Over-exertion in pulling or pushing objects

Employee pulled hard to open a track switch, switch opened easy causing him to pull a muscle in his left rib.

Caught in, under or between (Not Elsewhere Classified)

In the process of building a brattice; the ee was placing a block when another ee tossed him another block-catching the injured ee's finger between the blocks, resulting in a fracture to the tip of the finger. There was no medical attention administered, only diagnostic and first aid.

Caught in, under or between a moving and a stationary object

The ee was injured while spot bolting one-break outby the #6 heading. The bolt rotated when the ee attempted to place the bolt into the drilled hole. His little finger was caught between the bolt and drill chuck resulting in a laceration and fracture to the finger.

The ee was injured as a result of a man bus derailment. In an attempt to re-rail the man bus a ratchet type jack was placed under one corner of the man bus and two crib blocks were placed under the center to rest the frame. The jack handle suddenly released upward striking the ee on the left side of the face resulting in a temporal fracture.

The ee was installing a roof bolt and lifted his left hand to hold the plate in position. His left middle finger was caught between the plate and mine roof. The ee received a laceration to the finger requiring several sutures.

The ee was struck in the face with a 4"" water line. The empty discharge line was struck by the shuttle car and in the turn hit the ee in the face. The ee was dazed but stated that he was ok and completed his shift. Later the ee went to the hospital and it was determined he had three fractures to his jaw.

The ee was in the process of tramming miner and the machine became fouled. The ee de-energized the miner and was traveling on the right side of the machine when a piece of rock measuring 12"" X 11.5"" X 5' rolled out and pinned his right leg between the rock and mine floor resulting in two breaks to the tibia.

Inhalation of radiations, caustics, toxic and noxious substances

The employee has been diagnosed with Coal Worker's Pneumoconiosis(CWP) Dominion Coal was notified of this illness 12/10/2012. Employee is current off work for other medical. Dominion was notified by a medical doctor.

Accident type, without injuries

A roof fall was discovered on U-section at Survey station 9755. The fall area was approximately 6' in height, 20' in width and 20 plus feet in length. The fall area was mined in 10/1997 and was supported with point anchor bolt system. The fall area was dangered off to prevent access.

Over-exertion (Not Elsewhere Classified)

The ee had traveled by scoop to T-Section intake to retrieve conveyor belt to supply a belt move on another section of the mine. The ee inserted a chain through the layers of belt that were on the mine floor & jumped across the belt to the opposite side. He felt a ""pop"" in his right knee resulting in pain & swelling to the knee.

Fall onto or against objects

A belt person uncoupled a 2"" waterline on the off-side of the #7 main line belt. He placed the 20' joint of water line across the belt to the track side to reconnect the line. The injured ee slid across the belt & as he stood up his feet slipped on the muddy mine floor resulting in the ee falling & striking his right hip on the track side belt rail resulting in a bruised muscle.

The supply tram had derailed and the ee was in the process of rerailing the tram using a hydraulic jack to lift the arm. A pin in the jack bar bracket inadvertently came out of the jack resulting in injury to the ee's ring finger on his right hand/being caught between the jack bar and the equipment. The ee received 3 sutures to close the laceration.

An electrician was working on an airlock door located near the portal of the mine. The electrician was dismantling the guard for the roll-up garage type door used as the outby airlock door. The guard suddenly moved upward pinching 2 fingers on his right hand resulting in lacerations to his middle and index fingers requiring 3 sutures to close the lacerations.

A third shift utility crew was dismantling a conveyor belt take-up unit. The ee attempted to block up the unit to allow a scoop bucket to be positioned under the unit. The take-up inadvertently slid off the scoop bucket resulting in the ee's right hand being caught-between a metal block and a piece of angle iron located on the mine floor.

The 3rd shift utility crew had locked-out the R-2 belt & were removing slack from the belt. The day shift section crew stopped at the R-2 drive & found the belt was locked-out. The day shift electrician thought the lock on the cathead was a lock he had placed, and had lost the key to the lock. The lock was knocked off & the belt energized resulting in the incident.

Struck against stationary object

The ee was positioned in the operator's compartment with the driver of manbus. The bus traveled near break #36 the mine height changed from 72"" to 54"" quickly resulting in the belt man's head contacting the mine roof due to not being positioned low enough to provide vertical clearance. The belt man received an abrasion to the forehead.

A roof fall occurred in the intake air course of the #2 entry of the mains section at survey station #1156. The roof fall did not affect the ventilation or the passage of miners. The fall measured 30 x 65 x 5' 6"" & the area was bolted with fully grouted rods. The fall is a result of the stress applied to weak planes in the thin bedded laminated shale strata over a period of time.

Struck by falling object

The roof bolter operator had completed drilling a 5 ft. test hole that required using 3 sections of drill steel. As the operator removed the lower section of drill steel the 2nd section fell on the drill head, striking the operator on the lower lip resulting in a laceration which required 5 sutures to close the wound.

Struck against stationary object

The ee was attempting to remove a rock from the chute of the #1 belt dump over point that had gobbed off. As the ee moved the rock - a piece of metal band that could not be seen by the ee cut through his gloved hand resulting in a laceration to his ring finger on his right hand. The ee received 5 sutures to close the laceration.

Due to an accumulation of water in the #4 heading the bolter crew was experiencing vacuum problems on roof bolter. The employee changed the dust bags and was cleaning the mud and coal particles from the dump valve canister. His right hand was caught in the dump valve due to his co-operator setting the stab jack on the machine which also activates the dump valve canister.

The employee was cutting a plastic band from a bundle of roof bolts using a Schefield lock-blade knife. He had placed the knife under the plastic band and was pulling toward himself when the knife struck him in the lower lip resulting in a laceration to his lip requiring six stitches to close the wound.

The employee was helping the continuous miner operator move the machine. After pulling the miner cable pull rope and hooking the rope to the miner, he began having lower back pains. The employee finished his shift, but went to the doctor the following day due to the pain becoming worse. The ee was placed on restricted work duty.

The employee (contractor) was working with a belt mechanic installing skirts in the tailpiece at the T-1 belt drive on the #5 belt. The employee was crossing the conveyor belt at a crossover for additional supplies. As he exited the crossover his right foot slipped resulting in a twisted knee.

The CM was loading in #6 entry in right pillar cut when a methane ignition occurred near head of machine. Contributing factors to accident are unknown due to accident not being reported by hourly ee to a member of mine mgt until 4/28/11. This accident occurred on a retreat mining section making an investigation of the accident site inaccessible at the time of notification

The section crew was in process of pulling back the section power center and the employee was helping configure the high voltage cable as the cable was being pulled by a scoop. The employee's left leg was caught under the cable, resulting in the employee's left ankle being injured.

A roof fall was discovered in the drift mouth of the No. 2 Entry (survey station No. 2) on the old CJ & L mine portal. This mine is now a part of this active mine. The fall measured app. 15' wide X 15' long X 4' + in thickness and was composed of laminated sandy shale.

The cont. miner on the 004-0 (""U"") section accidentally cut into an active gas well in the face of the No. 4 entry. The gas well had not been plotted on the mine map at the mine. No gas escaped into the active workings. The power on the section was cut off and the entire mine was evacuated and the underground power was cut off.

During the preshift of the 001-0 MMU, T-Section, at approximately 5:50 a.m., the preshift examiner encountered an explosive mixture of methane in the range of 5% in the #1 entry and 3.8% in the #2 entry of the pillar line.

The employee was a passenger on a little mac personnel carrier when his belt-wearable SCSR contacted a fallen rib and pushed his SCSR into his right side resulting in bruising of his side.

The employee was installing a 5' torque-tension roof bolt in a 40"" mining height which required the bolt to be bent. The operator had started the bolt into the mine roof and was attempting to straighten the bolt when he felt pain in his back.

The crew was on the manbus and stopped to raise an air-lock door. The employees riding on the inby end of the manbus changed from laid down position to riding on their knees. As the bus traveled through the door one employee caught his hard hat on the door frame, pushing his hard hat onto his safety glasses, resulting in a laceration to his nose and under his right eye.

Employee had drilled a 10' hole to install a cable bolt. As he was bringing the drill steel out of the hole; the pusher steel and the first extension stuck together. As he was pushing & pulling the steel to pull it apart; the steel came apart and one piece hit him in the mouth chipping two teeth.

Employee was walking from the No. 4 entry to the No. 3 Entry on the 001-0 section on a slight downhill grade. His right foot slipped and he fell on his left foot, fracturing his left lower leg and ankle.

The CONTRACTOR ee was riding a track vehicle to examine beltlines. The vehicle derailed in a switch at the No. 4 Drive. The injured was using a hydraulic jack to raise the vehicle & pull it over onto the track. As he was raising the vehicle, the jack swiveled causing the jack to fall and strike the ee's finger causing a laceration & fracture.

After replacing belt in a belt line, the ee was using a chain hoist to pull the belt together to splice. The crew had loosened a top roller and the ee placed his arm at that roller while operating the chain hoist handle. The chain caught the roller and raised it up. The forearm of the ee was caught between the rigid belt structure & the roller.

The employee was loading rock dust onto a rock dusting machine in a scoop bucket and strained or sprained his lower back.

The injured, a contractor employee, & a company employee finished making a belt splice. After loading the belt splice equipment on the personnel carrier, they boarded the ride to go to the section. The vehicle started faster than they anticipated & the vehicle ran under the belt line. The contractor emp. struck the side of his face on a belt roller hanger.

The employee was bounced in the shuttle car while traveling to the feeder. He stopped the shuttle car to reposition himself. He grasped the bottom of the operator's deck and the shuttle car drifted backward catching and rolling the tips of his fingers between the deck and the mine floor.

Employee was bolting in the No. 2 heading & had installed th first row of bolts in the cut. He had installed the first bolt in the second row & was moving from the drill controls to the tram controls on the side of the bolter when a slug of rock 49"" long X 20"" wide X 0"" - 7"" thick fell from the rib line over between the roof bolts installed striking him on the lower back & hips.

EE removed an empty oxygen cylinder from its storage area at S-1 Drive. He laid it down behind him & sat down while waiting on the track signal to turn green. After the signal turned green, he reached behind him to drag the oxygen bottle in front of him when the crib block he was sitting on creeled and his weight landed on his leg that was under him.

Fall onto or against objects

EE was installing a replacement water line over the section tailpiece where the feeder dumps into the beltline. EE stepped off of the tailpiece when a shovel that was laying upright against the tailpiece caught his buttock and caused him to fall backwards. EE landed on the section booster pump and fixtures. EE was placed on restricted work duty on 10/22/09.

A ROOF FALL occurred in the last open crosscut of the No. 6 heading on the 001-0 section, in the interesection & fell right & left into the crosscuts. The fall was composed of laminated sandstone & was approximately 19' wide X 83' long X 10'+ high. The fall is located approximately 140 feet inby survey station No. 8340. The fall caught the continuous miner cable.

A ROOF FALL was discovered on the 001-0 section during preshift examination. The fall measured app. 4' thick X 20' wide X 65' long. The fall occurred in the crosscut from the No. 5 to the No. 4 heading & through the No. 4 intersection. The crosscut had been cut through at the end of the 2nd shift on 9/26/09 & had not been bolted prior to the end of the shift & had been left down.

Emp. took a load of rock dust in with the track mounted rock dusting machine attach. to rock dust belt lines. He went to get more rock dust arriving at Bratt. No. 65 on No. 2 belt line, one of the steel tracks broke-and one of the ends raised up & caught bottom of the rock dust mach. & the emp. was forced into the side of the operator's compartment of the tram.

The mantrip had stopped to re-hang a water line that was across the track. The op. got off the vehicle to help and hit the dump valve for the parking brake. The dump valve didn't fully relieve & in the process began rolling toward the employees. The injured jumped to the side and was caught between some stacked rock dust & the mantrip vehicle & was rolled beside the vehicle.

Employee was assisting with the loading of a 1000 ft. piece of conveyor belt. The roll was on a stand/ the steel band holding the belt had been cut. Employee was pulling the steel band from the belt & the end of the steel band came out from under the belt & sprang back striking the employee on the neck causing a 2 inch laceration which required 7 stitches to close.

Piece of draw rock fell, striking EE on right hand while shoveling belt. Rock size 17"" long, 8"" wide and 1' thick.

Accident type, without injuries

Roof fall occurred on the active pillar line on 001-0 Section while 3-cut pillaring. The cont. miner was completing right hand cut in pillar between Nos. 5 & 6 headings when rock fell fouling the head. Before miner was freed, a large slip fell on cont. miner, across #5 heading & into the intersection. The fall was in a slip & had not been detected prior to pillaring activities.

A roof fall was discovered in an area of known roof conditions. The fall measured approx 60' long x 20' wide x 4'-10' thick. Fall occurred in the #7 entry (intake) at survey station # 5629. The fall had previously been supported with 10' cable bolts & wooden cribs had been set around & under the roof before it fell. Fall is located approx. 12,000' outby face of 001-0 section.

The employee had come out of the mine after working a couple of hours late. As he was traveling down an inclined walkway from the mine shop to the bath house, his foot slipped in some loose gravel.

Contact with electrical current

Tramming feeder to surface, employee got hold of cable splice and came in contact with 480 A.C. power.

The roof fall was discovered in the bleeder entries of the 002-0 Section during a weekly examination of those. The fall measured approx 20' wide X 50' long X 6' thick and fell between crosscuts. The fall is located approx 20' inby S.S. #4068 in the #6 Entry and is approx 3500' from the active section.

The EE contracted Coal Workers Pneumoconiosis while working underground.

Caught in, under or between a moving and a stationary object

The employee was taking a load of rails in on the rail car - the car derailed. Using a hydraulic jack, he jacked the car up from the middle of the inby end. After doing this, the car still had to be jacked up some in order for the wheels to clear the track. EE had his hand on the car as he was jacking the car up, the car creeled onto the hydraulic jack catching his left hand.

Struck by falling object

Bolting machine broke down because the drill head became fouled. As the EE was attempting to free the head, the bolts in the bottom of the drill feed jack broke, allowing the lower part of the jack to become free as the EE was attempting to free the head up and he placed his left leg under the head of bolter. It suddenly became free and fell approx 36"", catching his left leg.

The EE finished bolting the face of the #3 heading on the 002-0 section. He backed the machine up approx 40' out of the face and attempted to turn the machine around in the heading. As he was turning the machine, he twisted his body to the left to look at the location of his cable and in doing so, placed his elbow between light mount and coal rib.

The employee was checking fire outlets along the No. T-5 belt line. He was riding a 3-wheeled personnel carrier. The personnel carrier hit a rock or dip & jerked the steering wheel out of his hand. The area where the accident occurred was sloping toward the belt line & the carrier slid under the belt which was not running. EE struck his shoulder on the belt rail.

The employee was taking a loaded shuttle car to the feeder. As he passed his cable anchor he thought the cable was coming in the deck on him. He threw his arm up & stopped the shuttle car. The cable then took up catching his elbow & pulling his arm upward & backward into the canopy.

A roof fall occurred 10'-15' inby survey station #6023 in the No. 3 entry on the 003-0 section. The fall measured approx 40' long x 20' w x 8' thick. The area had been bolted with 48"" resin bolts & supplemented with 12' cable bolts. The fall occurred in an area of known bad roof & began working on the 3rd shift. The area was dangered off. The fall occurred at approx 6:35 am.

Struck by falling object

The employee stated he was shoveling and rock dusting around the R-1 Belt Drive. He stated he hit the roof at the edge of the drive hole with the shovel handle and when he bent over to shovel, a slug of draw rock struck him in the back and slid down his right shoulder.

A roof fall occured in the old 2nd developement of the 002-0 section. The fall occured in the No. 6 intake entry at survey station no. 4868. The fall measured approx. 30' long x 20' wide x 8' thick and fell above the anchorage zone of the 48"" fully grouted bolts installed in the area. The fall occured approx. 3000' inby the active section.

THE EE STATED HE WAS PULLING LINE CURTAIN IN THE #3 HEADING ACROSS THE LAST OPEN CROSSCUT AFTER HE HAD BOLTED THE PLACE. AS HE WAS PULLING ON THE CURTAIN, HIS RIGHT FOOT SLIPPED FROM UNDER HIM AND HE FELL BACKWARD ONTO HIS LEFT FOOT.

Struck by falling object

Mechanics were installing a canopy on a shuttle car. The canopy was laying upside down; in order to orient it properly they had the scoop operator bring the scoop to it. The mechanic hooked a chain to the canopy & to the scoop's ram jack to pull the canopy up in the scoop bucket. The canopy caught on the lip of the bucket, swung around & the corner landed on the injured's foot.

The employee was getting onto a battery personnel carrier. He had put his right leg onto the carrier and when he was pushing his body onto the carrier with his left leg he felt something pop in his knee and it locked up.

A roof fall was discovered during a weekly examination. The fall occurred approx. 210' outby s.s. #507 in the #3 entry in an old panel. The fall measured approx. 50'long x 20' wide x 6' thick. The area had previously been bolted with 48"" fully grouted resin & cribs had been set in the area. The fall was composed of a mixture of sandstone and slate.

A roof fall occurred ont he 1st right panel off ""R"" section mains approx. 70' inby S.S. #4465 in the No.6 intake entry. the fall measured approx. 90'l x 5'-7'Thick x 18'w. the fall occurred in an area of known bad top and supplemental roof supports (10' cable bolts & cribs) had been installed int he area prior to the fall. the fall is approx. 3000' from the active 002-0 section.

A roof fall occurred on the 1st right panel off ""r"" section mains at S.S. #4487 in the No.4 neutral entry. the fall measured 40'L x 10'h x 18'w. the fall occurred in an area of known bad top and supplemental supports (10' cable bolts and cribs) had been installed in the area prior to the fall. the fall is located approx. 3200' outby the active 002-0 section.

Shoveling #5 belt and allegedly strained his back.

A ROOF FALL OCCURRED IN THE NO.1 RETURN ENTRY APPROXIMATELY 140' OUT OF THE FACE ON THE 002-0 ""R"" SECTION. THE FALL MEASURED APPROXIMATELY 100' LONG X 18' WIDE X 8' THICK. THE AREA WAS KNOWN TO HAVE SOME ADVERSE CONDITIONS AND THE AREA HAD BEEN NARROWED TO 16' WITH CRIBS ON ONE SIDE AND 10' CABLE BOLTS INSTALLED IN THE INTERSECTION.

Over-exertion in wielding or throwing objects

THE SECTION IS PILLARING AND THE EE HAD CUT SOME TIMBERS FOR A BREAKER LINE. AS HE WAS THROWING THE TIMBERS OUT OF THE SCOOP BUCKET ONTO THE RIB IN THE NO. 1 HEADING, HE PULLED A MUSCLE IN HIS LOWER BACK. THE EE STATED HE TWISTED HIS BODY WHEN HE THREW THE TIMBER. THE TIMBER MEASURED APPROX. 56"" LONG X 4"" SQUARE.

2 OTHER MEN HELPING EMPLOYEE JACK TRAM UP TO GET BACK ON THE TRACK WEH JACK BAR SLIPPED OUT OF THE JACK CAUSING EE TO TRIP AND FALL BETWEEN TRACK & RIB & THAT'S WHEN HE PULLED SOMETHING IN HIS NECK.

AFTER ARRIVING AT THE END OF THE TRACK, THE SECTION CREW LEFT THE TRACK VEHICLE AND MOUNTED THE RUBBER TIRED PERSONNEL CARRIER TO RIDE TO THE SECTION. AS THE PERSONNEL CARRIER WAS BEING BACKED OUT OF THE BREAK, THE EE TURNED HIS HEAD TO THE RIGHT TO LOOK BACK. HE STATED ABOUT THIS TIME, THE PERSONNEL CARRIER HIT THE COAL RIB & HIS NECK STARTED HURTING.

THE FALL IS LOCATED 1 BRK INBY AND 1 BRK TO THE LEFT OF SURVEY STATION NO 2316 AND MEASURED APPROX 75' LONG BY 18-20' WIDE BY 3-5' THICK. THE ROOF HAD BEEN BOLTED WITH 72"" TORQUE TENSION BOLTS AND FELL IN A SLIP FORM. ROOF IN THE AREA WAS LAMINATED SLATE AND SANDSTONE.

A ROOF FALL WAS DISCOVERED IN THE NO.3 ENTRY(NEUTRAL). THE ROOF FALL IS COMPOSED OF LAMINATED SHALE AND SANDSTONE. THE AREA HAD BEEN BOLTED WITH48""FULLY GROUTED ROOF BOLTS. CRIBS HAD BEEN SET AROUND THE FALL AREA PRIOR TO THE ROOF FALL. THEFALL MEASURED APPROX. 50'LONG X 20'WIDE X 6'THICK AND WILL NOT BE CLEANED UP. THE FALL IS LOCATED TWO BREAKS INBY S.S. #1120 AT STOPPING NO.163.

A ROOF FALL WAS DISCOVERED IN THE NO.3 ENTRY(NEUTRAL). THE FALL MEASURED APPROX. 20'LONG X 20'WIDE X 5'THICK AND FELL AT S.S. #571. THE FALL IS COMPOSED OF LAMINATED SHALE AND SANDSTONE. A SECTION OF THE HIGHWALL OVER THE NO.3 (INTAKE)ENTRY FELL ON AND CRUSHED THE CANOPY AT THE MOUTH OF THE PORTAL AND FELL BACK INTO THE ENTRY APPROX. 15 FEET. THE SECTION OF HIGHWALL THAT FELL MEASURED APPROX. 15-20 FEET LONG BY 15-20 FEET THICK BY 16-18 FEET WIDE. THE ROOF JUST INBY THE PORTAL HAD BEEN BOLTED WITH 48' & 60"" RESIN ROOF BOLTS.

THE FALL MEASURED APPROX. 40-50' LONG X 19' WIDE X 5' THICK AND IS LOCATED APPROX. 2100' FROM THE ACTIVE 003-0 SECTION. THE ROOF FELL ABOVE THE ANCHORAGE ZONE OF THE ROOF BOLTS AND CONSISTED OF LAMINATED SLATE AND SANDSTONE. THE FALL WILL NOT BE CLEANED UP.

THE FALL MEASURED APPROX. 60'LONG X 18'WIDE X 6'THICK ANDCONSISTED OF LAMINATED SHALE. THE ARE HAD PREVI-OUSLY BEEN ROOF BOLTED WITH 48"" AND 60"" GROUTED ROOF BOLTS.

THE INJURED AND ANOTHER EE WERE CHANGING A TRAM MOTOR ON A SHUTTLE CAR. THE NEW TRAM MOTOR WAS BROUGHT TO THE SHUTTLE CAR IN THE BUCKET OF A SCOOP AND THE EE WAS PUSHING ON THE TRAM MOTOR TO T URN IT AROUND WHEN HE FELT A SHARP PAIN IN HIS BACK. THE EE CONTINUED TO WORK HIS REGULAR JOB THRU 8-24-01 AND WENT TO A DR AFTER HIS REGULAR SHIFT DUE TO TO WORK HIS REGULAR JOB THRU 8/24/01

THE FALL MEASURED A PPROX. 20' LONG X 30' WIDE X 5.5-6' THICK AND FELL IN AN INTERSECTION. THE FALL DID NOT AFFECT VENTILATION AND DID NOT INVOLVE ANY EQUIPMENT OR PERSONNEL. IT WAS DISCOVERED BY THE MINE SUPT.

ROCK WAS LOCATED 1 BRK OUTBY SS NO. 65 & 1 BRK INBY SS NO. 59 FROM WEST VA PORTAL OF MINE.
