EE STATED THAT HE WAS ROOF BOLTING WHEN HE SAT THE DRILL HEAD OF THE ROOF BOLTER ON HIS RIGHT FOOT RESULTING IN A CRUSHING INJURY TO THE MIDDLE TOE. THE SKIN WAS SEWN OVER THE VERY END OF HIS TOE AT THE NAIL. THE STEEL TOE ACTUALLY MASHED THE END OF THE TOE.

EE STATED THAT WHILE THE ROOF BOLTER WAS BEING TRAMMED AND HE WAS WALKING ALONG SIDE OF THE BOLTER HE PLACED HIS HAND ON TOP OF ATRS. THE ATRS CAME INTO CONTACT WITH THE ROOF AND PINCHING EE'S HAND. EE WAS TREATED FOR FRACTURED FINGER AND RETURNED TO WORK ON NEXT REGULAR SHIFT.

EE STATED THAT HE WAS OPERATING THE MINER WHEN A PIECE OF ROCK FELL FROM THE EDGED THE RIB AND STRUCK HIM IN THE RIGHT LEG. EE WAS X RAYED AND WAS TREATED FOR LACERATION AND RELEASED. HE CONTI NUED TO WORK UNTIL 12-13-99 WHEN THIS ACCIDENT BECAME A LOSS TIME.
