Injured was moving set of ""L-frames"" with overhead crane. The frames came together and hand got caught between.

Over-exertion in lifting objects

Improper lifting procedure to recover fallen handle held stone breaker. Pulled muscle in hip. 2-day lifting restrictions of 15 pounds.

Fall to the walkway or working surface

Employee was loading drill in quarry, walked around platform and slipped on snow covered ice, fell and hurt right shoulder and arm. EE didn't see doctor right away but pain continued to bother so ee went to doctor on 1-13-2017. Recommended restrictions of lighter duty - 10# lifting, pushing, pulling limit. Site able to accommodate - No Lost Time.

Over-exertion in pulling or pushing objects

Employee was helping a vendor (Austin Septic) pump out water tank, built up with silt. EE was pulling on the suction hose & felt rib pop. EE went to the chiropractor to get adjustment, x-ray taken, no fracture. No lost time.

Caught in, under or between a moving and a stationary object

Right thumb was caught between table and piece of iron while trimming to size.
