INJURED WAS SITTING IN T HE CAB OF A GALIS ROOF BOLTER UNDER THE CANOPY. AS HE WAS DRILLING TOP A PIECE OF ROCK DISLODGED BOUNCED OFF OF THE HEAD OF THE BOLTER & STRUCK INJURED ON T HE LEFT TE MPLE.

INJURED WAS SITTING IN THE CANOPY OF THE MINER TRAMMING THE MINER BACKWARDS. THE CANOPY CAUGHT A ROOF BOLT, FORCING THE POST OF THE CANOPY AGAINST THE CONTROL LEVERS CATCHING INJUREDS HAND BETWEEN THE CONTROL LEVERS & CANOPY POST.
