The operator was accessing the cab of a CAT 633D Scraper. With the door open, the operator began to enter the cab when a gust of wind caused the cab door to close. The door impacted the operators back. More than 2 hours later, the operator reported EE felt some pain in EE's groin region.

Employee was utilizing a 12# sledgehammer to tighten a hammer union seal. Employee struck seal 2-3 times and then experienced pain in EE's right forearm and bicep.

Employee was on platform of 980 wheel loader cleaning the front window when EE slipped and lost EE's grip on the hand-hold. EE fell over the side, hitting the wheel well and then to ground. The wiper dispenser wasn't working well. The platform footing may have had dew causing the slip but not identified at time of inspection. Employee later complained that EE's back was sore.

A slow leak of a reagent tote spilled to ground while the plant was closed. The vapors/fumes were blown in the direction of the operator by the wind which was inhaled by the operator and upset EE's stomach. Later, the operator felt additional symptoms (headache, nausea) and vomited. Diagnosis was chemical inhalation irritation.

Employee was setting up a 10" by 8' plastic pipe (35-40 lbs.) under a loadout bin. As they started to lift the pipe, the felt a pain in right shoulder and set the pipe back down. Employee stated they didn't lift the pipe from the end and walk to a near vertical position as was common, but attempted to lift the pipe from the bottom 1/3 of length on opposite end. Reported 8/15/22.

Injured worker was holding an 8" bolt in left hand while another worker was striking head of bolt with a sledge. During a strike, the sledge missed bolt head and struck the injured workers left hand causing a fracture to the tip of employee's left ring finger.

The worker was walking down a slight incline on a hillside while inspecting a fenceline at an area away from all mining activity. While in stride, employee's lead foot (right) slipped on dry grass. As momentum carried employee forward, back foot caught on something and stayed in place causing a sprained left ankle.

Driver felt foggy and disoriented when truck finished being loaded. As EE drove away, felt hot and uncomfortable and stopped to get out for fresh air. As EE exited the truck feeling disoriented, stumbled from the step and fell to ground. A doctor said that dehydration and drop in blood pressure caused the disorientation resulting in the fall to ground and injury.
Plant Manager was in office when EE experienced chest pain. Weigh master called 911. Plant Manager was taken by ambulance to local hospital. Plant Manager never lost consciousness or needed CPR. Plant Manager underwent angioplasty procedure and received (2) stents to open artery blockages. Plant Manager was admitted to hospital and had overnight stay.

The worker saw what he believed to be a piece of slag on a steel support. While using a 4 lb sledge hammer to position an I-beam between 2 supports, he made contact with the side of one of the supports while hammering in the downward position. The area where he noticed slag was in fact a sharp edge in which he made contact with. Worker was wearing gloves at time of incident.

The operator opened a screen door in the screen house and as he was brushing the screen with a long-handled brush heard a loud noise and the screen door then dropped onto his right arm causing a contusion. The cable that is attached to the screen door to assist in opening the door broke from the counterweight causing the door to drop.

Climbing ladder to do maintenance on screen on our extec screen machine. He said he slipped and grabbed on to the hand rail. He came back to work for next day we requested a doctor visit before returning. He returned on 4/25/06, 4/26/06 and 4/27/06. Then went to a chiropactor and was told to stay off work until futher notice.

Ee began to lose his balance, coming down some stairs in screen house, between #2 & #3 screens. To regain balance, he grabbed the sides of the screens. In doing so, he jabbed his left thumb into a blunt edge of a piece of rectangular steel on #3 screen. Upon initial consultation ee thought he didn't need medical attention. After continued tenderness he was sent to comp dr 4/16.

EE USED 4 BY 4 PIECE OF WOOD AND PLACED IT ON HE JACK STAND, THAN PLACED THE CUTTING EDGE ON TOP OF IT. BECAUSE THE EDGE/ WOOD WAS NOT SECURED OR CONTROLLED, IT SLIPPED AND SHIFTED CATCHING THE EE'SFOREARM BETWEEN CUTTING EDGE AND PADDLE.

EE WAS ON A LADDER CUTTING BOLTS FROM BETWEEN 2 FLANGES ON A WATER LINE. ANOTHER EE IN THE AREA WAS TALKING TO HIM, WHEN HE TURNED TO SEE WHAT THEY WANTED, HE ACCIDENTLY MOVED THE TORCH ACROSS HIS LEFT FOREARM. EE WAS BARE-ARMED AND NOT WEARING ANY PROTECTIVE ARM COVERUINGS.

WHILE PULLING THIS HOSE HIS RIGHT HAND WAS BELOW THE NOZZLE WHERE A HOSE CLAMP WAS WITH AN ENTERED FLAP. AS HE PULLED AND TURNED THE FLAP FROM THE HOSE CLAMP LACERATED HIS RIGHT THUMB.

IN TRYING TO REGAIN HIS BALANCE HE BAGAN TO FALL AND ONCE REACHING THE CONCRETE HE STEPPED ON A TWO INCH WATER HOSETHAT WAS LYING NEAR THE BOTTOM OF THE STAIRS AND TWISTED HIS LEFT ANKLE WHEN HE FELL.

EE WAS DRIVING A SCRAPER, HE NOTICED THAT THE SEAT SEEMED TO LOOSE, CONTINUED TO DRIVER BUT AT A LOWER SPEED. HE HIT A POT HOLE IN THE HAUL ROAD AND THE FRAME WHICH SUPPORTS THE SEAT BROKE. WH EN IT BROKE IT SLAMMED DOWN UNDER HIS WEIGHT CAUSING A SUDDEN JAR TO HIS LOWER BACK. THE SEAT ALSO LEANED TOWARDS THE LEFT CAUSING THE EE S SHOULDER TO HIT THE WINDOW.

STABBING WITH THE SCREWDRIVER A SECOND TIME, THE RECOIL FROM THE STABBI NG CAUSED HIM TO LOSE HIS BALANCE AND HIT THE RADIATOR WITH HIS RIGHT ELBOW. DR. REPORT STATES THAT HE HAS OLECRANON BURSITIS WITH BONY SPUR.

EE WAS ATTEMPTING TO ALIGN THE FORK TO ENTER THE FORK SADDLE UNDER THE MAN LIFTING BASKET. HE WAS SLIDING THE FORK TO THE CENTER OF THE DYRAULIC MASK CYLINDER CATCHING LEFT RING FINGER BETWEEN THE FORK AND FORK CARRIAGE.
HE WAS WORKING INT HE MAINTENANCE SHOP. HEWAS PERFORMING A CUTTING TASK ON THE WELDING BENCH. HE WAS WEARING SAFETY GLASSES AND WELDING HOOD. DURING HIS WORK A PIECE OF SLAG WAS BLOWN UNDER HIS WELDING HOOD AND INTO HIS EYE. HE REPORTED THE INCIDENT THE NEXT DAY COMPLAINING OF DISCOMFORT 5-10-91. HE WAS IMMEDIATELY SENT TO THE CLINIC FOR AN EXAMINATION.

AFTER HE GOT HOME FROM WORK HE NOTICED SOMET HING WAS IRRITATING HIS RIGHT EYE. HE CAME TO WORK 12-7-89 AND THE COMPANY SENT HIM TO THE DOCTOR. HE HAD A PIECE OF METAL IN THE EYE.
WHILE DRIVING D9G TO THE OVERSIZE POND AREA TO PUSH SAND THE FUEL LINE BORKE SPRAYING FUEL IN THE OPERATOR'S AREA AND IGNITED OFF ELECTRICAL WIRES. THE OPERATOR SHIFTED THE D9G INTO NEUTRAL AN D JUMPED OFF THE OPERATOR PLATFORM. WHEN HE LANDED HE FELL ON HIS LEFT SHOULDER AND ARM.
I NAME WAS OPERATING THE WET PROCESS HE WAS CHECKING THE OVERSIZE PUMP PU-05 IT WASNT PUMPING RT HE WALKED UP T O LOOK AT IT IT STARTED TO MAKE A GRINDING NOISE & HE BACKED AWAY HE WAS ABOUT 1 0FT AWAY WHEN THE OUTSIDE CASE ON THE PUMP EXPLODED INTO PIECEWS BLOWING THE PUMP ABOUT 10FT FROM THE TANK & I NAME WAS HIT IN THE H IP WITH A PIECE OF THE PUMP

INJURED EMPLOYEE AND ANOTHER EMPLOYEE WERE REMOVING THE OLD SIZER FEED BOX, INJURED EMPLOYEE STEPPED INTO A HOLE OUT IN THE CAT WALK.

CLEANUP PUMP WAS PLUGGED WITH SLUDGE.EMPL TURNED OFF PUMP AND USED HOSE TO CLEAN OUT PUMP.EMPL TURNED PUMP BY HAND TO FREE SLUDGE.
EMPL LIFTED WATCH GLASS OFF SOLUTION-DRIPPED INTO CUT ON LEFT THUMB.
