A new hire was helping load scraped iron, was told to stay back from swing area, he got in swing area and some iron fell from bucket and landed on top part of left foot, foot has 4 broken bones, he was not wearing steel toes boots as safety training required

This is to correct date of accident, regular customer got out of truck, started to climb on loader, slipped on step, fell and hit head,had site specific signed before entering pit area.
