Operator was drilling a hole with a TD230 bolting machine. A piece of drawrock fell onto the ATRS, when he lowered ATRS, the rock slid off the machine, scratching his back. Also, the drill steel fell against his neck, buring his neck.

Employee had bent a roof bolt. While using the head of the bolter to push bolt in hole, the bolt slipped off and hit him on the right side of his face. Employee was sent to the ER for treatment. He received one stitch to the right side of his face and returned to work.
EE STEPPED ON A NAIL THAT WAS DROVE THROUGH A BIT BUCKET LID. PUNCTURE WOUND TO LEFT FOOT. EE WENT TO DR AFTER HE LEFT WORK. OFF WORK 4 DAYS. RETURN 4-16-04 FULL DUTY.

A ROCKFALL WAS FOUND ON THE #2 MAIN IN THE #7 ENTRY AT SPAD #179. THE FALL WAS 60 TO 65 FEET LONG, 18 FEET WIDE AND 4-5 FEET THICK. THERE WAS NO EQUIPMENT INVOLVED IN THE ROCKFALL.
