EMPLOYEE WAS ATTEMPTING TO REMOVE A PIPE PLUG FROM AN AIR LINE WITH A CRESCENT WRENCH. THE WRENCH SLIPPED OFF THE PLUG CAUSING THE EMPLOYEE TO FALL INTO THE PIPE HURTING HIS RIGHT SIDE.

A SERVICE CO (APPALACHIAN TIRE CO) HAD REPAIRED TWO TIRES FOR SLAB FORK COAL CO THEY WERE PICKED UP BY THE COMPANY UPON ARRIVING BACK AT PLANT INJURED EMPLOYEE WAS UNLOADING TIRES FROM PICKUP TRUCK EMPLOYEE WAS SETTING TIRE ON GROUND IT DROPPED ABOUT A FOOT TO THE GROUND AND EXPLODED ON IMPACT RIM STRUCK EMPLOYEE ACROSS BOTH LEGS KNOCKING HIM ABOUT 6FT
