Failed to maintain 3 points of contact dismounting excavator on to uneven ground, twisting ankle resulting in minor soft tissue damage.

Caught in, under or between a moving and a stationary object

The miner as installing a belly pan on the D-6 dozier and inadvertently caught left hand in a pinch point. medical treatment stiches in left hand mid palm

Fall from machine

On 2/2/2020 the miner reported to the safety supervisor that the miner had fallen while dismounting dump truck on 1/31/2020. Miner stated miner mis-stepped one of the steps causing miner to fall to the ground at a height of approximately 3ft.

Struck against a moving object

A 745 CAT articulating truck tipped over in a curve road conditions were wet. No equipment damage. Employee received medical treatment for bump on head and knee.
