EMPLOYEE WAS CHANGING TEETH ON THE LOADER BUCKET. HE WAS HITTING TOOTH WITH HAMMER AND A PIECE OF METAL CHIPPED OFF AND HIT EMPLOYEE'S LEFT ARM. THE METAL CUT ARM. IT REQUIRED 4 STITCHES.
