EE was walking on grating on the 5th floor at the washer to retrieve a shovel while cleaning out 4/5 mudball chute. As employee was walking, EE stepped on some rocks and rolled their right ankle. EE was not carrying anything at the time. EE received prescriptions, was placed on restricted duty, and prescribed physical therapy on 2/24/26.

Employee was coming down from an 8-foot A-frame ladder when right foot slipped off the bottom step and twisted right foot causing a fracture in the 4th metatarsal bone.
Employee was unhooking a large chain being held by the crane with no load on the chain. The chain became unbalanced and run thru the hook and the end came around and struck the employee on the right flank causing a contusion and two minor abrasions. The employee was terminated for not following company policy (positive drug screen).
The door fell on the EE's ankle and the force knocked EE to the ground. It was determined the drain valve was plugged and did not drain as expected. EE had xrays that are negative. Seated duty. EE was involved in a motor vehicle accident side swiped by vehicle. EE was wearing seatbelt. There was no loss of consciousness. EE was ambulatory on scene. EE complains of continued discomfort and stiffness to the left hip and lower back area. An MRI was ordered and results reviewed on 11/4/24. Dr ordered PT Restrictions are no lifting or carrying greater than 20 pounds
EE states EE closed the door to the mobile equipment onto right middle finger. EE has a distal tuft fracture in right middle finger. This is a small fracture at the tip of the finger. There is no indication for any further treatment for this type of fracture.

EE was removing bolts with left hand on impact, right hand holding bolt head with wrench. Wrench slipped off pinching the tip of the rt thumb. EE received 5 sutures.
Maintenance mechanic reported elbow pain due to repetitive sledgehammer use on 9/25. After conservative treatment, EE was diagnosed with tendinitis on 12/4 and prescribed physical therapy.
EE stated the pipe pushed EE back at the level of the chest and knocked EE onto EE's bottom. The MRI was on 8/15/2023. On 8/23/23 EE was placed on restrictions.
At the time of the accident, EE was traversing through a wetland on foot while performing vegetation management practices as part of the mine's reclamation processes. As EE entered an area that was densely populated w/ various types of vegetation, a Bulrush stem slid under EE's safety glasses and made contact w/ left eye, causing an abrasion of the left cornea.
IW placed EE's hand between two sticks of rail. Eq. Operator moved the stick of rail that was being installed, without clearing red zone. Stick of rail being installed slid forward and caught the end of the index finger of the R/ hand causing laceration.
Injured employee (IE) completed pre-operation inspection of the truck and proceeded to operate the truck, hauling material. IE shortly after realized right eye was sensitive to light and felt discomfort. IE stopped and notified supervisor. IE was taken to an optometrist where a small foreign object was found and removed from the right eye. No specific event was indentified.
Crew was working at the Mosaic Ona location. The crew went to take their lunch break, one employee went in to take lunch in the conex. The employee in the conex was attempting to open the window to let air in. As they were trying to slide the window open with their hands the glass broke. As a result, the employee received a laceration to the palm of the right hand.

EE was driving company vehicle when EE believes EE ""dozed off"" and drifted to the right of the mine road into a ditch and landed on passenger side of truck. Airbags deployed, the EE was wearing seatbelt.
EE states EE was taking the cable off the tub on the #18 dragline and felt a pop in EE's left arm. EE states pain with movement ""7""/10 at rest ""2""/10. observed bicep deformity. Deformity noted on upper bicep/shoulder. A gap in muscle at shoulder attachment. bicep muscle firm. No bruising or swelling at this time.
Employee was by personal vehicle leaving work on 11/9/22 and reports something was blown into their right eye.
Operator was dismounting haul truck. Operator lost footing during transition between steps, followed by losing grip to handholds, resulting in falling to the ground landing onto feet. Upon standing, employee felt pain in lower back.
EE was standing on a pump platform turned to walk and re boot was caught. EE fell onto rt shoulder. EE had MRI done on 12/2/2022. EE placed on restrictions on 1/13/2023.
EE fell asleep while driving. Hit a berm and landed in ditch, injuring left wrist. EE taken to hospital and prescribed Toradol and Robaxin.
EE states EE was operating a bulldozer and there was some malfunction of the machine while EE was backing up that made EE lunge forward. EE was holding the gears with EE's left hand, which put oppositional force on EE's left arm/neck. EE states EE has pain in the left side of EE's neck, left shoulder and upper left arm. Identified restrictions by MRI on 11/5/2022
Operator was climbing up access point to the cab of an off-road haul truck. The operator missed the fourth step with left foot causing the operator's body to fall into the steps. The operator then attempted to dismount the machine and missed the bottom swing step causing the operator to fall to the ground on right side.
EE was working on a water jack. The employee became over heated and started having cramps. EE sat in the truck to cool down and a supervisor came by and they called the ERT team. They checked EE's vitals and gave EE something to drink to cool off. EE was transported to the ER. EE received fluids by IV for approximately 3-4 hours then released.
EE was driving work vehicle and hit a bump. This caused EE to hit head on roof of vehicle. EE states grabbed shoulders because of pain caused by head hitting the roof and when EE looked ahead, EE was driving directly into sun. The next thing EE remembers is people talking to EE after vehicle had turned over. EE also injured left knee and was placed on restrictions on 6/7/22.
EE reported to the clinic stating at 7:00pm EE was ""stepping off maintenance truck placed right foot on ground then left foot on ground turned to step away from truck and rock rolled out from underneath foot rolled right knee inwards and felt pop."" Restrictions placed 6/5/2022.
While getting off the D6 dozer had 3 points of contact, misjudged the height to the ground, was still holding on to the handrail when foot hit the ground and hyperextended EE's right shoulder. Became reportable when EE order PT on 8/2.
Employee was entering a payloader and as they reached to close the door, a gust of wind slammed the door shut smashing the employee's pinky finger in the door jamb resulting in a laceration to the tip of the finger.
EE slipped and fell when EE stepped in a cutout of a metal platform that was submerged under water. This caused EE to twist neck and left leg. EE was treated conservatively until 4/11 when EE was given a prescription for PT making it recordable.

Installing cap on the R39 pinion side with the rod bolts for the carrier and cap. Nut on bolt that holds the bearing cap. Bar slipped from holding the rod bolt and pinky finger was between the nut and cap. EE has an injury to the pinky finger on nail bed.
Employee was operating a Polaris XP 1000 UTV on a mine road and hit a pothole which caused employee to briefly lose control of UTV. After hitting the pothole employee briefly lost control and swerved to the left off the road. This caused the left tire area of the UTV to strike a tree.
EE has been diagnosed with bilateral carpel tunnel syndrome in Left Wrist. Surgery performed on September 20,2022. No lost time only restricted duty.
While aligning the faceplate to the pump shell for a centrifugal slurry pump being reassembled in the mine field, a maintenance employee injured their left ring finger when it was caught between the two components. The employee was wearing gloves at the time of the incident.
While going to work on a pump block valve near #8 on 20, it was dark, stepped into an unseen hole and landed, or impacted the ground on left knee. EE was unable to stand. Restrictions were placed on 8/27/2021 due to restrictions of sedentary duty with minimal walking due to safety issues and EE wearing a knee immobilizer and using crutches to ambulate
EE was walking down the steps of the 852 dragline when EE lost footing. EE was holding onto the handrail with left hand and twisted right ankle upon breaking the fall. EE also injured left shoulder in an attempt to catch self. After ongoing care and treatment and following an MRI, physical therapy was ordered to address issues identified with the shoulder. PT prescribed 3/24/22
Employee was getting into svc truck and hit head on the door jam.
Employee stepped on grating to troubleshoot dredge ladder welds securing ladder broke loose causing employee's left leg to fall leading EE to strain EE's right leg causing the injury. EE was on restricted duty until 7/6/2021
Employee was loading old rollers into the loader bucket for disposal and smashed EE's left little finger between two rollers.
EE was operating a Front End Loader positioning some HDPE Pipe, when setting the pipe down EE apparently disturbed a bee hive. EE was seen by a co-worker swatting the area around EE and EE exited the Loader quickly after putting it in park. EE indicated EE had been stung multiple times by the bees.
EE was assembling steel pipe with wrench EE struck lt thumb with wrench EE was wearing gloves Injured ee was sent for further medical eval on Feb 23 and was transported off site for testing, x-ray of the injured thumb Results of the x-ray revealed a small tuft fracture beyond the first joint.The ee has been placed on restrictions and is awaiting treatment from a hand specialist
IE was seating a PVC pipe. While holding a pvc screen, IE crossed over EE's left hand with EE's right hand and tapped the screen with a 2 lb hammer but caught EE's left pinky finger with the handle subsequently resulting in a compression injury.
While using a Back Out Hammer to remove a keeper pin from Dragline, miner felt a pain in the right arm. A metal fragment, created by striking the B&O Hammer against the keeper pin, propelled a metal shard causing laceration to the arm.

Miner was in the process of rotating a coupling, on dragline pumping system. As miner slid hand down toward the end of the bar, EE's hand slipped off resulting in a laceration to the webbing between the thumb and forefinger. The employee was wearing gloves at the time the incident occurred. Miner has received pain meds, sutures, and is on restricted duty.
(Ground Conditions) Employee was walking through ankle deep water applying herbicide via backpack sprayer to invasive aquatic plants. During this process employee was bitten by a snake (water moccasin) on the left foot, inside area. Employee was wearing waterproof boot waders and only one fang punctured through wader boot. This bite was determined to be a dry bite.
While using an overhead hoist to position the No.8 trummel for routine maintenance, the lifting hook pulled through the steel structure and struck employee on the right wrist. The injury became recordable on February 10, when the employee was treated with an injection of anti-inflammatory medication and placed on restrictions during an ongoing care visit with a physician
During a routine DOT physical, EE complained of right hand numbness and tenderness. EE stated that this event was over a course of time and thinks it was due to using impact tools and exposing hand to excessive shock and vibration. Incident elevated on 11/25/20 following a case management evaluation performed on 11/11.
As miner was ascending the dragline, the miner's foot slipped and EE twisted ankle and felt a pinch in left foot. MRI results received on Oct 19 showed an acute tibia fracture with a torn ligament. The results elevate the incident to a recordable medical claim.
While changing Drag cables a tractor was in the process of pulling the cables from the spool secured to a sled. As the employee approached the sled the last two wraps of cable released from the spool, striking the employee on the head and arm resulting in sutures.
Miner pinched the tip of EE's right middle finger as they were closing the door to the Payloader. The Miner had entered and was seated in the cab of the equipment, when they pulled the door closed they caught a finger between the door and frame of the equipment.
While in the process of loading a flatbed truck, the tip of a maintenance employee's left index finger was caught between a pump impeller and a shaft sleeve. The injury occurred when the employee lost their grip while lowering the sleeve into position next to the impeller.
While in the process of relocating tool lockers. One employee was assisting a forklift operator when they walked behind a locker after it had been placed on the ground. As the forklift operator retracted the boom, the fork tips contacted the bottom of the locker causing it to tip and fall backwards landing on the employees back. Given Injection, prescription & PT given 10/13/20
While changing out a drain tee on the pumping system the employee felt a pop in their left shoulder. Following injury case management the employee was placed on restricted duty and prescribed Physical Therapy on May 18.
While traveling through the mine employee became disoriented due to decreased visibility caused by fog and other factors. Employee failed to navigate a turn and drove off the mine road into a BMP clear water ditch. Employee was treated on 03/19 and released, upon a follow up appointment on 03/23 the EE was placed on restricted duty pending further diagnostic testing.

Employee lost balance and fell against the truck with left hand (pinky finger extended) causing a joint dislocation. Employee was placed in a removable splint.
Employee experienced pain while using a 3/4 drive impact. Employee was treated conservatively for tennis elbow. Company physician was consulted to review work relatedness and was confirmed on 2/10/20 that illness is work related. Employee has been placed on restrictions and physical therapy.
Employee complained of continual elbow pain on 9/30/19. EE was sent for MRI. On 11/07/19 Orthopedic doctor diagnosed employee with tennis elbow, prescribed anti-inflammatory medication and ordered physical therapy.
Employee felt discomfort in the left forearm while changing out rollers on the belt system. On 1/6/20 employee had follow up appointment with company NP who ordered OT since conservative treatment has not been successful.
5 man crew was working in a wetland harvesting plants to be relocated to a different reclamation program. harvesting began at approximately 8:30 am, around 10:45 am a snake approximately 8-10 inches in length bit employee on middle finger between the second knuckle and hand. Snake was submersed in water around the base of the plant.
EE stated was ""working under the trap, lifting a 2x2 steel bar (short piece) when I felt a pain in muscle in the right shoulder."" EE had MRI on 11/22/2019 showing that the EE has 2 complete tendon tears and the bicep tendon is completely unattached. The EE was placed on Restricted Duty at that time.
While employee was tightening a nut with a torque wrench the locking arm slipped causing the torque wrench to jump off the nut jarring the employee's right shoulder. At a later date employee complained of continuing shoulder discomfort. A MRI was performed on 10/15/19 and employee placed on restrictive duty 10/16/19.
Mechanic operating high reach lifted basket to back out of the tight area. The other mechanic in the basket was in the process of checking top side of pipe when EE became pinned between top rail of basket and I-beam structure. (As a result of the basket impact, the man lift could not be operated from the basket, requiring a nearby employee to operate the basket from the ground.)
Employee trying to free wedged pump between pump truck bed and pump. As Operator raised the pump, employee's thumb was caught between the truck bed and pump, resulting in left thumb injury

EE states that they placed block pipe down on block with the retention build up and it slid back and struck shin causing EE unable to move leg and fell back. Possible Meniscus Tear in left knee.
Employee was in the process of tossing 5# extension cord up one level on dragline and felt pain in left shoulder.
Employee was moving a piece of pipe on a trailer and EE's right hand ring finger was injured due to impact between the pipe and the steel headache structure on the trailer.
EE presented with swelling and pain in LT wrist. Upon seeing doctor EE was diagnosed with Carpal Tunnel. EE is scheduled for surgery but has no work restrictions at this time
EE & co-worker were tasked w/applying herbicide to plants & were using ATVs to access the site. EE became stuck. EE walked up to co-worker's ATV & tried to play out the winch line. After finding it wouldn't play out EE placed middle finger of lt hand in winch line hook & asked co-worker to run winch out. They didn't use remote winch controller & EE didn't use safety pull strap.
Electrician was troubleshooting the limit switch circuit on #8 control sled on #20 dragline pumping system. Electrician entered the main disconnect cabinet to trace the wiring for the circuit. While standing on 4 ft step ladder, electrician reached toward back of cabinet to twist the label on the wiring for the limit switch to read it & experienced electrical shock.
EE was throwing a switch at the bottom of the rail yard at Agrock when ee felt a sting in left shoulder.EE noticed stiffness to upper shoulder and when raising arm, ee applied cream but stated it's slightly worse. EE had MRI on 5/1/18 at which time was determined to be work related and put on restrictive duty.
Employee walked by scaffolding, rubber flashing fell off overhead work area and struck employee on left arm.

12/6/17 Date of injury, right knee pain after stepping down off a ladder. 1/22/18 Follow-up and referred to orthopedic doctor, dx from MRI, ACL tear, medial meniscus tear.
EE and co-worker were carrying a cross tie. Co-worker dropped one end causing EE to drop the other end on right foot where steel toe ends. EE had x-ray and right toe was fractured. Foot was taped up. No work restrictions at this time.
Employee bent over to pick up a welding stinger weighing less than 5 pounds when they felt a sudden and sharp pain in the lower right back area. Employee was unable to put any weight on their legs. Resulting diagnosis was Sciatica.
Employee walked onto ramp to enter office trailer when EE was bitten by a snake (water moccasin).
EE reported tingling and numbness in right hand on 8/15/17 to supervisor. EE has operated Heavy Equipment for over 40 years, had Carpal Tunnel Surgery 19-20 years ago. After being seen by a specialist on 10/4/17 it was determined that the illness was work related and EE needed Carpal Tunnel surgery again
Employee ascended the stairs on a Komatsu500 loader. Employee was cleaning the loader. Employee PPE:boots,hi-vis,earplugs,hardhat,goggles,textured nitrile gloves. Task required a different implement. Employee said their goggles fogged when they reached for the handrails to descend,causing them to miss the rails and fall,resulting in broken vertebra,L5 and T12 rupture.
On the above date, time and location, Security Officer was working the main gate of the Four Corners Mine when EE's chair hit an indentation in the floor causing EE to fall from the chair. The arm of the chair hit EE in the chest. EE did not report the incident at the time of the incident.
The employee brushed hand on the face of the flange on the gun sled in order to clean off the dirt. In doing so, EE cut right pinky finger on a piece of metal on the flange.
Employee was using a machete to cut Invasive plants in a wetland area. Employee stumbled in muddy terrain causing the machete to come in contact with left knee cap. Employee received approximately a one inch laceration as a result from this impact.
Employee was operating a bulldozer watching other employees put together a 24"" joint of pipe. EE exited the tractor and was walking toward the employees to assist in the task as they flagged the rubber tire operator to let the pipe down. When the rubber tire operator let the pipe down, the pipe shifted forward striking the employee in the lower leg causing EE to fall to the ground.
Tractor operators were working the tailings discharge line. The poly pipe at the discharge started to slide down the hill at the discharge. The tractor operators moved to hold the pipe in place with their blades. As the pressure on the line increased, it slid out from under the tractor blades and struck tractor
EE unloaded a bottle of argon on 1/19/17 and felt a ""twinge"" in shoulder. Reported to supervisor on 2/8/2017. EE seen by doc and diagnosed on 3/20/2017 for shoulder sprain. Delay in reportable due to task and injury evaluation to verify work relatedness by EHS and Medical Director.

Mechanics had removed the South Scalp pump shell when a come-a-long securing the discharge pipe failed causing one end of the line (10"" diameter, approximately 20 feet in length) to swing downward and strike one of the mechanics in the head and shoulder area. Incident occurred on 12/08/2016. Case became a recordable with restricted duty and a prescription on 12/15/16.
EE struck self in the face with a pry bar. EE reports EE was pulling back when the pry bar slipped causing it to strike EE in the face. EE sustained a laceration on EE's left eyebrow. EE reports EE was slightly dazed after the impact. When EE stepped down off of the pedestal EE rolled EE's left ankle
Contractor employee was loosening a nut on a U-bolt in order to remove a section of 3"" stainless steel pipe. The nut broke loose unexpectedly causing right forearm to strike the sharp edge of a water nozzle located on the bottom side of the pipe. This caused a cut that required sutures.
EE hit elbow on a guard rail 8/3/16. EE continued to work full duty without difficulties. EE was seen in the clinic on 9/12/16 for continued right elbow discomfort. On 9/14 was seen by Dr.EE was prescribed an anti-inflammatory and helio pad for the discomfort. Dr. cleared EE to continue regular duties so EE is not on any restrictions at this time.
A trespasser died on mine property. The driver lost control driving truck, struck the toe road berm, partially ejecting the driver and pinning driver under the truck.
Employee was performing work/labor in hot outside environment. Employee became dehydrated causing employee to suffer from heat exhaustion.
EE was operating a dump truck on a dyke road of the F-9 Phase III dam. EE had traveled this road over 100 times. While traveling in the middle of the straight run, EE turned the truck hard left causing the truck to flip off of the road & roll approximately 28 feet. There were no conditions that contributed to the accident, other than driver negligence. EE received serious injuries.
The EE experience pain over time. EE went to EE's personal dr. 1/2016. EE had some tests done. Results indicated EE had Bilateral carpal tunnel. EE was advised to report this to employer. Reported on 2/8/16. Employee was evaluated and an appointment was made to see specialist on 3/10. The injury was determined that work was 80% of cause. Injury demined recordable on 3/10/2016
Employee was moving bike racks from under railroad belts. When EE got back into sweeper to sweep under belts EE noticed a sharp pain in lower back.

Mechanic was tightening a packing gland on log washer when the wrench slipped, pinching his finger between the bearing assembly and the wrench. Employee was treated with first aid at on-site clinic and sent to ER for sutures.
Herbicide spraying - EE assisting local crew for 1st time. Crew has sprayed for 8 years with no incidents. Insect sprays (natural & DEET) applied to skin. Some herbicide contacted skin on arms. Symptoms included itching & hives. Antihistamine injected at clinic; symptoms disappeared. History of skin irritation & allergies. Diagnosis = allergic & chemical or environmental exposure.
Employee was standing between a scaffolds bin section. The employee lost his balance and fell hitting his right leg on the edge of the bin resulting on a fracture about 6"" above his ankle.
Employee was found unconscious, waiting for medical results, possible dehydration, heart condition, or other. Employee regained consciousness when found by other employee. Doesn't remember what happened. Employee is currently returned to work and doing well.
Employee was walking up to dragline to make relief when he went to pull himself up on the stairs of the dragline. Employee felt a pop in left knee. Injury became recordable 9/29/15 when MRI revealed a torn meniscus.
Employee was welding and felt debris enter eye. Reported to supervisor and washed out eye. At 2:30 am taken to hosp. in Bartow, Fl. Employee returned to Tx. on 6/10 and went to doctor in Tx on 6/11/2015, 6/12/2015 and was released for work on 6/15/2015. The employee did not lose any time.
Employee was lifting and moving nitrogen compressed gas cylinders for a fire suppression system inside a dragline. The awkward positions due to tight conditions inside the dragline and the weight of the cylinder contributed to the employee straining his back.

Mechanics were changing out motor on south coarse feed pump. Job was nearing completion and they were using a string to align the motor and pump sheaves. As one mechanic stepped up on pump base, an unsecured piece of the sheave guard slid off the pump, striking the employee on top of the left foot just behind the steel toe.
A fire occurred within rubber piping at the washer during maintenance activities. The fire started after bolts that were being burned from a different section of pipe made contact with nearby flexible rubber pipe. EE checked by nurse & EMTs. Employee was sent to Lakeland hospital for further checks. Employee's oxygen level was low, and he was administered oxygen.
Employee slipped from the second step of a maintenance truck, resulting in his left shin contacting the step causing a laceration.
Incident is under investigation.
Employee was working up in the bin area when he felt some cramping. After descending the stairs, employee's condition worsened and he was feeling severe cramping in his whole body. Employee was given fluids and taken to the clinic area.
Employees were removing a wooden box from the bed of a pickup truck for disposal. The top was laying beside the box and as the employees attempted to place the top on the box, the injured employee's right forearm was struck by the sharp end of a deck screw that was protruding from the wood.
Employee was working on top of a floater platform that was pulled from the C/W ditch and sat on the ground to change out a 30 amp air cooled sealing water motor for the Z North #2 TLP. Employee finished installing the motor, stepped back and lost her balance and fell about 2.5' to the ground.
While operating engine #3802 the engineer was bit by a spider on his lower right side. This was on Sunday night and he reported it on Monday night.
Previously reported as first aid-""As employee was throwing 2-1/2"" x 15' sealing water hose off the truck, felt a pain in his right shoulder."" Injury became recordable when employee had surgery.
Employee injured his right thumb while making an adjustment to the forks on an off-road forklift in preparation for a lift. The fork came off of the support saddle and swiveled pinching the employees hand between the base of the fork and bottom part of the carriage.
Injured employee was a passenger in a pickup truck of which the driver lost control of the truck just south of the 18 BMP ditch. He drove through the south side BMP berm and landed on the North side of the BMP water ditch.
The employee was repositioning cribbing when his thumb was caught between the boom point and cribbing causing a fracture.
While inspecting dragline 19 bucket employee went to step over a patch of high ground and right foot slid into low spot twisting leg and knee.

Employee was in the process of removing a bracket that supports wire inside a switchgear cabinet. While pulling on the old bracket it unexpectedly broke causing the injured employee to strike the back of his hand on a small bolt that was protruding through an adjacent hinge.
Employee was walking up a ladder when his retractable locked up causing him to lose his footing. He fell backwards off of the ladder and fractured both legs.
Employee was working in a wetland extracting herbaceous plants and was bitten on the right hand by a snake.
On 5/5/2013 injured felt like something blew into his eye. Eye was flushed but incident not reported until next day. Employee was sent for X-rays and examination which showed no foreign body in the eye. Employee was prescribed an antibiotic with a diagnoses of Corneal Abrasion with Conjunctivitis and Sclerosis in left eye.
Injured was sliding a hydraulic unit from truck, the unit toppled to the left snatching the left arm and shoulder of the employee. Injured treated In-house for severe shoulder strain; pain reoccurred 5/23/2013 exam was performed and MRI & Cervical X-Rays ordered. Employee placed on restrictions, Rx given for pain and muscle relaxation.
Employee was walking back to the strainer and slipped on some mud/algae and fell to the ground. He landed on his left side.

Employee started having chest pains co-worker took him to the site nurse. The site nurse took hiss vitals and called the ambulance. The paramedics check him out and he refused to go in the ambulance. He called someone to pick him up and take him to the family doctor.
Injured employee was working in the area of a field switch gear when an arc flash occurred and in the process of evacuating the area tripped and injured his knee.
Injured employee was standing on top of the gearbox pedestal and was using a pry bar to separate the coupling halves. While he was applying pressure to the coupling, the pry bar came loose and employee fell backward approximately 3'. When he landed he struck his head against some spare idlers that were staged for a separate job. This resulted in a 3' gash to the top of his head.
EE was called to #2 MLP on the 26 side to reverse motor for maintenance to remove impeller. After he changed motor back he closed the motor disconnect and an arc fault occurred inside the switchgear cabinet. The arc blast blew the door open. EE was standing away from the door but received minor burns.
Two E/I Technicians were re-installing the motor leads to the # 4 primary log. They hoisted the j-box into place and secured it. They then rigged the hoist to the 90 degree conduit fitting and were in the process of returning it to the support bracket when the employee attempted to roll the fitting into place and felt a pop in his right elbow.
EE was cutting rebar steel when the handle of the tool slipped, causing him to lose his balance and twist his right ankle.
While moving a trommel screen with a 20 ton overhead hoist, the trommel turned and the eye-bolt pulled through the urethane screen. A release of stored energy resulted in a laceration to the employee's chin area.
After bending over to measure grating, injured EE stood up and felt lightheaded and dizzy. Employee fainted, collapsed, and struck the knockout drain frame on the log washer. Bruising was apparent. EMT was contacted. EE was transported to the hospital for X-Rays.
While installing the drive coupling on new bearing unit, the taper lock bushing was spread with a small steel wedge. After positioning the bushing on the shaft, the employee started to tap the wedge back and forth in order to remove the wedge. With the first tap, the wedge flew out striking the employee in his upper lip area.
Employee tripped in feed product landing on his hands, after draining choked sump. Employee said hands and wrists where numb, first responders checked him out then ambulance came to check him out further.

Employee was cleaning up the lab area when she tripped on a rubber mat and fell to the floor landing on her right shoulder.

Employee was disconnecting cable from Junction Box on Electric Sled when he felt a burning sensation in his groin area as he was pulling out cable.
While repositioning a buried high voltage power cable employee stepped into a hole causing him to lose his balance. The employee fell to the ground injuring his right shoulder. The injured was treated on 11/1 as a first aid injury. Employee was given prescription medication on December 19th at a follow-up appointment making this a recordable injury.
Employee was using a socket universal on a air impact to remove 1"" bolts, as the mechanic was removing one of the bolts the swivel pin on the universal dislodged and the universal came apart in the mechanics hand, causing a laceration to the inside of his right ring finger.
When changing out 45ft 22in pipe being lifted into place with a 60 ton crane. The injured employee pulled the tag line to position pipe the pipe swung past the attachment point. The injured grabbed the pipe with his left hand to stop the pipe and his left index finger was pinched between the pipe and the hand rail.
Employee was performing work/labor in hot outside environment. Employee became dehydrated causing employee to succumb to heat exhaustion.
Employee was disconnecting the #6 sealing water pump electrically for relocation. The cable associated with the pump was partially buried. As the employee was pulling on the power cable, he experienced pain in his lower back.
Injured employee was holding a 2 1/4"" wrench on bolt head supporting the middle of the wrench with left hand. He was holding the end of the wrench with his right hand. When the torque wrench was engaged the hand wrench turned pinching employee's left thumb between the wrench and pump pedestal.
Employee was using a 1"" drive impact wrench un-bolting a flanged coupling on a 24"" joint of pipe. He was loosening the nut when it flew out of the socket striking the employee on the right side of his face above his lip.
Employee was working in the plant area under one of the concentrate bins. Employee went to dismount from the rear of the truck (FL70) step onto a brace bar then to the ground. When he placed his right foot on the ground his foot slipped and caused his right ankle to roll over. Employee then went with body motion and fell to ground.
On road truck driver was exiting mine on paved entrance road with a fully loaded dump truck. Truck wheels dropped of right shoulder, driver steered back to the right, lost control and overturn on the right side. Driver sustained non-life threatening injuries to right shoulder and ribs. Vehicle was damaged extensively. Driver is being treated for injuries at local hospital
IW was removing a 167lb cutting edge from the cab of a dozer (Komatsu D275AX-5). The cutting edge slipped from his grasp and landed across both feet behind the steel toe portion of his boots.
Employee was walking across a dry non-skid floor in the sample chopping area of the laboratory and the toe of his steel-toed boot caught the floor prior to completing a full stride step causing him to fall to the floor. During the moment of falling, he extended his arm to catch himself. He landed with his full weight on his right elbow.
The injured employee was holding the head of a bolt with an end wrench while another employee was getting an impact wrench in position to remove a nut. The air hose caught on work platform causing the air hose to break away. The male end of the coupling hose struck the employee in the mouth breaking 2 teeth and cutting his lip.
Employee slipped while descending stairs and fell down 3 steps landing on asphalt.
Employee was trying to hoist overloaded trash bag into dumpster with right arm while holding the dumpster lid with left arm. On the employees drive home he began to feel pain.

While Employee was walking down the stairs of the 19 dragline he missed a step causing his right leg to miss the rest of the steps; hitting the dragline shoe. When the right leg slipped, employees left foot got caught in-between two steps. Employee was stuck for a couple of minutes and then was able to get himself up.
EE smashed right thumb on existing flange of a pipe, while he was guiding the replacement pipe into place.
Mechanic was driving freightliner 089 truck to 18 dragline for repair day, when he felt something hit him in his left eye. When he arrived to the dragline he noticed his left eye was irritated and he immediately notified his supervisor.
Employee was adjusting the packing on the #7 MLP on the 19 MTX line. After completing the task, the employee was exiting/dismounting the pump platform using the steps provided. Upon stepping off the last step, employee lost her footing and slipped backwards hitting the ground with her lower back.
Employee was dismounting locomotive with remote control in her right hand. She was facing the machine and backing down the stairs while carrying the remote control box. By carrying the remote she did not maintain 3 points of contact. As she went to step back to dismount backwards from the last step, she lost her balance and fell onto remote control box.
The injured employee finished guiding a reel of ground wire onto the back of truck. The injured employee descended down from the bed of the truck down to the first step (driver's side). The employee felt a sharp pain in his left knee when he stepped down with his left foot to the ground.
Employee cleaning in the lower section of the 18 D/L in the #3 Swing Compartment around 11:30 am. The employee was mopping in a front to back motion facing the #3 swing shaft and standing on the floor man-way cover. Employee reported radiating pain down his arm on 2/17/10 and was treated by company doctor for Tendonitis. Initial treatment helped but over the course of the past few weeks pain radiated down both sides of employees arm. Doctor re-examined employee on 4/14/10 and employee was give prescription medication and placed on restricted duty for at least one week.

WHILE GUIDING OUT WINCH TRACTOR CABLE; CABLE TWISTED HITTING INJURED EMPLOYEE
Employee was walking on uneven ground, fell and twisted her right ankle.
While adjusting the packing on a pit pump employee lost his balance and fell off the pump hitting his back on the step and scraping his left arm and top of his head on the pump.
EE cut middle finger of the left hand with a pocket knife while cutting plastic banding straps that held boxed bolts to a pallet.
While removing chain link fence sections from our contractor yard, a section became entangled in a nearby palm tree and while trying to dislodge the fence employee fell to the ground and injured shoulder. The eventual outcome was a torn ligiment. Based on an internal audit, the accident was inadvertantly reported on our OSHA 300 Log in error
While pulling power poles out of the ground, employee used a hot stick to push cable above center of gravity. Hot stick got hooked to cable and went above employee, released and the butt of the hot stick fell and struck employee on the nose.

Employee was preparing some floor plates for welding by beveling and grinding. The employee would manuever the plates to gain access to the outer edges. While moving the plates he felt a sharp pain in the groin area.
Tightening bolt with wrench and wrench slipped off bolt. Handle of wrench struck employee in the mouth resulting in minor chip to edge of tooth.
Injured EE fell between trommel 6 and 5 approximately 15-20'. EE was able to work his way out the #5 secondary flat screen flop gate. EE then pulled himself onto the catwalk and was able to work his way to the elevator where another EE assisted in getting the injured EE to the dispensary for help.
Employee had been using a grinder off and on since he was hired on 2/6/2008. He stated that he was feeling some pain and discomfort in both of his arms and wrists.
While employee was maneuvering a flatbed truck through water the front wheels hit a washout, causing the steering wheel to turn sharply striking driver's left wrist and hand area.

Injured employee was closing the gap on the forks on the Sky Trac Forklift. The forks are designed to be slide back and forth on the round fork support bar to accommodate the application. The employee had raised the end of the fork to make the relocating process easier. When he lowered the end of the fork, his finger was caught between the fork and the frame mount.

Injured party was riding in passenger seat of rock truck when the driver lost control. The truck turned on its side. Accident took place on paved road near mine exit onto Route 37. Employee seen by doctor and taken off work for three days pending follow-up visit.
Tractor operater shut down machine so service truck could service machine. Employee climbed out of cab was walking down tracks of machine when he slipped falling 4' to the ground landing on his left shoulder.
Employee stepped out of float truck on unlevel ground, injuring his ankle.
Employee were changing out the hoist cables, when the chain securing the snatch block to the forward pad eye holding the house winch cables broke. The hoist cables stretched tight, making contact with the platform in between the drag and hoist drums breaking the platform loose. At this time the employee was thrown against the drag drum, handrails, etc. injuring the employee.
Employee was lining up bolt holes on pipe using a spud bar. Bar rolled, shifting pressure to left side. Employee felt pain in left shoulder and neck.
Employee lost his balance while positioning a 6"" trash pump flexible suction. The suction piping pulled him down causing him to hit a ball valve handle cutting him above his eye.
The injured employee was attempting to turn the 45 degree fitting as it was supported by a 4 Nylon strap when he felt a slight pull in his right neck area.
