Accident type, without injuries

While cleaning rock from expansion joints in shaft a rock struck wire on the buffers knocking it loose disabling the elevator.

Crews 1&2 were riding elevator when car stopped at landing. Crew 1 exited car sending crew 2 to landing. Pushed down button that sent car into over travel locking doors. Crews were removed from car at 8:00am.

Slope hoist became inoperable at 1:30pm due to a failed solenoid on the hydraulic system. Repairs were made and operating at 4:00pm.

While bringing out a man lift from the underground mine up the inclined slope the functions of the lift failed and could not be moved blocking the travel way. Foot traffic around the stalled machine was still available.

The main hoist went down at 6:23 am on May 12, 2016. The hoist was down about 15 minutes. At 6:35 am it was shut down to check the brushes. At 7:18 am it was determined the motor had to be replaced. The hoist service was restored at 4:55 pm.

At approximately 10:50am an explosion occurred in the #1 Kiln coal system.

Absorption of radiations, caustics, toxic and noxious substances

The employee was in the process of opening the Draco Inspection Door for the #5 Hopper in the #5 Bag House. Once the door was opened material came out of the hopper onto the employee who was positioned below and off to the side of the hopper. The material, Envirolime, was hot and caused injuries to the employees neck, forearms, and eyes.

Over-exertion in lifting objects

Employee was loading roof bolts onto roof bolter and strained right shoulder. Missing work due to prescribed narcotic medication.

Fall from machine

Contract truck driver (Evergreen) tripped on bottom step descending from loadout.

Injured had removed a 3' section of DCR screw conveyor cover in an attempt un-jam it. To clear material, EE ran the screw in reverse. The employee then walked to the screw to observe progress when right glove was snagged by the end of the screw flight and pulled in causing injury.

Fall to the walkway or working surface

Employee was walking on the dock / stone pile area placing signs for the boat delivery when EE fell on top of the stone which caused a contusion to shoulder. Injured requested medical treatment on 5/09/2016 when condition didn't improve.

Fall down stairs

Raking kiln, stumbled stepping down a step, landed on wrists.

Fall onto or against objects

Employee was climbing up the steps of a 771 D truck to get back in the cab and EE's foot slipped on the bumper step. When EE's foot slipped EE's right inner thigh was cut by the hanger bracket for the steps.

Employee suffered a laceration to right thumb while replacing buckets on a hydrated lime elevator. They pinched their finger between the bucket and chain as they tightened the nut with a pneumatic driver.

Employee was walking along the rail tracks in the lime load out area and slipped on wet pavement. Employee didn't fall completely to the ground but sprained leg.

Employee slipped on the wet walkway hitting left knee on the walkway as EE was attempting to close the lid on the kiln 3 reclaim hopper.

Injured was attempting to unplug a stone chute with a bar when EE pinched finger causing fracture.

Struck by flying object

Employee was putting together a rail on a DM45 drill. EE was hitting a pin with a hammer and a piece of metal chipped off and hit left hand. It punctured the palm of hand.

Employee was stepping from barge onto tugboat and caught right hand thumb in U-bolt welded to the pole on toe knee causing a fracture.

While working on thermo couple on #1 cooler, lime dust entered employee's eyes. Eyes were flushed having 1 piece of dust having to be removed by physician.

Struck against stationary object

Employee was retrieving parts located in the storage building outside the shop. While EE was walking back to the truck EE was observing how the parts fit together and noticed there was a skid lying next to the metal dumpster. Focusing on going around the skid employee came in contact with a piece of metal grating sticking out of the metal dumpster cutting EE's lip.

The employee was cutting a metal plate with a hand grinder equipped with a cutting wheel to get it to fit. There was some pressure on the plate that caused the grinder to kick back and when it did, the rotating cutting wheel hit ee left forearm and cut it.

Employee was cutting zip tie when knife slipped. EE suffered a minor cut on left index finger.

Fall onto or against objects

Employee was throwing a rail switch and slipped. EE then fell, face first, into the handle and suffered a laceration requiring two stitches.

Fall down stairs

Customer truck driver was seen hanging onto handrail at fall protection station near the Hydrate plant by plant employees. Once they arrived the driver was found laying on the ground. Driver experience,birthdate and SS# unkown.
