Struck against a moving object

At 09:35AM on 30 June employee called HR manager to notify he was not coming to work because he injured his back the day prior while operating a JLG. He made no mention of injury prior to this, so exact information is not known. He stated he went to the ER and received a note from his doctor not to go to work.

Caught in, under or between a moving and a stationary object

Victim was moving skid steer in tight quarters and pinned himself between skid steer and column

Fall to the walkway or working surface

Employee was walking from office to the warehouse when she slipped on ice and broke her ankle

Fall from ladders

EE had finished setting a handbrake from the railcar platform. During the transition from the end ladder to the side ladder his left foot slid across the foot rung causing him to lose his balance and grip on the ladder. He fell back 3ft and as he hit the ground he put his right hand out to break his fall. The employee suffered a broken wrist.

Struck by falling object

Employee was using a prybar and struck left index and middle finger.

Occurred prior to start of shift. Employee twisted and felt pain in back when coming into work.

Fall to the walkway or working surface

While walking to vehicle, employee slip and fractured his elbow.
