Accident type, without injuries

Residual grease laying in bottom of gear shroud. Although unknown at time of report, radiant heat from the kiln may have been the source of ignition. Minimal heat damage to a thermal coupler as a result of the incipient fire condition.

Fall to the walkway or working surface

Employee was tarping a trailer at Carmeuse-Grand River when they slipped and fell, striking their head on the ground. Employee sustained a small laceration above the right eye, a concussion and vertigo. Employee was transported to health center via ambulance.

Absorption of radiations, caustics, toxic and noxious substances

EE was trouble shooting a plugged lime handling elevator when lime from above fell causing lime dust to puff out and get in both eyes. Injury - Chemical Burn / Ophthalmologist treatment /Rx Eye drops.
