Victim was part of a crew that was welding hdpe pipe using a McElroy fusion machine. While attempting to position pipe in to fusion machine, using an excavator, the pipe struck the victim in the head.

Employee badged in through turn style was walking towards bus stepped on a rock fell forward to EE's knees
Employee was tasked with installing a vulcanized splice in 17-6 conveyor belt. As part of the assigned task, employee tripped which caused them to fall forward and strike their mouth against the cooker. Contact resulted in a broken tooth and laceration to employee lip.
Maintenance Technician (M1) was tasked with installing the track on a Bulldozer. M1 used a hammer to align the track master link. During master link alignment M1 felt flying debris enter their right eye. M1 was transported to the onsite clinic for evaluation and then transported to an offsite medical facility.

Effected employee (EE) was helping load a grease barrel on a fuel truck when another employee was on top of the truck pulling the top of the barrel causing the bottom of it to slide out and pinch EE's left thumb under the barrel and deck of the truck. The barrel was 16 gallons and weight approx. 120 lbs. Weather conditions were gloomy and misting rain.
Operator in Training (O1) was tasked with wash down duties on the north side of the Mill Washer Building. While performing the assigned task O1 used both hands to close a 2"" ball valve with an oval handle. While closing the valve, O1 felt a pop in their left ring finger. O1 was evaluated at an offsite facility, where imaging revealed a chipped bone in the finger.
Employee was arc gouging bulkhead weld seam off Matrix pump sled. While cutting the last of the weld seam the wall shifted pinching a finger between the floor and head of the gouging torch. Employee received laceration to right second finger. Employee was transported to Nutrien site first aid office and then ER and returned to work on 08/22/25.
Employee was welding and tacking troughs in Enviroclear tank. EE took a break, 5-6 minutes, kneeling in front of fan to drink gatorade. When EE went to stand up, EE passed out.
Equipment operator stated EE wasn't feeling good and asked to go home. Operator missed two days the week prior with the flu. After operator badge out and left active mine area to personal vehicle area the operator entered EE's vehicle at approximately 7:51 and was found diseased in EE's vehicle approximately 6:15 p.m.
Employee alleges that EE strained EE's lower back while performing repairs on an excavator. EE did not notify management until 2/10/2025. EE states that EE was handling a started when the event occurred.
Mill Maintenance Mechanic (M1) bent down on their right knee to observe the rotary gate valve at the #6 calciner. M1 began feeling pain in their right knee as they attempted to stand back up from the kneeling position. M1 was transported to the onsite clinic and was later transferred to an offsite facility for additional evaluation.

Maintenance tech. was in the process of replacing an air cannon @ the #2 gate east side. M1 had a ratchet in left hand and a 15/16 wrench in their right. The ratchet slipped off the bolt and struck M1 on the bottom lip. M1 immediately reported to the supervisor & went to the first aid clinic. M1 was carried to an offsite medical facility and received three dissolving sutures.
Mine conditions were favorable with no adverse weather conditions.
EE was operating a Porta Batch Machine for a company, Lhoist, with whom we have a contract on the Nutrien Mine site in Aurora, NC. During the operation, EE saw a bubble forming in the rubber hose and went to change it out. The hose burst & the lime mixture struck EE in the face/eyes.
C1 was assigned wash down duties. C1 placed a nozzle on the hose and began opening the water valve. As water pressure increased the hose nozzle moved upward, striking C1 on the right side of their face.
Haul truck operator misjudged distance of approaching haul truck and proceeded to enter the haul road causing a front glancing collision of two haulage trucks.
Operator 1 (O1) was tasked with placing stakes in the ground to identify dig limits near the dragline. O1 was using a steel hammer to drive stakes. O1 was holding the stake with their left hand while holding the hammer in their right hand. O1 struck their left index finger with the hammer which resulted in a laceration. O1 was transported to an offsite clinic for evaluation.
Operator (O1) was tasked with assisting with repairs being made to a screen at the Washer Building in the Mill-Flotation area. As part of the assigned task O1 was installing a wooden wedge on the side of the 14 mesh screen. During installation, the wedge failed pinching O1's left thumb. O1 was transported to an offsite facility for evaluation.

The technician was exiting the bed of service truck with a filter in one hand and sprained an ankle when their foot landed on the ground. Video reveals that they were not using 3 points of contact on descent and the area was muddy.
Mechanic (M1) was tasked with working with an automated welding machine. During welding activities M1 began experiencing pain/itching in both of their eyes. M1 was diagnosed with flash burns and received prescription medication for both eyes.
Temp Employee (T1) was tasked with performing wash down duties in the Mill area. A sudden disconnect of the wash line caused T1 to lose their footing and slip/fall. T1 struck their head resulting in a laceration. T1 was transported to an offsite medical facility where they received sutures.
Fuel technician fell 3 to 4 feet to the sand while fueling a Bulldozer. Policy was not followed; dozer was not shut down and key removed. Bulldozer operator seated inside cab bumped the forward shifter casing the Bulldozer to move 6 inches forward. Fuel technician did not follow proper egress and climbed the Bulldozer track from the rear of the machine.
A contractor truck driver (O1) was fueling a haul truck at the Mine Services fuel island (Tank #6). O1 stated that while lifting the fuel nozzle up on the truck's catwalk above their head, O1 felt a sharp pain in their shoulder. O1 then contacted Supervisor. O1 was transported to local medical facility for evaluation.
Haul truck operator (E1) used right knee to hold hose/nozzle in place while fueling haul truck. After completing fueling and resuming hauling duties, E1 began to feel pain and noticed swelling in right knee. E1 notified supervisor and was transported to local medical facility for evaluation.

An employee (E1) stepped out of the truck onto concrete, unaware it was icy. E1 slipped and started to fall. E1 landed with left arm extended straight out and felt instant pain in the left arm. *Fracture diagnosis was found 1/4/2023.
Operator struck a dead tree with bucket. Top of dead tree struck Excavator, bounced and broke top glass. Part of tree top entered cab but did not touch operator. Operator closed eyes and did not see what happened. Operator stated may have lost consciousness/doesn't remember. Two medical professional examined operator, no sign of head trauma or loss of consciousness.
Employee was correcting a vibration in a screener unit. Tech noted the issue was a loose bolt on the shaker box cylinder rod end. While attempting to pry the the cylinder enough to allow the R&R of the bolt, the bar slipped and pinched the employee's left index finger, lacerating the tip
A Mill Maintenance Technician was in the process of disassembling #1 & #2 ITV (Internal Transfer Valve) on the #1 Calciner. While performing the task dry calcine rock dust fell from overhead. The technician felt something in their left eye.
Concrete labor was filling fiber form bag with grout using the nozzle of a pump truck. During the process the concrete labor got grout liquid on legs. On 10/19 the concrete labor noticed an itch to their knees, on 10/20 the labor reported a slight redness in the itch area. On 10/24/22 the concrete labor reported the need to see a doctor for redness and a possible chemical burn.
EE was attempting to move an approximately 3' x 1.5' x 1"" thick piece of steel plate out of work area. They attempted to lift one side of the plate and slide it out of the way. As they attempted to slide the plate, they lost their grip and the plate fell on the metatarsal area of their left foot.
Mechanic (M1) and Contractor employee (C1) were tasked with replacing an expansion joint. M1 removed the last bolt, and pushed against the joint to dislodge and lower to the floor. The joint came out it snapped back pinching M1's wrist between the expansion joint and pipe flange.
Employee was tasked with removing screening panels held in place by wedges. While trying to remove the wedges with a hammer, the employee's hammer missed the intended target and struck employee's left thumb.
While flying a welding machine down from an elevated platform, the load shifted. The employee's hand became pinched between the welding machine and an adjacent I-beam.
Operator (O1) noticed conveyor rubbing against structure. O1 determined contact was caused by material build-up near bend pulley. O1 used hammer to remove material build-up. O1's hammer made contact with the pulley, resulting in both the hammer and O1's hand being pulled toward the pulley. Fractured and lacerated left ring fingertip.

WHILE WORKING IN MILL MAINT. SHOP EE WENT TO REMOVE A PIECE OF METAL SHAVING FROM THE VALVE AND IT CUT THROUGH EE'S GLOVE. EE SUSTAINED A CUT ON EE'S RIGHT HAND 4TH (RING) FINGER.
After cooler Cyclone Dust Box experienced a structural failure, as an apparent result of a rapid expansion of the internal atmoshere.
While physically assisting in moving a track, employee felt strain in groin area.
Operator was digging into an Ore stockpile loading a screen plant. At approximately 2:26 a.m. the Ore pile peak shifted from East to West and slid down striking a 336 Excavator and a Screen Plant. During the slide the Ore entered the front glass of the Excavator covering the legs, waist and lower chest of the operator.
An associate, smashed the end of a finger in the process of removing a bolt from an existing steel pipeline. Once the bolt was removed the pipe moved unexpectedly. Associate's finger was caught between the cable sling and the pipe.

Employee was placing rope over a tooth on the bucket of an excavator. While employee was positioning rope, the excavator operator started to curl the excavator bucket. This movement caused two fingers on the employee's right hand to be caught between the bucket tooth and the rope.
Employee was getting the scale off of the calciner wall with a Johnson Bar when a large piece broke off causing the bar hit the end of EE's index finger and dislodging EE's fingernail.
Employee was removing a carton of parts from a cart and twisted EE's knee when EE turned to walk away. EE did not realize EE's foot was caught under the cart.
Employee was raising the side gate of a flatbed truck at the same time a forklift was pulling away from the truck. The tire of the forklift struck the gate and the employee's hand was caught between the tire and the truck gate resulting in a laceration to the right index finger that required sutures.
An associate of HDPE, Inc. was injured by flying debris while dislodging a rock from a tire. EE was taken to a local medical facility and released

During the installation of a bin pulley on 70-4 an employee (E1) attempted to reposition the bearing. E1's right pinky finger became pinched between the frame and bearing. Contact resulted in a fractured finger tip and a laceration which required two sutures to close.
As employee (E1) was descending a stairway, while exiting a building, they over stepped the bottom stair, catching the edge of the step with the heel of their left foot. As forward motion continued the left foot began rolling outward causing E1 to fall to the ground. E1 was transported to a medical facility where x-ray's revealed a break and fracture of left ankle
Equipment Operator (E1) had been assigned tasks involving the operation of a D6 dozer. While dismounting the equipment E1 placed left hand on the right hand rail which had some spots of grease on it. E1 slipped and fell making contact with the dozer blade. E1 reported pain in left shoulder.
Technician was removing an idler pulley from a bulldozer and was striking the pulley with a sledge hammer. A shard of metal was ejected and it struck the technician penetrating the technician's leg. The metal was imbedded in the leg and had to be removed by a medical professional.
Employee rigged a 90 degree elbow of a piece of pipe to be removed using a come-a-long. The employee affixed the rigging device by wrapping the chain and hook around the pipe. While trying to remove the pipe, the pipe shifted, and pinched the employee's finger between the pipe and the chain. Employee sustained a fracture and laceration to little finger of right hand.
While pushing out a pin in the boom dipper with a 200 ton hydraulic jack one of the spacers came out and struck the employee in the head.
Pipe crew had been tasked with removing a pressure sensor from the discharge spool of #18 ore line. Employee had been tasked with using box wrench to prevent movement by a nut located on the bottom of flange. Wrench slipped off of the nut it was wedged again and struck the adjacent pipe. At time of impact right ring finger was positioned between the wrench and pipe.
Mine Refuse Attendant climbed the steps of a Volvo A40G haul truck to observe the material in the bed for a dump location. While descending the steps, the attendant lost footing on a step approximately 38 inches tall and fell to the ground. The ground consisted of compacted reject material.
Employee (E1)had been tasked with operating a 50-ton haul truck. E1 parked the truck and while dismounting placed right foot on the ground. As E1 stepped downward and positioned left foot on ground, the right ankle began to roll outward resulting in a fall to the ground. At local medical facility x-ray revealed a right ankle fracture.
Employee found unresponsive. Apparent heart attack.
Crane was being used to remove pump impeller, placing it in bed of truck. Rigging failed resulting in impeller falling into bed of truck. The impeller rolled off bed of truck and struck employee in right leg and foot. Employee transported to local medical facility.

EE had exited cab of Komatsu excavator. EE turned & was holding the handrails to dismount. EE dropped EE's right foot to step on the step. Right foot slipped & went to the ground while EE's left foot remained on the track due to EE's boot catching on the track. During the slip EE held onto the handrail with left hand, overextending left leg & sustained a strain to thigh muscle.
Conditions were wet due to rain. Area was wet,slippery & rocky. While moving a Stacker, EE fell forward tracking the stacker with the remote. As EE fell EE dropped the remote in the mud realizing the remote toggle switches did not return to the neutral. EE moved to find remote, doing this a track of the stacker ran over EE's arch & toes on right foot causing fractures.

While making repairs to the coal exhaust fan, employee was rolling a belt manually. Employee's right ring finger and pinky were caught between the belt and pulley resulting in fractures and sutures.
Employee was using a jigsaw to cut siding on #11 conveyor in area. While cutting Employee was taken to first aid where the eye was flushed again. Employee was then taken to local eye care center where debris was removed and antibiotic eye drops prescribed. Employee was wearing required PPE at the time of incident.
The EE was operating the Hemtt fuel truck. It was raining and EE was running behind schedule because of the rain. EE was driving down a steep ramp to fuel a water jack and states that the brakes did not work. Before EE got to the bottom of the ramp EE decided to jump from a moving vehicle. Had EE stayed in the vehicle at the slow rate of speed EE may not of been injured.
Employee was removing & replacing the bucket teeth on an excavator. Employee cut them with the torch and was using a hammer to break them free when a piece of metal struck the employee in the leg resulting an ER visit to remove the foreign object. Released to work the following day.

While dumping #16-1 Screw Conveyor, an Operator was approximately 10' from the conveyor while a front end loader operator was removing hot rock. As the front end loader approached, a flash occurred which caused hot product to get on both of employee's legs. Employee was taken to first aid where burns were treated, then employee was taken to a local medical facility.
Employee was tightening a band around a 36"" pipe on the recycle lake line at the west side of Phos Acid. Employee's hand slipped and contacted the edge of the band. The contact resulted in a cut on right hand that required four sutures to close. Employee was wearing gloves at the time of the incident.
Employee stepped off of a step on the side of a flatbed truck and twisted right ankle. Sprain with a hairline fracture
Employee was changing drill bit on a press drill. Employee failed to engage lock fully and as EE removed bit the drift pin fell. Employees left thumb was struck. Thumb bone was fractured.
Employee was replacing dump rope on the 2550 dragline. When repositioning the dump socket to enable safe removal of the wedge, the socket shifted, pinching employee's right index and middle fingers between the dump socket and drag chain. Employee was wearing gloves at the time of the incident.

Miner was operating a skidder when the blade on the front of the skidder abruptly fell without warning causing the skidder to stop suddenly. The miner's face, body and left leg contacted the steering wheel and steering column causing his mouth to clench down forcefully causing several chipped teeth, a laceration to the chin requiring 4 sutures and severe bruising to his left leg.
An employee was performing hands free hydro blasting on the #3 cold cyclone, the water contacted hot product when the blockage broke through and caused steam to blow back up the pipe. The employee operating the unit stepped off the peddle, and then tried to quickly back away. He stumbled over the foot pedal and fell.
Miner was using a 2.5 gallon urea (DEF fluid) container with a flexible funnel to fill a DEF fluid tank. While lowering the jug after filling, miner stated a few drops of DEF fluid entered his eye causing a minor chemical burn.
A mine electrician sustained burns to his arm when a live 7,200 volt power cable arc flashed at a junction box. Employee was taken to first aid for initial treatment and then to a local medical facility for evaluation and treatment. Employee was then transported to regional burn unit for treatment of his burns, and for further testing.
Employee was walking at Mill flotation. He stepped in an unprotected trench. Grating that covered the trench had been removed, water was covering the area due to a pump failure at the time. Received an abrasion to his right shin and reported that his hip was sore. He returned to work at that time. Became a recordable on 6/23/15 due to the employee having physical therapy.
At 05:00, Mill operations started the #6 Calciner preheat process. At approximately 05:46 an explosion occurred in the unit causing extensive damage to the vessel and the building. No injuries or environmental impact occurred during this event.
Employee was in the transfer station assisting with trying to get the #39 conveyor back running (frozen). He was walking back towards the head pulley area. As he passed under the 17-3 flop gate platform, he was concentrating on his walking/working surfaces and bumped his head on the 17-3 flop gate platform resulting in a laceration to his forehead.

Employee was cleaning up around 1-4 mesh reject pump. He stepped in soft reject material and rolled his right ankle.
Employee was driving a 3/4 ton pickup down ramp. Contract company was moving 16"" pipe across bottom of ramp. Employee's truck hit the pipe causing pain in his right arm/shoulder. Was seen at orthopedics and prescribed physical therapy.
While performing dragline repair work, employee's fingers were pinched between a pin component and flange while the pin component was being moved into position. Resulted in lacerations on two fingers requiring sutures.
Damage to a phosphogypsum stack berm resulted in the release of an undetermined amount of gypsum transport water. There were no injuries. The breach was closed at 6:25 PM.
Employee was operating a CAT 740B haul truck in block 8 cut and drove thru a dip in the road while staging up for loading.
Employee was adjusting the air flow on the green float bank B-Cell when the middle finger on his right hand got caught between a moving paddle wheel and the guide frame for the tee handle used to adjust the air.
Operator exited the cab of the crane and walked across the deck toward the ladder to climb down. Just as he approached the ladder, he slipped and fell to the ground landing on his feet and then buttocks. Employee was taken to hospital with complaints of pain in his right ankle/foot. Xrays revealed a fracture a small heel bone that required a cast to heal.
Operations was in the process of relocating a de-energized power cable. Employees were positioned as spotters as a backhoe was used to pull the cable. One employee was on the opposed side of the cable from the others. As cable was pulled it became hung up on a berm. As employees signaled for the backhoe to stop the cable broke through the berm and struck employee at the knee.
The employee allegedly walked off of a stair well and fell approx 3 ft to the ground fracturing his elbow. The injury was reported on day after the alleged incident and there were no witnesses to the fall. The validity of the claim is questionable.
Employee strained his right shoulder while relocating a magnetic drill.
Employee has worked as a welder for 36 years. It was determined to be a work-related STS on 1/13/14.
Employee was positioned about 8-10 feet from two other workers that began to remove the shroud from the dragline bucket using a hammer. Upon impact a shard of metal from the shourd contacted employee cutting through his jeans and into his left knee. He felt a burning sensation and noticed his knee was cut. Employee received five stitches to his left knee.

While exiting a connex employee used a cross support bar as a step. His foot slid backwards causing him to fall. Pain in left leg. Employee taken to First Aid where ice pack was applied, OTC Aleve was given for pain. He was released back to work. On 1/2/14 he was still having pain. X-ray done on 1/3 showed an avulsion fracture at the base of his lower left leg.
Stripping cable with a knife and cut the thumb on his left hand. 5 sutures required to close the laceration.
Employee was tasked to check the cyclones and read the tank level at the ore storage tank. When he got to the tank, he called to the control room to shut down the 1 & 2 storage pumps because the overflow box was leaking onto steps. He went to the top of the tank of perform duties. When he came back down, he slipped on the wet steps. He strained his left shoulder and right wrist.
Wind blew debris into employee's eye. The employee was wearing proper PPE at the time of the incident.
Prestrip operators were assisting shift maintenance in the installation of a roller on the 239 conveyor belt. Employees were working on each side of the belt. As employees on one side of the roller positioned it into place the other end of the roller moved and pinched employee's left ring finger between the roller and the mounting bracket.
Mashed little finger between impact wrench and steel plate on clean out tee.
Employee was stepping off transportation bus attending a company wide safety meeting. He was stepping off the bottom step to the ground when he twisted his right ankle. Ankle was evaluated by a physician and he was diagnosed with a sprained ankle.
Employee was placing body bolts with a port a power when the shim plate used to distribute the load pressure fell on his hand causing a fracture to his ring finger.
Removing old rollers from wet rock feeder area. Picked up last roller to toss through handrail. Right hand, ring finger got caught on sharp end of roller while throwing roller through the handrail.
Employee was trying to knock a bearing in with a battering ram at top of boom. On the third attempt to strike bushing, the ram entered the pin hole and continued swinging forward toward the frame. Employee's left ring and small finger were pinched between the ram pad eye and the excavator.
Contract miner was using a two inch wash down hose. The miner used a worm gear hose clamp to connect a fire hose type nozzle for wash down. After inserting the spray nozzle and tightening the hose clamp, miner pressurized the hose. After pressurization, the spray nozzle came out of the hose and the hose clamp retracted into his hand causing a minor cut and two stitches.
Employee was struck by a torch causing a 2nd degree burn to his left wrist.
Miner reported injury to right thumb while opening a soda during lunch. First Aid treated for a minor cut by EMT and returned to work. The Miner returned to work on 03/18/13 (next scheduled shift) with complaints of pain, wound obviously inflamed and red. Miner taken to see M.D. where he was dignosed with an infection and provided prescriptions. Return to work 03/21/13.
Contract miner was shoveling dirt into a bucket taking 20 minute shifts. Contract miner completed his 20 minutes of shoveling and was standing, talking with other miners when he complained of a back spasm. Contract miner was taken to first aid for evaluation, given heat pack and returned to duty.
Mill maintenance mechanics were changing rollers on #3 conveyor belt. While both mechanics were replacing roller, one mechanic lost control of the roller. The weight of the roller shifted, pinching other mechanic's left index finger between the roller and conveyor support frame.

Miscommunication between rigger employee and forklift operator led to a steel beam toppling over onto another beam, trapping the rigger employee's leg between the beams
Crawling through compartments inside tub & EE felt discomfort to left elbow. On 12/21/12 we were informed EE's work status had changed from full duty to light duty.
Employee allowed the door to the office to close on her finger, fracturing the most distal phalange.
While dismantling some scaffolding on the contractor scaffold yard an employee suffered a broken tooth when a scaffold member was mishandled by a coworker. The scaffold member was mishandled while being passed to the injured employee. Injured employee suffered a broken tooth and busted lip. The injured employee received prescription medication and the tooth was filled.
Operator was removing a johnson bar he had used at the conveyor flop-gate platform inside of 19/39 transfer station when it slipped from his hand and fell on his right fot and struck him behind the end of his steel toe.
While cleaning under the prestrip feeder, employee was dumping a wheelbarrow of material over the edge of a concreted area. The front of the wheelbarrow become stuck in a pile of mud, so employee grabbed the legs of the wheelbarrow for better grip. While pulling the wheelbarrow loose from the mud, he hyper extended his left thumb.

After exiting pickup truck, miner stumbled slightly while closing vehicle door causing the door to close on his left hand. The resulting injury was of a nature that required sutures.
While working to move 20 inch gyp pile pipes (filled with acid water) employee was standing between 2 pipes. Back pipe rolled into back of legs pushing his legs into pipe in front of him and with feet wedged under front pipe.
Employee stepped down 18 inches or so from one level to another on a catwalk. While doing so he extended his arm slightly behind him and up to just above shoulder height. At full extension he felt sudden severe pain.
Mechanic was loading a wooden crane pad (65 lbs)onto the front deck of a 35 ton crane. Employee slipped in the mud and fell forward catching his little finger between the crane pad and a metal loop designed to hold the pad the pad on the crane.
While Cutting a Tagline with a Knife employee's hand slipped and knife penetrated Right Thigh. Laceration resulted in six stitches.
Employee walking on concrete walkway. Caught toe of boot and began to fall forward. Reached out with left hand (was carrying 2.5 gal. jug of oil in right hand)to catch his fall - jammed left hand into solid metal handrail. Hand flexed back at wrist.
Contractor Employee (C1) was moving refractory bricks from a manway inside a calciner when a second Contractor Employee (C2) accidentally dropped a tweeter pipe on C1's right hand. C1 was taken to Beaufort County Hospital and x-rays showed C1 had a fractured index finger.
Employee exited a two ton flat bed truck. Employee has the edge of the door between his ring and little finger and began to swing the door shut, when his elbow contacted the flatbed that extends out beyond the cab. This jammed the edge of the door in between the ring and little finger.

Working 16 hours for Mill outage - welding. At the end of shift he felt weak, dizzy & nausea. On the way home he felt worse and went to the hospital.
Employee stated he was welding in confined space with a sweaty t-shirt on. While holding wand in left hand and another tool in right hand, he leaned over a metal pipe and when his shirt touched the pipe he felt an electric jolt through his arms and chest. Ambient temperature was 98 degrees and heat stress could have been a factor in the employee's condition.
Employee fell while exiting a caterpillar excavator. Employee states that he was using three points of contact, and was egresing the ladder correctly. Investigation of the ramp did not reveal any adverse condition of the equipment or the ppe worn by the employee.
The train sat idle waiting to come on site. Carbon built up in the engine. As the train began moving forward it threw sparks into the brush.
Employee was moving oversize material using a CAT 966H Loader. He stopped to blow his nose and dropped the paper towel he was using and tried to catch it. As he was reaching between the seat and the steering column, he jammed his middle finger on the steering column, dislocating the middle finger on his right hand.

Bucket Wheel Mechanic (BWE1),with thirty five years experience, was removing bolts from a brake pack/adapter plate secured in a vise on the back of a work truck. When the vise pressure was released to rotate the position of the assembly the unit rolled out of the vise pinning left index finger between the adapter plate & the truck bed. BWE1 was transported to First Aid.
Employee was helping remove bolts from a pump. With two bolts remaining, employee decided to go under the suction line to the east side of the pipe due to restricted working room and possible line of fire issued should the pipe move. When he crouched down to step under the pipe, his foot slipped on the metal.
Technician stepped onto fold down step at rear of service truck to enter rear service truck area. Weld on fold down step failed.
After carrying buckets weighing approximately 40-50 lbs., employee felt strange feeling in right groin area.
Employee was picking up a piece of plastic pipe approximately 25 - 30 lbs. Picked up with left arm and felt pain immediately. Released pipe.
Employee was removing a safety can of diesel fuel from the deck of a Genie Telescoping Boom forklift while the operator of the forklift was lowering the boom. The employee's left index finger was pinched between the boom and the handle of the can.
Reaction to respirator mask seal on face due to warmer weather. Told employee to keep mask clean and use protective cream on face.
Employee was using a hammer wrench when a bolt broke free and hit the employee's pinky finger against an adjacent nut resulting in a laceration requiring 4 stitches and a fracture to the tip of the finger.
Employee was using a knocker wrench to tighten the bolts on #1 sandtailings. When he hit the knocker wrench with a sledge hammer, the wrench came off and hit him in the mouth.

The backhoe operator was pulling power cables with a backhoe. The employee was standing between junction box and backhoe. The cable became tight and hit him.
Employee was using a belt knife to cut oil cloth. Knife slipped and lacerated left indext finger. On-site nurse practitioner placed 6 sutures.
As employee was picking up a 2 horse power motor (approximately 45 lbs), he started to turn and his right ankle rolled under him.
Employee was walking down stairs from hot wind box level to free board level doing morning checks. He felt a pop in his left knee.
While employee was looking at the walkway to ensure it was clear, he walked into a low hanging overhead pipe. On 11/13 he reported the incident to his supervisor and complained of neck stiffness. He was evaluated at Safety & classified as first aid. Follow-up scheduled 11/14. On 11/14 he was re-evaluated & sent to the doctor. Pain medication was prescribed.
Employee was removing conveyor belt rollers on the prestripping system conveyor with the assistance of a backhoe. Employee placed his hand on the roller support bracket to remove it from the handler peg once the weight of the roller was supported. He signalled the backhoe operator to lift the roller. As the roller was lifted, the hanger bracket rotated and pinched his thumb.
Employee was removing steel grating in the Flotation Bldg. He rigged the hoist hook to the connecting rod of the grating and bumped the hoist up. Not knowing that the grating was not free, he bumped the hoist again, causing the connecting rod to break. The hoist hook released from the grating and struck him in the left hand, causing a cut and a hairline fracture.
Employee was working on #1 conveyor replacing return rollers. Employee had both hands on the roller with his right hand on the end and left hand supporting the weight of the roller. As he pulled the roller out from conveyor belt, roller slipped; his 4th finger of left hand was pinched causing a superficial abrasion.
Was sprayed in face with phosphate when valve was opened. Blew safety glasses and hard hat off.

Mechanic was assisting Foreman in putting guard back on Wet Rock Feeder Belt after tightening belt. Mechanic's finger got caught between bottom of guard and tail pulley shaft on Wet Rock Feeder.
Employee was changing out conduit. Was coming down turntable to platform and stepped on jack plate. Right foot rolled. Jack plate was sticking up 2 to 3 inches and was covered with dirt.
Fell 2 to 3 feet resulting in a contusion to his left buttock.
Miner not task trained on removing electrical leads.
Employee working when a 40 lb roller fell 1-2 feet striking him on the side of his hard hat, knocking him off balance. Stated he felt o.k. and went back to work. Later felt stiff and went to FA. At FA he was placed in a cervical spine collar and placed on backboard & transported to CRMC. X-rays & CT were neg. Given presciption meds & placed on restricted duty.
Employee states he was trying to drive out cut off bolts using ball ping hammer. Does not know what caused injury but looked down and noticed blood on left lower arm. Applied pressure and was brought to first aid. Sent to New Bern Family Practice. X-ray revealed foreign body in left forearm. Employee was given prescription medication and 2 days restricted duty.
Presented to first aid on 4/9/07 stating he wanted to claim his wrist pain as work related. States the pain and numbness is bothering him more and he wants something done about it. Wants to be seen and treated for this. Employee had carpal tunnel surgery bilateral on 7/30/07.
D9 dozer operator walked up to co-worker to ask where to pull tail pulley for repair. Co-worker was bent down hooking up a 3/4"" U shackle to the tail pulley. Co-worker raised his arm to point with the shackle in his hand and hit the operator in the mouth, resulting in a chipped and cracked tooth.
Operator attempted to close manual block valve. It wouldn't close completely. When trying to open and reclose, the handle broke. The Operator lost his balance, fell backwards and hit the back of his head on the floor grating causing a laceration that required stitches. Valve damage about $100.00.
Employee reports wrist/hand discomfort. Medical tests performed on 2/27/07 shows bilateral carpal tunnel. Received doctor's note regarding tests on 3/22/07.
Employee boarding bus to be transported to work site. He lost footing and slipped falling forward. He attempted to break his fall with left hand and fractured left fifth metacarpal.

Employee was assisting with a vulcanized conveyor belt splice using a Hyde hook blade knife to remove excess rubber from the belts internal cables. While pulling the knife toward his body with his right hand the blade tip slipped off the cable and came in contact with his left forearm about 4"" above the wrist causing a 3.25"" laceration which required 6 sutures to close.
Employee was changing out a lower cone on a secondard cyclone. Employee was tightening up bolt in flange when the rachet slipped off of bolt and stuck operator in the mouth (lip) causing a 1/4 inch laceration.
Using knife cutting a strap off of a water hose; as strap broke lose, cut self with knife.
States he was on gypsum pile and changing a fuel filter on 974 backhoe when cover fell on top of his head. Cover weighs approximately 25 lbs. Complains of neck pain with lateral movement.
Grinding bolt off of a flange. The grinder snagged and it was torqued out of his hand. Grinder fell and cut his side.
Assisting in changing out a section a tailing piping. He was using a 1"" impact wrench to remove bolts from the flange. The impact wrench slipped off of the bolt head. This caused the employee to lose his balance and he used his left hand to catch himself. This resulted in a laceration of his left palm.
Employee bend down to look at a pipe on the ground and poked self in right eye with a tall reed.
Welding on 2570 boom, noticed pain in right elbow.
Employee was welding and lifted his shield up. Hot slag ""popped"" and went over safety glasses and into left eye.
Employee was trying to close a drawer (approx. 70 lbs). The drawer was stiff and he lifted up on drawer. As he was closing it the bottom drawer was slightly open and it acted as a scissor and caught right hand 2nd and 3rd fingers in drawer.
Employee was replacing an internediate cable on the 8200 Dragline. The task involved swinging a 20lb hammer, pulling on 3-ton com-a-longs, and walking up and down the mast of the dragline. While performing this task, his back started feeling stiff. Woke up the next morning with back pain.

Employee was working on a CAT 988 front-end loader putting on an engine guard. Leaning over equipment and felt ""pop"" and felt pain in left side, lower rib cage.
Putting a bearing in head puley when sling slipped and mashed left hand.
Onset 3 weeks ago with left hand pain, no trauma, no increase in use. Works as a welder and uses grinders/welds daily.
Onset one month with pain in left elbow lower arm. Worstened over past two days with using left hand to carbon arc, increased squeezing.
EE WAS WALKING FROM GUARDHOUSE TO PORT-A-JOHN WHEN EE HEARD A CAR. EE TURNED TO SEE IF THEY NEEDED TO BE LET OUT OF THE FACILITY. WHEN EE TURNED BACK AROUND, EE MISSTEPPED ON FOOT, SLIPPED OFF THE PAVEMENT, TWISTING LEFT ANKLE AND KNEE.
A Calciner Operator was winding up an extension cord while walking. His right leg contacted a pressure tap, causing a puncture wound. This was initially treated as a first aid case. Subsequent treatment (2/9/2005) changed the status to a recordable due to removal of dried blood from the bruised area.

Calcined rock was dumped from #5 calciner hot cyclone at 1:00 pm and cooled with water. After removing half the pile, steam was created during a typical lift when a hot pocket of material contacted water in bucket. Steam and hot material burned employee.
While greasing H-link on backhoe, two grease fittings would not accept grease. Mechanic removed glove to adjust grease fitting. Without putting glove back on, mechanic began greasing fittings. The first fitting accepted grease. While attempting to grease the second fitting, the grease line leaked, injecting a small amount of grease into his right index finger.
EE WAS PUTTING FLANGES ON 18"" DIA. X 50' LONG USED PIPE. ROLLING PIPE TO GAIN ACCESS TO OPPOSITE SIDE OF FLANGE, USING A 40""-LONG BAR WITH PEGS. PIPE WAS CURVED FROM PREVIOUS USE AND SHIFTED DURING ROTATION. PIPE JUMPED WOODEN WEDGES (USED TO STOP PIPE), BROKE PEG OFF OF PIPE ROLLING BAR AND FORCED EE'S RIGHT HAND BETWEEN BAR & FLANGE ON NEXT PIECE OF PIPE. STITCHES REQ.
An operator initiated start-up procedures on the #5 calciner. He started the exhaust and tempering blowers. An ignition occurred. While the unit was down, bunker C had vaporized and collected in the upper calciner compartment. When tempering air was added, it provided additional oxygen to an already hot compartment resulting in the ignition. Estimated cost to repair, $30,000.
GRATING WAS UNDER WATER. WHEN EMPLOYEE STEPPED ON GRATING IT ROCKED. HE FELT A ""POP"" IN HIS RIGHT KNEE (INSIDE AREA).
EMPLOYEE USED DOZER TO PUSH A 60' LENGTH OF RAILROAD RAIL OUT OF THE WORK AREA. HE POSITIONED THE RAIL PERPENDICULAR TO THE DOZER BLADE. THE RAIL STOPPED MOVING FORWARD AND THE DOZER CONTINUED TO TRACK. THE RAIL BOWED AND SPRANG UP OVER THE DOZER BLADE AND CAME DOWN THROUGH THE LEFT SIDE CAB DOOR GLASS AND ONTO OPERATOR'S LEFT KNEE.
EE USED DOZER TO PUSH 60' LENGTH OF RAILROAD RAIL OUT OF THE WORK AREA. HE POSITIONED THE RAIL PERPENDICULAR TO THE DOZER BLADE. RAIL STOPPED MOVING FORWARD & DOZER CONTINUED TO TRACK. RAIL BOWED & SPRUNG UP OVER DOZER BLADE & CAME DOWN THROUGH LEFT SIDE OF CAB DOOR GLASS ONTO OPERATORS LEFT KNEE.
While adjusting seat in 8200 dragline, noticed cable on seat adjustment was broken. While tightening a screw on the cable the nut slipped in wrench and screwdriver went into top of right thumb.
Employee was putting a cable back in a sheave on right foot of the dragline. Right 3rd & 4th fingers were mashed under the sheave.
Replacing scalping deck. The deck was out of position and he was trying to reposition it. The load shifted causing middle finger, right hand to get caught between deck and angle iron. Employee had gloves on.
Employee was taking a jib pin wrench out of rack. The jib pin wrench rotated and jammed right hand against crane body mashing right ring finger. (had gloves on)
Putting tail pulley guard back in place. Piece of angle iron stuck out from the conveyor bed which supported guard. Hand caught between guard and support.
Employee was watching an operator energize switchgear. The operator called electrician to let him know power was on. Employee stepped back and turned to the right to look at AB drive. Employee felt severe pain in right knee - caught self on a roll around ladder.
EMPLOYEE WAS USING A HAWKBILL KNIFE WORKING ON GROUNDING STICK. CUT HIMSELF AT THE BASE OF HIS LEFT THUMB.

EMPLOYEE WAS HOLDING AN OBJECT WHILE THE WELDER WAS TACKING. EMPLOYEE HAD CUTTING SHIELD ON. HE WOKE UP AT 3:50 AM ON 12/16 COMPLAINING OF RED, TEARING EYES. HE FELT LIKE HE HAD ROCKS IN BOTH EYES.
HOLDING ONTO PRYBAR, ROLLING OVER A COUPLING AND COME-A-LONG THAT WAS CONNECTED TO A STUMP. A PIECE OF THE STUMP BROKE OFF CAUSING THE COUPLING TO ROLL BACK. LEFT SHOULDER AND PRYBAR CAUGHT IN FORWARD MOTION.
WORKING ON WET ROCK FEEDER ON A WINDY DAY. DEBRIS BLEW BACK IN FACE AND HE FELT F/B IN LEFT EYE.
WHILE SPLICING A BELT. EE WAS KNEELING DOWN ON BOTH KNEES, CUTTING THE BELT. HE WAS HOLDING THE KNIFE WITH BOTH HANDS AS HE CUT, WHEN BLADE SLIPPED OFF OF BELT AND CUT INTO HIS RIGHT CALF. 5 INSIDE AND 7 OUTSIDE STITCHES WERE REQUIRED TOCLOSE THE WOUND.

EE WAS CHECKING PUMP STARTER TO INSURE PROPER VOLTAGE. CONTRACTOR HAD TIED INTO WRONG SIDE OF TRANSFORMER. VOLTAGE WAS APPROX. 2400 VOLTS, CAUSING ELECTRICAL BURNS.
CUTTING A PIECE OF ROPE OUT OF TOP ROLLER ON D10 DOZER. CUTTING WITH LEFT HAND IN AWKWARD POSITION. CUT HIS RIGHT THUMB.
WHILE STANDING ON A STACK OF BRICKS, EE WAS HOLDING UP WOOD FOR OTHERS TO DRILL. WHEN HE TURNED TO GO TO THE NEXT PIECE OF WOOD, HE LOST HIS BALANCE & FELL ONTO ANOTHER STACK OF BRICKS.
