Employee was leaving work for the day. They got into their own personal vehicle which was on the mine site, a bee followed them in at the same time, stung them on the left cheek. Employee is allergic to bees, did not have an EpiPen. Employee drove to Emergency Room for treatment. Treated and released with prescription, no work next day. Returned to work 7-29-2021.

Employee was climbing down from Tractor 1807 seat, employee's foot was not in the step all the way, foot slipped out and employee hit the ground, rolling ankle. Medical doctor diagnosed right ankle fracture, off work until reevaluated. Lost time injury.
Employee was hurrying to get out of the rain as heavy rain was coming down during severe thunderstorm. In haste, they got their left middle finger caught between door and door frame of forklift as door slammed shut, causing laceration requiring sutures. No lost time, no lost wages.
Employee was setting up saw at quarry, walked around the end of saw tracks which were close to edge of granite block, EE tripped over the saw tracks and fell, hitting EE's head on sharp edge of stone. Has a laceration above left eye which required sutures, some facial scrapes and a fractured nose.
Employee had hearing test. It was determined employee has a Standard Threshold Shift (STS) in the left ear.
