A ROOF FALL OCCURRED ON THE 3RD NORTHWEST MAINS BETWEEN THE #1 & 2 ENTRIES. THE FALL IS 1 BREAK OUT BY SPAD # 1863 #1 ENTRY. THE FALL IS APPROXIMATELY 6 FT. HIGH AND IS APPROXIMATELY 20 FEET L ONG. ADDITIONAL ROOF SUPPORT (CRIBS) HAVE BEEN SET. THE 2ND LEFT #4 SEAL CAN STILL BE MADE.
OVER A PERIOD OF TIME FROM SMALL CUTS BY HANDLING DRILL STEEL, ROOF BOLTS AND PLATES, THE EMPS LEFT HAND BECAME INFLAMED. THIS WAS DETERMINED 11-8-96, BY DR AT HOSPITAL.

EE WAS WORKING AS A CONTINUOUS MINER HELPER. HIS BOOT GOT CAUGHT ON THE BUMPER OF A SHUTTLE CAR THAT WAS TRAVELING FROM THE MINER. HE COULD NOT GET HIS BOOT LOOSE AND WAS DRAGGED SEVERAL FEET.

THE REPAIRMEN WERE CHANGING A TIRE ON A SHUTTLE CAR ONE WAS USING A PIECE OF METAL TO LOOSEN THE TIRE A SMALL BIT OF METAL FLEW INTO ONE OF THE MENS EYE
THE MINER CONVEYOR RAN OUT OF BOOM WHEN A ROCK GOT IN UNDER IT. THE CONVEYOR RAN OVER ON TO THE TOP OF THE MINER. A BUCKET OF MINER BITS HIT THE MINER OPERATOR.

EE SAID THAT WHILE HE WAS RUNNING THE MINER THAT A SHUTTLE CAR HIT THE MINER CAUSING HIS BACK TO HURT HE HAD QUIT THE JOB BEFORE TELLING THIS BECAUSE OF DIMOTING HOW TO MINER OPERATOR.
ROOF FALL OCCURRED #7 ENTRY 180'LONG. WEAK ROOF STRATA IN #6 & #7 ENTRIES WAS DETERMINED TO BE THE CAUSED OF THIS ROOF FALL. VENTILATION ON SECTION NOT BLOCKED.#6 & #7 ENTRIES WERE STOPPED AND CRIBBED OFF.
THE EE STATED THAT HE WAS REMOVING A LARGE ROCK FROM THE OUTSIDE HOPER WHEN HE HURT HIS BACK. THE EE DID NOT REPORT THIS TO THE SHIFT FOREMAN BUT STATED THAT HE DID TELL A FELLOW EE HE WENT TO THE DR ON 9-28-91 AND TURNED THIS IN ON THE INSURANCE. AFTER THE EE WAS ASK TO MOVE TO ANOTHER LOCATION HE WENT TO THE DR. THE EE WORKED UNTIL 10-17-91 AND DID NOT REPORT TO HIS FOREMAN.
