Struck against stationary object

Employee was removing the cap screw on the filter housing of the HP 300 cone crusher using a 36"" aluminum pipe wrench. The wrench slipped off the nut causing the employees hand to strike a stationary object.

Fall down stairs

The operator of the base plant was standing on the guarded platform guiding the backhoe operator to place the rock breaker on the rock jam. The plant operator signaled the backhoe operator to swing to the left and it would not stop. The rock breaker struck the plant operator on the left cheek. EE fell down 4 stairs and injured.
