Accident type, without injuries

A fire ignited on the L-500 conveyor at approximately 3:10 am on October 4. The fire was the result of hot lime contacting the tail pulley area of the conveyor. The hot lime fell through a small floor opening next to a dust collection pipe. The fire was extinguished in approximately 40 minutes. No injuries occurred as a result of this fire.

Accident type, without injuries

Crusher hydraulic sys. failed & oil dripped onto the hot material & ignited. EE put out fire w/ fire extg. Fire would rekindle. Occurred several times, EE call fire dept, knocked heat out so oil not ignite again. Single fire did not burn for longer then 30 min. Report being filed becuase MSHA believes was a reportable fire. 7000-1 is not an admission that a reportable fire occurred

Accident type, without injuries

Kiln 4 was in the process of being shut down when the oil in the bull gear housing caught fire. The fire was fully contained within the gear housing and was extinguished in approximately 30 minutes.

Over-exertion in lifting objects

Employee was moving the railcar unloader and when the employee went to rehang the 60 pound railcar vibrator he heard a pop and had a sharp pain in his mid and lower back. Employee recieved medical treatment at local ER, was given Rx Drugs and work restrictions.

Fall from machine

DRIVER WAS COMING DOWN LADDER FROM TOP OF TRAILER, WEATHER WAS NOT AN ISSUE, MISSTEPPED AND FELL. WAS NOT WEARING FALL PROTECTION HARNESS.

Struck against stationary object

Employee was inspecting around the stone screen when he stepped on some large stones. The large stones rolled when he stepped on them causing his knee to twist. Rx Drugs, Knee Brace, & Modified Duty.

Injured was attempting to free a stuck feeder with a pipe wrench. Injured struck the pipe wrench with a hammer causing the wrench to bounce toward the employee and strike him below his right eye.

Over-exertion (Not Elsewhere Classified)

Employee was shoveling around the kiln hood when he felt pain/cramping in his lower back and legs. Employee stopped shoveling and went to ER where he was given Rx Drugs and was not allowed to come back to work for 2 days.

Struck by falling object

The maintanence crew was replacing an electric motor. The motor was lined up with two of the bolts and the motor was suspended approximately 2 inches from the base. The EE proceeded to reach between the motor and the base to line up the support bolts. As his hand was under the motor, the motor fell crushing three of his fingers. Rx Drugs, Stiches.

Over-exertion in lifting objects

EE was opening a 3 ft by 8 ft fiberglass top railcar lid. He raised the lid and it became stuck. The employee went around hitting the door with a hammer to break the ice free. He then went back to the lid & gave a hard jerk; the lid became stuck again. At that time the employee felt a twinge and pain in his lower back. Rx Pain Drugs and Modified Duty.

Injured was using an impact wrench, with a 4 inch socket, to remove a nut from the axle of the plant dump truck. The socket broke and struck the injured's left index finger, causing a laceration.

Injured was removing coal mill liners using a pneumatic impact wrench. The employee was holding a bolt between his thumb and index finger when he activated the wrench. The head of the bolt caused a laceration to his thumb and index finger.

Fall to the walkway or working surface

Injured was attempting to feed coal throught the Kiln 4 coal grates when he stepped into an opening twisting his knee. It occurred on 9/7 but medical attention was not requested until 9/20 when the condition did not improve.

Injured was washing stone fines in the boat house with a water hose. The employee shut off the ball valve on the sprayer causing pressure to build and the hose to pop free from the hose barb. When the hose popped free, one of the clamps caused a laceration to the palm of his hand requiring stitches.

Over-exertion in lifting objects

Employee was shoveling lime under conveyor by Robo Bagger-he felt a pull and twinge in lower back. On September 13, 2007 still has pain-Had MRI and found ruptured disc.

Injured was raking coating the previous day when he developed pain in his left elbow. Injured requested medical attention the following day when the condition didn't improve.

Fall to the walkway or working surface

Injured was walking down the stairs on top of #2 coal silo. When he reached the bottom of the stairs and turned the corner, he slipped on a patch of ice causing him to fall. When he fell, he landed awkwardly on his right wrist.

Struck by flying object

Injured was preparing to weld anchors to the kiln shell and was cleaning dust from the area. When the employee cleaned the dust from the area, airborne dust entered his right eye.

Contact with hot objects or substances

EE was cutting bolts on a screen deck using a hand grinder. The sparks created by the grinder where hitting his leg & one of them burned thru his jeans & burned his shin. The burn was small and had a small blister. The blister broke and he got an infection in it so he went to the clinic and was given an antibiotic.

Struck by falling object

Digging in skid loader, large rock fell striking employee on both lower legs: right contusion, left ankle laceration.

Injured was hitting a storage tank with a hammer to get it to flow. When the material inside broke free, a large amount of dust was generated. The airborne dust entered the injured's left eye.

Employee was trying to unhook chain from #4 kiln scale discharge chute. Cable was tight & stuck on the hook, when employee puled on cable, cable came loose & struck employee in the lower lip. Employee had minor laceration to lip the required 2 sutures. minor bleeding, no ambulance intervention needed and No loss time event.

Absorption of radiations, caustics, toxic and noxious substances

Injured was taking tank measurments on 6 tank when lime entered his eye. He initially rinsed his eye, but the condition did not improve and he requested medical attention.

Injured was attempting to clean frozen material from under a conveyor belt with a bar. Injured was holding the bar, while another employee struck it with a sledge hammer. The employee swinging the hammer missed and hit the injured's finger.

While cutting a wire tie with a utility knife, the knife slipped and cut the employee's left thumb.
