Roof fall approximately 25' long x 25' wide x 7' thick in #3 entry at spad no. 761. Fall was dangered off and additional roof support has been installed in area.

ROOF FALL OUTBY SPAD 2907 FROM LEFT WALL EXTENDING 3 1/2 CROSSCUTS. EIGHT TIMBERS WERE SET ON ALL SIDES OF. SPOT BOLTING AND ADDITIONAL ROOF SUPPORT HAS BEEN INSTALLED IN AND AROUND FALL AREA.

Accident type, without injuries

Roof fall on intake inby spad 2890 in #2 entry. Intake air was not blocked and no employees were injured. The area was dangered off and timbers were set.

Contact with electrical current

The victim was demonstrating the features of the fault wizard. During the demonstration in the mine office he received an electrical shock and collapsed.

Struck by falling object

Employee was bolting top when a piece of rock fell between last row of bolts and the atrs on the bolter stirking the victim on the head after breaking on the canopy.

EMPLOYEE WAS EXITING THE MANTRIP WHEN HE STEPPED OUT ON THE GROUND, HE FRACTURED TWO BONES IN HIS RIGHT ANKLE.

Caught in, under or between running or meshing objects

GLOVE CAUGHT BETWEEN STEEL AND DRILL HEAD. WHEN ROTATION PRESSURE WAS APPLIED, EMPLOYEE'S HAND AND ARM WERE TWISTED AROUND DRILL STEEL CAUSING COMPOUND FRACTURE OF LOWER ARM.

Over-exertion in lifting objects

EMPLOYEE WAS LOADING A BOX OF ROOF BOLT RESIN ONTO A SCOOP. AS HE LIFTED & THREW IT ONTO THE SCOOP HE FELT A SHARP PAIN IN HIS LOWER BACK.

Over-exertion in pulling or pushing objects

Employee was pulling bolt machine cable. As he leaned forward he felt pain in left knee. Had fluid removed from knee on 05-03-05 - became reportable injury.(HAD KNEE SURGERY ON 5-16-05 BECAME LOST TIME ON 5-17-05 BY MISC MAIL 5/53/05 #10). RTW per MISC MAIL 7/5/05 #6.

Fall to the walkway or working surface

EE was loading supplies from scoop bucket when his feet slipped and he fell. He has experienced pain in his lower back.

Struck against stationary object

Employee was cut on his right wrist by the end of a band of a bundle of roof bolts while picking up a bundle of glue lying behind the bundle of bolts. The laceration required 3 stitches.

EMPLOYEE WAS MAKING SPICE IN CABLE ON SHUTTLE CAR WHEN KNIFE SLIPPED CUTTING THUMB - NEEDED 2 STITCHES.

EMPLOYEE WAS STANDING IN SCOOP BUCKET RUNNING CENTERS. AS HE STARTED TO SPRAY THE TOP HE SLIPPED AND FELL TO THE GROUND. AS HE HIT THE GROUND HIS RIGHT LEG WAS UNDER THE SCOOP BUCKET. THE SCOOP OPERATOR LOWERED THE BUCKET TO ASSIST HIM NOT KNOWING HIS LEG WAS UNDER THE BUCKET.

EMPLOYEE WAS BOLTING TOP AS HE WAS PUSHING A BOLT INTO THE HOLE HE HIT THE ROTATION TWISTING HIS LT. LITTLE FINGER. THE FINGER WAS DISLOCATED.

Employee was lifting a back saver up with a slate bar, when the bar slipped and pinned his right forefinger against bottom and the bar.

Employee was reaching across the drill pot trying to bend the roof bolt for installation. As he leaned over he hit the rotation lever with his body causing drill chuck to twist his left hand.

Struck by falling object

Employee was drilling 10' test holes and extension fell out striking his left hand index finger causing a laceration.
