EMPLOYEE WAS NOT FOLLOWING COMPANY POLICY AS HE WAS CLEANING RETURN ROLLER ON BELT/ CONVEYER 310/ EMPLOYEE KNELT DOWN WITH HAMMER AND MISSED. HIS HAND WAS PULLED BETWEEN ROLLR AND BELT. HE THE N PULLED E-STOP CABLE TO STOP CONVEYER. EMPLOYEETREATED AT E.R. FOR 5 STITCHES AND A FRACTURE OF THE TIP OF THE MIDDLE FINGER OF THE RIGHT HAND.

WELDERS WERE REMOVING I-BEAM FROM THE #3 BLENDING TOWER, A BOLT THAT WAS CUT OFF FELL INTO THE 5X16 BUCKSHOT VIBRATOR AND IGNITED THE BALCK RUBBER SCREENS. UNABLE TO PUT OUT FIRE WITH FIRE EXT INGUISHERS. FIRE DEPT WAS CALLED. FIRE WAS EXTINQUISHED AT 11:45 A.M. TWO VIBRATING JIGS WERE DESTROYED. PRODUCTION PLANT WAS CLOSED ON 11-5-99 AND WINTER MAINTENANCE WAS IN PROCESS.

MOVING POND MATERIAL-HE TOOK TOO MUCH MATERIAL IN THE BUCKET RAISING THE BACK OF THE LOADER OFF THE GROUND. WHEN THE LOADER CAME BACK DOWN HE HIT HIS CHEST ON THE STEERING WHEEL. HE WAS WEARIN G HIS SEAT BELT.

SAND PLANT IS SHUT DOWN FOR WINTER MAINTENANCE. HE WAS USING AN AIR CHIPPER TO REMOVE SCALE FROM AN I-BEAM. HE WAS WEARING SAFETY GOGGLES AND A SURGICAL MASK. THE SURGICAL MASK HELD THE SAFETY GLASSES UP 1/8" WHICH ALLOWED ITEMS TO REACH EYE. PURCHASES HEAD SHIELD TO PREVENT REOCCURRENCE.
