EMPLOYEE, WITH THE HELP OF ANOTHER EMPLOYEE,W AS IN THE PROCESS OF LOADING OXYGEN TANKS ONTO A PICKUP TRUCK WHEN HE REPORTED TO HIS FOREMAN THAT HE PULLED SOMETHING IN HIS BACK. EMPLOYEE WAS WEARING A BACK SUPPORT AT THE TIME OF THE ALLEGEGED ACCIDENT.

EMPLOYEE WAS INSTALLING A METAL DOOR THAT HAD TO BE CUT TO FIT.DURING THE CUTTING PROCESS,METAL AND SAWDUST SHAVINGS ALLEGEDLY CAME IN CONTACT WITH EMPLOYEE'S EYES. EMPLOYEE WAS WEARING A FULL FACE SHIELD.EMPLOYEE WENT TO THE DOCTOR ON 10-17-92 AND WAS GIVEN EYE DROPS AND RELEASED FOR WORK.
