FRONT END LOADE9 WAS LOADING ROCK, OPERATOR WAS BACKING UP ENGINE ON LOADER QUIT, LOADER WAS ON INCLINE AND STARTED ROLLING BACKWARDS. OPERATED THEN DECIDED TO LEAVE LOADER WHILE ROLLING BACKW ARDS, FALLING O3 WHEEL CAUSING EMPLOYEE TO BE VRUSHED TO DEATH.
