Accident type, without injuries

A unintentional roof fall occurred on the #11 belt at spad # 5935. The belt line was the only equipment involved in the incident. No injuries occurred. The fall measured 24' wide 28' long and 6' high.

Accident type, without injuries

A pillar fall occurred on the 003 MMU 60' in by SS#8673 in the #2 entry resulting in 2 MRS units being trapped. The fall occurred on pillar section in pillar line. Continuation of the pillar fall was the cause.

Fall from machine

Employee was climbing up the steps on the 980H loader. EE reached for the door handle, the door handle was slick from the rain causing hand to slip off the handle as the door opened. This caused ee to fall off the loader hurting back, shoulder, neck and arm.
