While moving the vulcanizer out from under the conveyor belt, a retaining bolt moved and caught the tip of the belt tech's finger, injuring the finger which required medical attention.
Person Injured themselves when conduction maintenance work on a haul truck brake system. Person was lifting a brake disc and injured their back muscles. The injured person went to the ER when back pain was not controlled by over the counter pain medication. ER documented injury as strained back muscles and instructed person to take 4 days off work to recover.

Employee's were involved in adjusting a motor base to replace the motor drive belts, when the motor mount bolts broke and struck employee in the lower right leg.
While standing with one foot on a catwalk and the other on a hyrdaulic hose bracket the employees foot slipped. He slipped about 18-inches down and hit his ribs onto the hydraulic lines. Left side ribs (6th & 7th) suffered a non-displaced fracture.

EE left index finger was lacerated while trying to remove a plow bolt on the cutting edge of the WA-900 loader with a impact wrench. The spinning bolt cut into his finger. Durabond was applied.
EE sprained his left knee after stepping off a locomotive. He went to the doctor on June 23.
Employee's tooth was broken after the Derrick hoist control box struck him in the mouth while he was pulling on it.

WHILE OPERATING A LOCOMOTIVE, EMPLOYEE STRUCK HIS RIGHT ELBOW WHEN THE LOCOMOTIVE CAME TO A SUDDEN STOP. INITIALLY, THE INJURY REQUIRED FIRST AID TREATMENT ONLY. HOWEVER, HE CONTINUED TO HAVE PROBLEMS WITH HIS ELBOW. HE WAS REFERRED TO AN ORTHOPEDIC SPECIALIST WHO RECOMMENDED SURGERY WHICH OCCURRED 12/19/03. EMPLOYEE RETURNED 1/5/04 AND RECEIVED FINAL RELEASE FROM DOCTOR 2/12/04.

THE ARK HOLDER STILL HAD ABOUT THREE FOURTH'S OF THE USED ROD IN IT. THE HOT ROD LAID AGAINST HIS LEG BURNING A HOLE IN THE RIGHT SIDE BETWEEN THE KNEE AND ANKLE.

WAS MOVING FROM ONE RAIL CAR (GONDOLA) TO ANOTHER WHEN THE CARS MOVED AND HE FELL TO THE TRACK BELOW.

EMPLOYEE WAS USIGN BAR TO PRY RAIL CAR DOOR CLOSED. THE BAR SLIPPED OUT OF LATCH, EMPLOYEE LOST HIS BALANCE AND FELL TO THE GROUND INJURING HIS BACK.
EE AND HELPER WERE TRYING TO CLOSE A RAILROAD CAR DOOR THAT HAD A DEFECTIVE LATCH. THE HELPER CLOSED THE DOOR BEFORE THE INJURED COULD SAFELY POSWITION THE LATCH. EE'S LEFT MIDDLE FINGER WAS C AUGHT BETWEEN THE LATCH AND THE DOOR AS IT CLOSED. EE'S FINGER WAS LACERATED AND MASHED, REQUIRING 5 STITCHES.

DRILL HELP ER LOWERED ROATION HEAD AS HE DID HYD CYLINDER ROD THAT MOVES HEAD CAUGHT RIGHT HAND BETWEEN EXINDER ROD AND TOWER SUPPORT THAT GUIDES LIFT CHAINS THAT CONNECT HYD CYLINDER TO HEAD.
KENNEY DANIELS WAS REPORT TO WORK. TO ENTER THE OFFICE FROM THE PARKING AREA HE MUST GO UP ONE SET OF STAIRS ACROSS TRUCK SCALE AND DOWN A SECOND SET OF CONCRETE STAIRS. THIS SECOND SET HAD ICE ON THE SECOND STEP OF A 3 STEP CAUSING HIM TO FALL ON HIS LEFT SIDE.
