Over-exertion in pulling or pushing objects

Employee was drilling holes in granite. The drill steel puller was not working correctly. Employee was pulling steel out by hand. A couple days later, on Sunday, Employee informed Supervisor that EE's right shoulder was hurting. EE sought medical advice, diagnosed Right AC Joint Sprain. At this time EE is unable to work until 2-24-2022, pending evaluation. Lost time injury.

Struck against stationary object

Employee was pulling a hydro bag out of its storage rack. Employee's left hand lost grip and slid across the far edge of the hydro bag, causing laceration on lower left wrist. Approximately 15 stitches were applied. Is referred to hand surgeon due to tendon damage. Lost time injury.

Struck by falling object

After breaking stone, employee walked in behind standing roughback to retrieve a wedge. They started walking out and roughback opened up and fell towards them, trapping their lower right leg/ankle underneath. Treated and released to modified duty, no lost time.

Struck against stationary object

Employee kind of hopped/stepped across an area. When employee's right foot hit the ground, employee fell to the ground with pain. Doctor diagnosed as right heel muscle bruise from bone spur. Employee treated & released. Needs to use crutches as needed. Employee is working modified duty at this time, follow up appointment 06/15/2022. As of now, no lost time, no lost wages.

Struck by falling object

Employee was installing metal pins on excavator bucket, left hand was on bucket, pin was resting on piece of wood. Removed wood piece, pin dropped unexpectedly, crushing tip of left middle finger. Medical treatment sought, sutures applied, returned to work. No lost time, no lost wages.

Struck by flying object

Employee was pulling hose off dust collector to clean it, pulled off a little quickly and got some dust in right eye. Employee tried flushing it out for a long while, however went to eye clinic to get the remainder out. Received prescription for eye treatment. No lost time.

Caught in, under or between a moving and a stationary object

Employee was trying to set a 75# rock in between granite slab and the quarry wall. The rock slipped and right ring finger tip got caught between the rock and granite slab. Contusion to end of right ring finger. X-ray showed displaced fracture, pressure needed to be relieved from fingertip. Employee returned to work, no lost time.
