Struck against stationary object

Injured was using a wrench to tighten a bolt on a crusher. The wrench slipped off and injured's right palm struck the bolt head. The resulting laceration required 6 stitches.

Over-exertion (Not Elsewhere Classified)

Employee reported on 7/14/2021 that EE's back was hurting and had been bothering EE since the week of 6/28/2021, when EE was operating various pieces of equipment. Employee stated EE's back hurt from climbing in and out of equipment. Employee received medical treatment on 7/14/2021 was released to full duty and again on 7/27/2021 when EE was given work restrictions.

Fall from machine

Employee stated that while mounting a haul truck, employee bent over to pick up the coffee that employee had placed on the bumper, lost balance, and fell off the truck.

Over-exertion in pulling or pushing objects

The employee states EE closed hopper inspection door on sweeper truck when EE felt a pain in EE's elbow. Reportable due to medical treatment on 4.28.2022.

Struck by flying object

Injured states ""I forgot my knife so blaster was throwing their closed knife at pillar next to me which hit and opened on impact then bounce (sic) and stabbed (sic) me in the arm."" 3 stitches required.

Struck against a moving object

Employee stated that while operating a loader, the bucket struck a toe on the floor and caused elbow to be stuck between the arm rest and the machine controls, resulting in elbow pain. No medical treatment until 04/12/2021.

Fall to the walkway or working surface

Employee was walking towards EE's vehicle because the mine shut down due to snowy weather conditions. EE slipped and fell on the snow in the parking lot near EE's vehicle. Physical therapy recommended to start on Feb 15.
