EE was welding on a portable tank in the welding shop with the tank sitting on a table, during welding there was a minor ignition that caused the tank to fall off the table striking the victim on the left arm.

While installing a sweat on hub on the loadout belt conveyor drive, the hub stopped moving on the gearbox shaft 1/8" shy of its desired location. The victim was struck by a glancing blow of a 4 lb. hammer in the mouth. The hammer struck the hub before striking victim in mouth.

Employee was prying a form loose. A slate bar slipped and hit employee in the nose, requiring two (2) stitches.

Removing gear from gear box of 56DA Crusher, employee ask another employee to pick remaining gear up; as employee picked up remaining gear, it flipped out of his hand and gear struck EE's left hand while he was holding other gear, cutting fingers. An x-ray was taken; no stitches required; wound was cleaned and bandaged; no additional medical attention required.
Employee was beating a rock out of scalping screen using a sledge hammer. While hitting the rock, his right hand on the handle of the sledge came in contact with another rock. He did not seek medical attention until November 17th. He was told he had a fractured finger on the right hand following an x-ray. No other medical attention was advised. Employee did not miss any work.

Reason unknown trucker climbed upon floats for dredge after they were loaded on a flat bed trailer. He either slipped, tripped or stepped off the float and fell approx 8 1/2 ft to ground striking his head and face on the ground. He was bleeding from nose & mouth also unconscious for approx 3 minutes then confused for 20 minutes.

Miner had tightened bolts in scalping screen and was using welder to weld nuts to keep them from loosening up. Slag came off of nut and fell down shirt, landing in navel area of abdomen causing burn.
Miner was welding on back plate for skirtboard of Plant Feed Belt when he began to have a headache, face began to tingle and pulse rate increased due to fumes from several welders being blown around due to frequent changes in the direction of the wind.

Moving an oxygen cylinder out of a hoisting cage, the second cylinder fell striking the left little finger of the employee.
Employee was cleaning up debris on the bottom floor. A ceramic lined chute was being removed 3 floors above. A 1/2" X 4" X 6" pc of ceramic fell from the chute into the machinery well, bouncing from side to side. The employee app 30' away from the barricaded area was struck on the rt ring finger.

EE & CO-WORKER WERE REMOVING SEAT FROM 410 BACK HOE & REPAIR TRANSMISSION. WHILE REMOVING SEAT HE FELT PAIN IN LOWER BACK.
EE AND ANOTHER EE WERE INSTALLING A CLEAN UP PUMP ON THE FLOOR INJ EE PICKED UP A 90DEG ALL AND FLANGE AND TURNED TO HAND IT TO OTHER EE AND THIS RESULTED IN EE PULLING ON MUSCLE IN HIS LOWER BACK.

EE WAS GREASING A BEARING ON THE UNIT TRAIN BELT, WHEN HE RAISED UP, HE HIT HIS HEAD ON A PIECE OF STEEL CAUSING A 1" TO 2" GASH TO THE LEFT SIDE OF HIS HEAD. THIS RESULTED IN EE RECEIVING TWO METAL STAPLES TO CLOSE THE CUT.

EMPLOYEE STATED WHEN HE WALKED TOWARDS HEAD TANK OVERFLOW VALVE TO ADJUST WATER HE FELL THROUGH GRADING INTO #2 CYCLONE SUMPWATER.
EMPLOYEE HAS AGREED TO A LUMP SUM SETTLEMENT ON HIS RETRAINING INCENTIVE BENEFITS FOR COAL WORKER'S PNEUMOCONIOSIS.

EMP WAS LIFTING A CAN OF COAL AND FELT A SHARP PAIN THROUGH HIS LEFT ARM AND CHEST.
EMP WAS LIFTING THE COUNTER WEIGHT ON THE #2 REFUSE BELT. EMP HAD 3 TON HOIST ATTACHED TO A HOLEIN BEAM. THE HOLE IN BEAM BROKE OUT AND CAUSED HIM TO FALL ABOUT 4'. LATER IN THE SHIFT, HE STAR TED HAVING PAIN IN BACK AND RT HIP AND LEG.
THEY WERE REMOVING BEARING HOUSING FOR THE AUGER SHAFT BY HAND AND WHEN THE BEARING HOUSING CLEARED THE STUD BOLTS, IT DROPPED DOWN AND CAUGHT HIS RIGHT RING ON A PIECE OF ANGLE WHICH WAS A PA RT OF THE SUPPORT FOR THE PLATFORM THEY WERE STANDING ON, RESULTING IN A CRUSHED FRACTURE AND PARTIAL AMPUTATION TO RIGHT RING FINGER DISTAL END AT FIRST KNUCKLE JOINT.
EE STATED THAT WHILE PUSHING COAL WITH DOZER, HE STEPPED UP ON STOCKPILE TO A HIGHER BENCH TO PUSH COAL. EE STATED WHEN HE BACKED UP HE BACKED UP TO FAR AND THE DOZER HIT ON THE LOWER BENCH CA USING PAIN IN LOWER RT BACK AREA.
PUSHING COAL UPWARDS WITH A BULLDOZER AND AS HE BACKED DOWN OFF THE COAL PILE HE STRUCK THE SOLID ROCK WALL OF THE COAL PIT. INJURED SAID WHEN HE HIT THE WALL IT CAUSED A WHIP LASH EFFECT AND HIS NECK POPPED AND HE HAD PAIN IN HIS NECK, SHOULDERS, ARMS AND DOWN HIS BACK. INJURED CONTINUED TO WORK THAT THIFT AND HIS NEXT TWO REGULARSHIFTS BEFORE MISSING WORK.
EMP STATES THAT WHILE SHOVELING COAL IN THE LOAD OUT TUNNEL HE FELT A PAIN IN HIS BACK.
EMPLOYEE WAS PUTTING A SPLICE ON A ROCK BELT, STANDING IN A BENT OVER POSITION AND HAD SEVERE PAIN IN HIS BACK.

THE ALLEGED ACCIDENT OCCURRED IN THE UNIT TRAIN LOADOUT ON THE CONTROL ROOM LEVEL STAIRS TO THE NEXT LEVEL. THEY WERE MOVING A 55 GALLON DRUM OF OIL UP THE STEPS. THE DRUM WS BEING MOVED BY A HOIST AND CHOKER. THE CHOKER BROKE CAUSING THE DRUM TO SLIDE DOWN THE STEPS AND HE ATTEMPTED TO ELUDE THIS DRUM WHEN IT CAME TO REST ON HIS LEFT LEG.
CLEARING BRUSH 5ND TREES FRO ADVANCEMENT OF IMPOUNDMENT. WAS CUTTING TREE LIMBS WITH CHAIN SAW. TURNED TO CUT ANOTHER LIMB AND AS HE TURNED A SMALL LIMB WENT IN UNDER HIS GLASES AND SCRATCHED HIS EYE 3
