Employee pulled hose up steep hill, and was coming back down when his feet slipped and he fell 15 ft down and then tumbled another 15 ft down.
Employee was working on road with a d-9 dozer when a rock rolled out of a berm and he backed over it jarring his lower back. Employee didn't go to doctor until 9/27/2011
Employee was putting batteries into a 993k loader when he twisted his knee the wrong way causing his knee to swell and cause pain when he bends it. He didn't go to the doctor until 08/15/2011
Employee was loading a rock truck with a 993 loader when the bucket of the loader dropped hard on a rock and jarred his middle back.
Employee was removing a final drive to the ground when the chain came loose and hit employee in the mouth.
Employee was working underneath 993 loader when he inadvertently raised up and cut his head on the bottom of the loader.

Employee was climbing ladder to get on grader when his right knee gave out.
