CREW WAS CHANGING SCREENS. BACKHOE WAS LIFTING SCREEN FROM THE PIT FLOOR ONTO A CONVEYOR. CHAINHOLDING SCREEN CAME UNHOOKED. THE HOOK ON THE END OF THE CHAIN THEN FLIPPED AND HIT TERRY IN THE RIGHT HAND THEN IN THE CHEST
