EE is believed to have been either chipping away several inches of top layer of brownish colored granite or drilling a chain hole through the granite near end of the block on the open face so block could be lifted using a chain. Struck by flying object
Employee was using a sledge hammer on a wedge. A portion of the hammer sheared and struck EE in the forehead causing a cut.

Employee was emptying dump truck when stone inside the truck bed hung up. When it released, the truck wheels jarred the ground with employee inside the cab.

Employee burned finger on right hand while lighting air fuel burner, quantifying a minor burn.

Fall to the walkway or working surface
Employee failed to recognize the saw block tipping over, which was very out of normal. The method used was the same as all routine extractions. He was able to distance himself enough to avoid serious injury, but was hit by the drill and dogs, knocking him down and causing hip fracture and arm lacerations.
