EE HAD COMPLETED THE CLEANING OF BATTERY TERMINALS ON DOZER CO.#660. HE WAS PLACING THE RUBBER INSULATION MAT ON TOP OF THE BATTERY WHEN APPERENTLY THE BATTERY GROUNDED OUT AND EXPLODED. THE E E WAS STRUCK IN THE EYES WITH BATTERY ACID.
EE WAS REPLACING SPROCKET SEGMENTS ON A BULL DOZER. ONE OF THE BOLTS WAS EXTREMELY TIGHT SO HE WAS USING A HAMMER TO LOOSEN IT. A PIECE OF THE BOLT BROKE OFF AND STRUCK HIS ARM CAUSING A LACER ATION ON HIS FOREMAN THAT REQUIRED STITCHES.
EE WAS OPERATING A DOZER ON THE BACK FILL, WHILE TRYING TO LEVEL OUT THE DUMPING AREA, HE BACKED OVER SOME ROUGH ROCKS. THIS ACTION CAUSED THE DOZER TO JAR AND HE STARTED HAVING PAIN IN LOWER BACK.

EMPLOYEE WAS PAINTING A NEWLY INSTALLED HANDRAIL ON A CAT 777 ROCK TRUCK. HE WAS HOLDING ON THE RAIL WITH ONE HAND. HE TWISTED TO THE SIDE TO CONTINUE PAINTING. THIS MOTION CAUSED A BACK MUSCL E STRAIN.

EMPLOYEE WAS LIFTING A ROCK THAT WAS BETWEEN THE D9N DOZER TRACK AND COUNTERWEIGHT. AFTER LIFTING THE ROCK, HE FELT PAIN IN HIS LOWER BACK AND BEGAN MISSING TIME ON 10-6-94.
EMPLOYEE WAS REPLACING 2 BOTTOM BUCKET PINS ON A 992C LOADER. HE SPENT AN EXTENSIVE AMOUNT OF TIME CRAWING ON HIS KNEES. HE STARTED MISSING WORK DAYS ON 3/17/94 DUE TO FLUID ACCUMULATION ON HI S KNEES.

THIS INJURY ALLEGEDLY OCCURRED ON THE JOB IN JAN. OF 1990 AND ACCORDING TO THE EE HAS FLARED UP SEVERAL TIMES SINCE. ON THE DAY IN QUESTION HE WAS HELPING LOAD A 120# BARRELL OF GREASE ONTO TH E THIN SEAM MINER. THE LIFT WAS 4' HIGH. HE STATED THAT DURING THE LIFT HE AGGRAVATED THE OLD BACK INJURY.
EMPLYEE WAS MOVING A TANK OF NITROGEN BY PLACING HIS ARMS AROUDN THE TANK & SCOOTING THE TANK ABOUT A FOOT AT A TIME DURING ONE OF THE UPWARD LIFTS HE ROTATED HIS UPPER BACK & FELT PAIN IN HIS BACK

54736YEE WAS MAKING REPAIRS ON A CLUTCH ON THE THIN SEAM MINER. THE CLUTCH HOUSING WAS FILLED WITH SMOKE. HE USED COMPRESSED OXYGEN TO BLOW AWAY THE SMOKE. WHEN THE HOUSING FILLED WITH OXYGEN 10851T SOURCE CAUSED IT OT EXPLODE FILLING THE EMPLOYEE.
