Struck against stationary object

EE was resting a 20 am motor on a handrail while still confected to the pully system, as employee tryed to move motor to the ground. The motor slipped off the rail pinching his hand in between the hand rail and the motor shaft.

Over-exertion in pulling or pushing objects

EMPLOYEE HURT HIS LEFT FOREARM WHILE HE WAS ALIGNING A CRUSHER FRAME, USING A PRY BAR TO SHIFT IT INTO PLACE TO ALIGN BOLT HOLES.

Fall from machine

Employee was exiting loader when he slipped and fell hurting his left leg.

EMPLOYEE WAS GETTING OUT OF THE WATER TRUCK WHEN THE DOOR STOP STRAP BROKE CAUSING HIM TO STEP DOWN BEFORE HE WAS READY. HE TWISTED HIS LEFT ANKLE AS A RESULT

EMPLOYEE SLIPPED WHILE CLIMBING DOWN STAIRS. FELL 1 STEP TO CATWALK AND BRUISED CHEST. RESTRICTED DUTY. * EMPLOYEE REPORTED INJURY ON 4/27/05 AND REQUESTED TO SEE DR. 4/28/05*

Struck against a moving object

DARK CONDITIONS BACKING UP TO DUMP, MISJUDGED TRUCK PLACEMENT & BACKED INTO SIX FOOT HIGH BERM & FLIPPED THE TRUCK OVER. EMPLOYEE FAILED TO WEAR HIS SEAT BELT & WAS INJURED AS A RESULT.

Fall onto or against objects

While installing a roller, employee slipped and fell hitting his lip on a steel cross over.
