At 7:45am the main Innofuel storage bin doors were opened to investigate the cause of a CO2 system discharge. When doors were opened, smoke was observed and fire department was contacted. No injuries or damage occurred.

All incidents in 2020
At 8:20am a fire was detected on #5 conveyor belt in the cooler pit. FD was contacted immediately and fire was extinguished at approximately 9:15am. No injuries resulted from the fire.
At approximately 11:32am on Saturday 08/15/2020, BU cooler belt (5 belt) ignited. Plant personnel attempted to extinguish the fire using portable extinguishers and the GFD was contacted for assistance. Fire was extinguished at approximately 12:40pm. Damage to equipment was limited and no injuries resulted from the incident.
Power was lost to the slope hoist & 10 employees walked approx. 1400' up the slope to the surface. EE contacted MSHA once reaching the surface at 4:45 p.m. to report the power outage. Five employees exited the mine via the Escape Hoist. All personnel were evacuated by 5:30 p.m. Power to slope hoist, communication and ventilation were restored by 3:00 a.m.
At 2:15pm, smoke was observed coming from the area behind the coal tent. Upon further investigation, flames were observed coming from the scrap wood dumpster. The fire department was called at 2:20pm and arrived on site at 2:40pm. Fire was extinguished at 2:50pm.
Injured was attempting to remove hydrator discharge pipe to clear blockage. After opening pressure relief valve and loosening pipe union, residual pressure caused hydrate material to spray into EE's eyes.
While an employee was descending stairs at scalping screen, they tripped/twisted right ankle.
Due to inclement weather the previous evening, injured donned ice grippers prior to going to work. While stepping up a step on coal silo, gripper caught on the stair grating which caused IE to fall, catching self with right arm causing a strain. IE was given work restrictions on 1/6/2021.
Assistant Kiln Operator (injured) and Kiln Operator were removing the kiln gun from Kiln 1 after shooting. When attempting to remove the gun from the hood bracket and place it onto the storage rack, injured felt a pop in right shoulder.
Injured was walking north along kiln 1 at 3-4 pier when they noticed something in right eye. EE went to the shop and and washed out right eye. The following morning they still felt like there was something in the eye and called to report the incident.
Employee felt something enter left eye after exiting # 1 kiln's baghouse. EE had earlier been working putting bags in # 3 compartment and went outside to stand for a moment. After leaving the compartment, EE removed dust mask and glasses for cleaning. This is when EE felt something in left eye.
During the lowering of the loading spout at the Lime Truck Load out an employee had a piece of lime enter right eye. Employee was standing at ground level and waiting to inspect the spout when this accident happened.
Reportable hearing loss as a result of yearly audiometric testing.
An employee was seen at a medical facility on 12/11/19 for hearing loss & I received the report on 1/10/20. The findings from this visit are that within a reasonable degree of medical probability that this employee suffers from an occupational-related noise induced sensorineural hearing loss.
An employee was operating a 772 haul truck underground when EE said they felt ill. EE radioed in to the day shift supervisor EE did not feel well and was told to park the truck where EE was at two north crusher. Once arriving on surface employee was transported by local EMS to the hospital. EE was then admitted and a medical procedure was completed the following day.
