THIS WAS AN UNINTENTIONAL ROOF FALL DUE TO FOLDER SEAM OF COAL OVER STRATA. THE ENTRY WAS FULL BOLTED AND SUPPORTED CALL OCCURRED ABOVE ROOF BOLTS THIS WAS REPORTED TO MSHA IMMEDIATELY AND ALL THEIR RECOMMENDATION WERE COMPLIED WITH.
