The EE stated he was standing out by the miner between the coalhauler and the mine rib while the miner was loading the hauler. The rib rolled out onto him knocking him down and into the hauler. The rib struck him in the lower leg around the calf area. The EE began missing work on 4/13/11 due to the incident.

Employee stated he was moving a 6"x6"x8' timber out of the roadway, when he picked up the timber he felt pain in his lower back on right side and had swelling on his abdomen. The EE did not miss work until 2/17/11 and became reportable at that time.

The employee stated that he exited the mantrip, reached for his lunch bucket, and as he started walking out of the mantrip shed he tripped over a bucket of pinner bits and fell onto his dinner bucket. The employee injured his left wrist and ribs. An x-ray showed a fracture to the radius. The shed had a concrete floor and was wet. The bucket was sitting 20" from the paarked mantrip.

Medical evidence received indicates a 10% black lung impairment.

EE stated that he strained his shoulder while carrying his tool bag from mantrip to the maint. truck approx. 100'. Incident became reportable when he started missing work on 5/21/10. *Medical Documentation indicates this is an occupational Illness*

The EE stated that he tripped while walking towards another EE, hyperextending his right elbow as he fell forward onto it. The EE continued to work and began missing work on 3/8/10, this incident is being reported as a lost time, reportable date of 3/8/10.

Medical evidence received indicates a 7.21% permanent partial disability hearing loss.

Medical eveidence recieved indicates a 0.1815% hearing loss.

Medical evidence received idicates a 10% permanent partial disability for Occupational Pneumoconiosis.

Medical evidence has been received which indicates a 8.51% hearing loss attributable to occupational noise.

EE stated that he felt a pain in his groin area and felt a knot swell as he was loading supplies on bolter. On May 22, 2009 he was diagnosed with a hernia that would require surgery.

Medical evidence received 3/2/09 indicates employee has a 6.78% permanent partial disability due to hearing loss.

Employee stated he was undressing at his locker preparing to put his work clothes on and while hanging up his ball cap, his finger came in cintact with the locker door and cut his middle finger.

Employee stated he was cutting a piece of dust hose with his pocket knife. The knife slipped out of the hose and cut his right thumb, Employee left work and went to ER where he received 3 stiches to close the wound, he returned to work his next shift.

Medical evidence received 9/2/2008 states diagnosis of occupational pneumoconiosis. Due to the information this incident is being reported to MSHA as an occupational illness.

Employee stated that the chain from one of the two chain hoists came back and hit him in the hand causing a laceation to his index finger.

Medical evidence received states 3.48% permanent partial disability due to hearing loss. This is being reported as an occupational illness.

Medical evidence received 4/4/08 states employee diagnosis of cts. He is to have first carpal tunnel release surgery on 4/7/08 and is being reported at this time as a lost time illness.

Medical evidence received 2/19/08 Black Lung claim.

Employee was making repairs to the #5 Belt Drive. Employee stated that he was climbing down off the speed reducer when he stepped to the ground; he stepped on a small rock causing him to twist his left knee.

Employee stated he was removing a rock from off the #5 belt at break 23. The rock spun on a top roller, cutting 2 fingers on his right hand.

Medical evidence received Nov. 5, 2007 states diagnosis of occupational pneumoconiosis.

EE stated that he was operating a Phillips Shuttle Car. While traveling up the #4 room toward the miner, he ran over some rough bottom and loose material in the roadway. This caused him to be jarred into the canopy, resulting in an inury to his head and right side of his neck. EE continued to work until 9/17/07 and was taken off of work by his doctor, then became reportable.

The miner operator was trying to get up out of the bottom rock. The miner was grading the bottom as it backed up the shuttle car "377" got hung on the back of the miner machine "224". This picked the SC up off the ground approx. 16". Miner helper tried to flag miner operator to stop, but operator was already moving forward. This caused the SC to drop off of the miner approx. 16".

Pneumoconiosis claim received medical April 2, 2007. This report being made to abate violation. Employee withdrew Part 90 status.

The EE stated he was loading supplies on the 410 roof bolter. As he cut the metal band on a bundle of roof bolts, one end of the band struck the EE on the right side of the face aournd the temporal region. This resulted in a laceration that required 2 sutures.

Pneumoconiosis claim received medial March 23, 2007. This report being made to abate violation. Employee withdrew Part 90 status.

The 413 RBM was tramming to #6 face. As it was being trammed, employee was hanging cable on the rib. As the bolt machine made a turn into #6 entry the cable tightened and was pulled towards the outby rib. The cable pulled the employee's feet out from under him causing him to fall to the bottom. As he fell he extended his arm to catch himself which dislocated his left elbow.

EE stated he was installing outside bolt using 2 piece's of 4' SRD bolt. He started to shove the bolt up in the hole and pinched his thumb in between the SRD coupling and the mine roof.

EE was shoveling at 1 Left Section tailpiece. He stated he was in bent over position facing inby next to belt line shoveling when a rock bounced off belt & struck top of his hardhat. There was damage evident to hardhat. He'd been cautioned prior to working in area concerning rocks bouncing off belt line as feeder cleared. Addendum: Absenteeism not due to any injuries rec'd @ mine.

The EE stated he was in a bent over position throwing bolter cable out of the way, when he raised up and struck the left side of his neck on a damaged draw rock shield (pizza pan) that was bent down from the mine roof. This was a low area with only 58" of height to the shield. The EE received 6 stitches as a result of the laceration.

Employee stated he was getting out of coal hualer when he twisted his left knee.

EE was pulling up to 223 miner when miner started backing up. His shuttle car got stuck on the bumper of the miner. He started flagging miner operator when miner started to pull forward dislodging the bumpers, letting the s/c drop to the ground throwing EE up into the canopy knocking his mine hat off. His head then jerked backwards into a bolt/nut lacerating the back of his head.

Upon an investigation, the EE alleges the RBM cable struck him across the head, knocking him down to the ground. The EE's complaints were the lower back and knee. EE was transported by ambulance to the ER where he was treated and released. He is on transitional duty with restrictions for a back and neck strain noted as subjective without spasm.

Investigation the ee was completing the rib bolt installment process along the l. hand rib #4 entry. After spinning resin he was appling hold pressure when 12" wrench broke allowing upper portion of wrench striking him in the l. side of head & face. He was trans. to ER ambulance and transferred to where he was admitted for contructive sugery to his cheek bone and nose.

Employee stated he was using a utility knife to open a bucket of plaster. The knife slipped cutting his right thumb that required 4 sutures as per the medical he received 10/17/06.

EE stated he was struck on the head by the EE's canopy when peparing to swing the boom over toward the inside bolt. He attempted to drop the canopy hy level control from the mine roof and it would not release. he then attempted slide the boom sideways thinking the canopy was hung on the bolt plate. As he slid the boom the canopy fell from it position and struck the ee on the head.

EE stated he was assisting the maintenance man on changing out a broken head torque shaft. He said he felt a pain in his right knee when he climbed back off the miner. EE stepped down exactly 48" from the top of the miner to the mine floor.

Co-worker stated that he and employee were helping do maintenance work on conveyor. Chain was fouled. He said they went to look if any miner bits were missing to see if that might have fouled chain. Co-worker was at bits, heard noise, looked and employee was laying on the ground with a piece of rock laying beside him.

EE filed a claim on black lung. Report came in the mail.

EE stated he was turning #2 MRS arround to tram across section when a rib rolled off behind him hitting his legs. He stated he jumped forward to get out of way of rib roll and tripped over MRS cable. When he fell, he stated he landed on his left shoulder and side causing his light to push into his side and back.

Employee stated he was handling cable on miner. As he was removing a loop of cable he got a sharp pain in his lower back. Medical received July 24, 2006 indicates possible hernia to the groin and a lower back strain.

Upon an investigation the EE was pulling a supply car into the mine using a diesel tractor when a piece of rock fell and hit him. The rock fell from the edge of a half header which was hanging on the roof of the mine. The rock was approx 14" x 17" x 2" & struck EE in the head. The mine roof was about 6 1/2 ft. in height. EE was transported via ambulance to Boone Memorial Hospital.

EE stated that he was nailing up fly pads while standing on the second step of a four foot ladder when he lost his balance and fell to the mine floor.

Employee stated he was bending over picking up dust bags that the pin crew had changed out that were full of dust. While throwing the bags into the scoop bucket he twisted and felt a pain in his lower back.

EMPLOYEE STATED HE TRAMMED THRU FLYS STRIKING OTHER HAULER. EE STATED THAT THE OTHER HAULER DID NOT HAVE LIGHTS ON. ALSO STATED OTHER HAULER WAS IN A DIP & HE DID NOT SEE THE HAULER.

INVESTIGATION REVEALED EMPLOYEE WAS ATTEMPTING TO CUT A SECTION OF SUCTION HOSE WITH A UTILITY KNIFE WHEN THE KNIFE SLIPPED AND CUT HIS LEFT INDEX FINGER. STITCHES REQUIRED.

THIS INCIDENT BECAME LOST TIME DUE TO EMPLOYEE HAVING SURGERY ON HIS FINGER.

EE stated he was attempting to get a large piece of rock off the 222 miner with the slate bar, when the bar slipped stricking his right ring finger against a metal stob on the rub rail.

EMPLOYEE WAS HELPING INSTALL POST WHEN RIB ROLLED OUT CAUSING A POST AND RIB TO STRIKE HIM AND KNOCK HIM TO THE GROUND. RIB MEASURED 14' X 10' X 41".
