employee was dismounting a dozer by stepping onto the track, and onto the push arm. As he was stepping from the track to the push arm, his left foot slipped and he tried to catch his fall with his left hand. The hand struck a piece of raised metal installed on the push arm for foot traction.

Employee stated he was traveling uphill when his truck powered out, causing him to drift backwards. He cut toward the bank which caused the trailer to flip onto its side and the tractor vertical.

Injured employee was driving a pin out a bed hoist cylinder on a rock truck with a slate bar. When the pin came out, the bar pinned against the dump bed catching his right middle finger causing a laceration requiring 8 sutures to close.

Employee was a passenger in a pickup truck being driven by a fellow employee. They were returning from the loadout to the plant after loading a train. The pickup struck a berm and rolled on its right side. Employee received lacerations of the right arm from the accident.

Employee attempting to shift gears going up grade with truck. Got it out of gear. The truck rolled backwards into a ditch and turned on its side. Employee got out, noticed a fire starting in engine compartment. Used fire exinguisher but did not get fire undercontrol.

Employee was drivng truck down mine road. Lost control and jumped from truck. Vehicle rolled over onto side. Employee did not see Dr. until 5-28-07. 3 days restricted duty. Returned to full duty on 6-1-07.

When shutting door on Cat water truck, employee alleges he hit left elbow with door.
