EE WAS USING SLEDGE HAMMER ON CRUSHER. HE HIT HIS LEFT HAND AGAINST THE CRUSHER WHILE USING SLEDGE HAMMER CAUSING BRUISING AND ABRASION SEVERE TO THE SMALLEST FINGER ON HIS LEFT HAND. HE WAS T AKEN TO MESA LUTHERAN EMERGENCY ROOM MESA, ARZ WHERE THE ABRASION WAS CLEANED.

EE WAS STANDING ON GENERATOR FRAME SPOTTING CONVEYOR BELT FOR ADJUSTMENT ON A WELDING LEAD THIS RESULTED IN A SMALL CUT TO THE RIGHT SIDE OF HIS FOREHEAD EE WAS TAKEN TO MIAMI INSPIRATION HOSP TIAL WHERE HR RECEIVED STITCHES TO CLOSE THE CUTRETURN TO WORK THE FOLLOWING DAY.
